Patient-specific Subperiosteal Implant for Dental Rehabilitation of Severe Atrophic Maxilla: A Single Institution Series.

This retrospective study evaluated patient-specific subperiosteal implants (IPS) for dental rehabilitation in severely atrophic maxillae. Eight patients with Cawood and Howell class VI atrophy were treated between 2021 and 2025. Rehabilitation was planned with prosthodontists and bioengineers using virtual surgical planning, CAD/CAM technology, and selective laser melting to create custom titanium implants. All implants were placed successfully without major complications. Follow-up ranged from 2 to 20 months (mean: 8.6 y). No infections, implant mobility, or inflammation were observed. Four p…

Late implant loss and complications of one-step implant therapy in severely atrophic posterior maxilla.

BACKGROUND/PURPOSE: Successful long-term outcomes of one-step implant placement in the atrophic maxilla have rarely been reported. This retrospective study aimed to evaluate the long-term efficacy of the one-step lateral sinus floor elevation and simultaneous implant placement (LSFESI) in severely atrophic posterior maxilla, and to identify the risk factors for marginal bone loss (MBL) and late implant failure (LIF). MATERIALS AND METHODS: Clinical data were obtained from Taipei Tzu Chi Hospital. One-step, non-submerged LSFESI was performed at sites with a remaining bone height (RBH) < 5 mm a…

Comparative assessment of various implant configurations for atrophic maxilla: A three-dimensional finite element analysis.

OBJECTIVE: This study aimed to evaluate stress distributions in zygomatic, trans-sinus, and pterygoid implants and the surrounding atrophic maxillary alveolar bone using finite element analysis (FEA). METHODS: Three-dimensional (3D) FEA models with 10-mm conventional implants in the lateral incisor regions were designed. For posterior anchorage, 30° trans-sinus or 45° zygomatic implants (placed using the extrasinus technique) were positioned in the second premolar region, with or without 45° pterygoid implants in the second molar region. Accordingly, four models were created: (1) zygomatic, (…

Superficial Circumflex Iliac Artery Perforator Flap With Iliac Bone-An Alternative Option for Maxilla Reconstruction.

A superficial circumflex iliac perforator (SCIP) flap is gaining popularity in microsurgical free flap reconstruction, especially for extremity defects. However, its application in the head and neck region is still uncommon. Besides a thin, pliable skin paddle, the SCIP flap can be elevated with a part of the iliac bone. In this brief clinical study, the authors describe 3 different reconstructions of maxilla defects, ranging from small to very extensive, using an osteocutaneous SCIP flap. In their opinion, this flap provides many advantages, such as a relatively large bony element that is hig…

Management of a severely atrophic maxilla using concentrated platelet-rich fibrin block. A case report.

Tooth loss often leads to significant alveolar bone resorption, presenting a challenge for dental implant placement. This case report presents the effectiveness of concentrated platelet-rich fibrin (C-PRF) in combination with bone allograft for horizontal bone augmentation in a severely atrophic maxilla. A 33-year-old female patient with extensive bone loss in the upper anterior maxilla was treated in two stages. The initial stage involved horizontal bone augmentation using a mixture of C-PRF and bone allograft. This was followed, 5 months later, by dental implant placement. The preparation of…

Long-Term Comparative Outcomes of Short Implants Versus Maxillary Sinus Elevation in Posterior Maxilla Rehabilitation.

Background: Vertical atrophy of the maxilla has traditionally been treated with sinus lift procedures and implant placement, performed in one or two surgical stages. Subsequently, the transcrestal sinus lift technique was introduced, offering distinct advantages in terms of indications and reduced morbidity. Most recently, short implants have emerged as a valid alternative to these procedures, even in cases of severe horizontal resorption, allowing for direct placement in many cases. This study was designed to assess the clinical outcomes of short implant placement in alveolar ridges with seve…

Modified 3-Dimensional Alveolar Ridge Augmentation in the Anterior Maxilla: A Prospective Clinical Feasibility Study.

Vertical and horizontal reconstruction of the alveolar ridge, especially in the anterior maxilla, is considered a clinical challenge for dentists. There is still a lack of a standard technique to address the hurdles in 3-dimensional bone regeneration in the anterior maxilla. In this clinical feasibility study, we aimed to modify Khoury's technique by combining the conventional guided bone regeneration standards with the principles of this technique. The autogenous bone blocks were harvested from the retromolar area and grafted into the deficient anterior maxillae by mini-screws, and the gap wa…

Bone-level implants placed in the anterior maxilla: an open-label, single-arm observational study.

PURPOSE: This study assessed marginal bone remodeling and soft tissue esthetics after the loading of single bone-level implants in the anterior maxilla. METHODS: An open, single-arm observational clinical trial with 3 years of follow-up was performed, including 22 implants. The patients presented with a single tooth gap in the anterior maxilla (tooth positions 14-24), with natural or restored adjacent teeth. An implant was placed at least 8 weeks post-extraction and healed submerged for 6 weeks. After the second-stage operation, a fixed provisional prosthesis was provided. The final restorati…

Immediate loading of single-tooth implants in the anterior maxilla. Preliminary results after one year.

According to the standard protocol, a load-free healing period is one of the most emphasized requirements for implant integration. Recent studies have encouraged a progressive shortening of the healing period for single-tooth implants and immediate loading has been proposed for the aesthetic zone in the maxilla. The present study evaluated clinical outcomes of immediately loaded FRIALIT-2 Synchro implants 12 months after placement in the maxillary incisal region. In the course of our investigation, nine patients have been treated following an immediate loading protocol. The stepped-screw type …

Primary tuberculous osteomyelitis of the maxilla mimicking a refractory post-endodontic infection: A diagnostic challenge.

Primary extrapulmonary tuberculosis involving the maxillofacial region is rare and often poses a diagnostic challenge because of its atypical presentation and the absence of pulmonary involvement. We report a case of a 38-year-old woman who developed rapidly progressive osteomyelitis of the maxilla following elective root canal treatment. The lesion was unresponsive to empirical antibiotic therapy and standard dental intervention. Cone-beam computed tomography revealed extensive osteolytic destruction, and histopathological examination demonstrated granulomatous inflammation with Langhans gian…
