Three-dimensional Analysis of Skeletal Stability in Cleft Lip and Palate Patients Undergoing Bimaxillary Surgery With Le Fort I Osteotomy and Intraoral Vertical Ramus Osteotomy.

Orthognathic surgery in patients with lip and palate clefts is challenging owing to scar tissue from primary repairs and severe deformities. In this study, we evaluated the stability of Le Fort I osteotomy with intraoral vertical ramus osteotomy (IVRO) in patients with cleft lip and palate using 3-dimensional (3D) analysis. This retrospective study comprised 14 cleft lip and palate patients (3 females, 11 males; the average age at surgery: 23.8 y) who underwent bimaxillary orthognathic surgery involving Lefort I osteotomy and IVRO. Cone-beam computed tomography (CBCT) images were obtained at p

Risk Factors for Delayed Surgical Recovery and Massive Bleeding in Skull Base Surgery.

BACKGROUND: To determine factors that delay surgical recovery and increase intraoperative hemorrhage in skull base surgery. METHODS: Factors related to delayed postoperative recovery were retrospectively reviewed in 33 patients who underwent open skull base surgery. Early and late recovery phases were assessed as "days required to walk around the ward (DWW)" and "length of hospital stay (LHS)," respectively. Intraoperative blood loss was cal-culated every hour and analyzed in 4 steps, i.e., craniotomy and intracranial manipulation, cranial fossa osteotomy, extracranial osteotomy, and reconstr

Juvenile idiopathic/rheumatoid arthritis and orthognatic surgery without mandibular osteotomies in the remittent phase.

BACKGROUND: Orthognathic treatment of patients with juvenile idiopathic/rheumatoid arthritis is still controversial. Mandibular procedures may result in relapse and further condylar resorption and pain. Maxillary osteotomies and genioplasty may be effective for the correction of malocclusion and for aesthetics. PATIENTS AND METHODS: Five patients with juvenile idiopathic/rheumatoid arthritis with severe temporomandibular joint (TMJ) involvement underwent orthognathic surgery with a simultaneous Le Fort I osteotomy and advancement genioplasty. The age of the patients at the time of surgery ran

Correction of infraorbital and malar deficiency using costal osteochondral graft along with orthognathic surgery in Crouzon syndrome.

In syndromic craniosynostosis, such as Crouzon syndrome, midfacial hypoplasia can cause exophthalmos and concave facial profile. Though midfacial hypoplasia in Crouzon syndrome patients can be treated with midface advancement, known as a Le Fort II or Le Fort III osteotomy, such method can change nasal appearance and frequently fails to achieve class I occlusion after surgery. This report presents a case of an aesthetically and functionally successful midfacial augmentation using rib and cartilage graft along with orthognathic surgery (Le fort I and bilateral sagittal split ramus osteotomy) fo

Simultaneous Le Fort I osteotomy and computer-guided implant placement.

BACKGROUND: Since 1977, osseointegrated implants have played a strong role in oral rehabilitation. Despite the large success of this technique for the edentulous maxilla and mandible with sufficient bone height, patients with atrophic jaws continue to be the challenging cases for an adequate reconstruction of aesthetics and functionality. Orthognathic surgical procedures have been developed to reposition the jaws and have been traditionally used in the dentate patient to correct a skeletal malocclusion; these procedures can be used on the edentulous patient to correct the discrepancies between

[Lateral mandibular deviations].

The mandibular laterodeviation is one of the most evident malformations of the face, because it alters the lower third of the face. Etiologically it can be classified into: Static laterodeviations caused by teeth; Static laterodeviations caused by skeleton change: by monolateral hypertrophy (condyle, condyle and neck of the condyle, half mandible hypertrophy); by monolateral hypertrophy (congenital pathological); Dinamic laterodeviations functional. The midline displacement with posterior monolateral cross bite is caused by width discrepancy between the upper and lower dental arch resulting in

A modified minimally invasive Le Fort I approach allowing four-point fixation osteosynthesis using lateral punctiform incisions.

Surgical techniques in orthognathic surgery have recently evolved and now tend towards soft-tissue-preserving approaches. The concept of minimally invasive Le Fort I osteotomy and down-fracture is interesting, as it preserves almost all paranasal and upper-lip musculature. It has already been described with a four-point fixation on the canine and zygomatic buttress pillars, depending on the mucosal incision length and plate type. This article proposes an alternative technique, respecting the concept of minimally invasive surgery, with a very small medial mucosal incision adding two lateral pun

Achieving Predictable Hydraulic Sinus Condensing Using an Innovative Functional Sinus Implant: A Case Report.

Inadequate alveolar bone height/volume and sinus anatomy can complicate the predictable placement of conventional implants and subsequent restoration in posterior maxillary sites. A variety of sinus lift and vertical bone height augmentation materials and techniques have been utilized for this purpose; however, generally they are clinically challenging, demand well-trained surgical proficiency, and require the use of multiple armamentaria. Simultaneously, clinicians must meticulously handle the sinus membrane to achieve elevation without perforation, attain successful bone grafting, and ensure

Facial asymmetry and the oral surgeon.

Facial asymmetry is often caused by deformities of the mandible or maxilla. Mandibular deformities are often pronounced, and various surgical procedures have been developed to treat such conditions. Deformities of the maxilla are similarly deviated from the normal. Although they are not always so noticeable as those of the mandible, they adversely affect function, esthetics and mental wellbeing. There are instances where both jaws are at fault and require treatment. More recently, osteotomies in the anterior of the maxilla or mandible have been performed with success. If the blood supply to th

Modified Le Fort I interpositional grafting of the severe atrophied maxilla-a retrospective study of 106 patients over 10 years.

OBJECTIVES: The objective of this study was to evaluate a modified Le Fort I interpositional grafting followed by dental implants for the rehabilitation of edentulous atrophied maxillae (Cawood classes IV and V). The surgical modification was a bilateral sinus floor augmentation prior to the osteotomy. This generated a closed recipient bed which allowed the use of particulated bone grafts (xenogenic bone mineral) and a reduced amount of autologous iliac bone grafts. MATERIALS AND METHODS: A total of 106 patients with maxillary interpositional bone grafts were included in this retrospective an

Accuracy of computer-simulated mandibular autorotation following single-jaw maxillary repositioning surgery using customized surgical cutting guides and fixation plates.

The literature on computer-aided surgical simulation (CASS) has focused extensively on the precision and accuracy of maxillary and mandibular repositioning surgery. However, the predictability of mandibular autorotation remains understudied. The aim of this study was to evaluate the accuracy of CASS-planned mandibular autorotation in 50 consecutive patients undergoing maxillary repositioning surgery with customized titanium surgical cutting guides and fixation plates. The study patients (32 male and 18 female, mean age 25 years for both sexes) underwent Le Fort I osteotomy, with or without sim
