Amifostine protects vascularity and improves union in a model of irradiated mandibular fracture healing.

BACKGROUND: Pathologic fractures of the mandible can be devastating to cancer patients and are due in large part to the pernicious effects of irradiation on bone vascularity. The authors' aim was to ascertain whether amifostine, a radioprotective drug, will preserve vascularity and improve bone healing in a murine model of irradiated mandibular fracture repair. METHODS: Rats were randomized into three groups: nonirradiated fracture (n = 9), irradiation/fracture (n = 5), and amifostine/irradiation/fracture (n = 7). Animals in the irradiation groups underwent a human equivalent dose of radiatio

Mandibular fracture severity and patient health status are associated with postoperative inflammatory complications.

PURPOSE: To identify risk factors associated with postoperative inflammatory complications (POICs) after treatment of mandibular fractures. PATIENTS AND METHODS: The investigators designed a case-control study and enrolled a sample of patients treated for mandibular fractures at Massachusetts General Hospital between August 2004 and January 2010. Subjects who developed POICs after fracture management were categorized as cases. A POIC was defined as 1) recurrent swelling, fever, increased pain, or trismus; 2) wound dehiscence with purulent drainage; 3) exposed or infected hardware; 4) abscess …

Massive subcutaneous emphysema developing before surgery for mandibular fracture: a case report.

Preoperative massive subcutaneous emphysema before intubation is extremely rare. However, this complication may be potentially lethal, depend on the condition of air spreading. Subcutaneous emphysema which occurs intra- or postoperative period is sometimes iatrogenic because the air is introduced into the tissue space through the hole injured by the operation. But the emphysema in this case occurred preoperatively by the pressure of the bag valve mask, because the patient had an intra-oral wound, which reaches the submental space. In this report, we describe an extremely rare case of preoperat

Temporomandibular disorders due to improper surgical treatment of mandibular fracture: clinical report.

A case of mandibular fracture surgically consolidated in a wrong position resulting in craniomandibular disorders is reported. The inadequate surgical alignment of the healed bony segments caused a malocclusion. This changed the original neuromuscular system such that compensatory mechanisms began to change the whole balance of the organism. The patient presented a mandibular crossbite, an asymmetry of the face, and extensive alteration of muscular, articular, and postural function. The bony malunion and malocclusion were treated using an interdisciplinary surgical-orthodontic treatment for co

Radiographic features of vertically fractured endodontically treated mesial roots of mandibular molars.

OBJECTIVE: The purpose of this study was to evaluate the most frequent radiographic appearance of bony lesions around endodontically treated vertically fractured mesial roots of mandibular molars. STUDY DESIGN: For the study, 49 extracted mesial roots with vertical fractures (study group) were evaluated and compared to 52 extracted roots without fractures (control). RESULTS: The "halo" (36.7%) and "periodontal" (28.6%) type radiolucencies were the most typical appearances of periradicular areas around the mesial roots of mandibular molars with vertical root fractures. By itself, bifurcation

Osteomyelitis and pathological mandibular fracture related to a late implant failure: a clinical report.

This clinical report describes an unusual consequence of peri-implantitis and subsequent implant failure in a 72-year-old man. Following uneventful implant removal, osteomyelitis developed at the implant site, which subsequently weakened the mandible sufficiently for a pathological fracture to occur. The patient was treated successfully by use of an intensive oral hygiene regime, antibiotics, and conservative management. The consequences and management of implant failure are discussed, as well as issues relating to the management of osteomyelitis and pathological bone fracture.

Utilization of panoramic radiographs to evaluate short-term complications of mandibular fracture repair.

OBJECTIVES/HYPOTHESIS: Detection of complications after mandibular fracture repair is generally based on the clinical examination. However, … ns. No complications were diagnosed based on radiography. CONCLUSIONS: After surgical treatment of mandibular fractures, panoramic radiographs add little to the physical examination and history toward the detection of complications. We believe that panoramic radiographs need not be used routinely, but may be reserved for patients who have complaints or physical findi …

Mandibular Fracture Fixation Using Various Plate Designs: A Finite Element Analysis.

INTRODUCTION: Mandibular fractures are common maxillofacial injuries requiring stable fixation. Finite element analysis (FEA) offers an alternative method to evaluate stress distribution. This study was done to evaluate the biomechanical performance of different mandibular fracture fixation plate designs (single miniplate, dual miniplates, grid-type plate, and low-profile reconstruction plate) using FEA. MATERIALS AND METHODS: Total 100 mandibular models with standardized angle fractures were generated for FEA. All four plate types used for fracture treatment were assumed to be made of Ti-6Al…

Retrieval Analysis of Fractured Mandibular Reconstruction Plates Using Scanning Electron Microscopy.

Mandibular reconstruction plates are widely used following segmental mandibulectomy, but plate fracture remains a significant complication. This study presents a retrieval-based protocol that uses scanning electron microscopy (SEM) to characterize fracture morphology and to infer site-specific mechanical failure directions in titanium mandibular reconstruction plates. We hypothesized that fracture-surface orientation would differ by anatomical fracture location and would exhibit qualitative similarities to reference fractures produced under different loading directions. Seven patients with fra

Novel miniplate designs to enhance biomechanical stability in mandibular symphyseal fracture fixation.

In this article, the performance of different geometries of specially designed fixation miniplates for mandibular symphyseal fracture is evaluated by investigating the effect of geometrical parameters on the biomechanical stability of the fracture. Experimental cone beam computed tomography (CBCT) images were used to generate and simulate the mandible model. Two novel miniplate geometries, including rectangular and elliptical designs, are presented to achieve better biomechanical performance than conventional design. Performance of each geometric design is evaluated in six different dimensiona
