We report on 133 patients with 138 subcapital metacarpal fractures who were treated between 1987-1989. The average duration of follow-up was 27.9 months. The indication for an operative treatment was given in only 4 cases. A good or very good clinical result was achieved in 94%. If the deviation of palmar angulation exceeded 15 degrees, the primary anatomical reposition remained stable in only few cases. A 23-year-old male recreational basketball player sustained an open ankle fracture and ipsilateral carpal and metacarpal fractures as a result of a fall while attempting a slam-dunk. The wrist and hand fractures were treated nonoperatively. The open ankle fracture required irrigation, debridement, and open reduction/internal fixation. In addition to the prerequisite leaping ability, long-term success in "playing above the rim" requires experience, exceptional physical agility, and the mental discipline to anticipate and avoid slam-dunk opportunities with high risk for personal injury.

Fractures of Metacarpal and Metatarsal II and IV (the splint bones) were treated in 283 horses over an 11 year period. In 21 cases the proximal portion of the fractured bone was stabilized with metallic implants. One or more cortical bone screws were used in 11 horses, and bone plates were applied in 11 horses. Most of the complications described in this paper can be prevented by strict adherence to well established principles of fracture care in the hand and careful attention to detail in the application of those principles. Some complications are unavoidable, however, and an attempt has been made here to offer some suggestions regarding the management of established complications following fractures of the phalanges and metacarpals.

INTRODUCTION: Trapeziometacarpal pre-arthrosis is a common condition with only limited therapeutic possibilities. Eaton and Littler designed a tendon stabilization.
MATERIALS AND METHODS: A series of 21 thumbs with painful carpometacarpal joints, treated with a ligament reconstruction according to Eaton and Littler, is described. Three failed and further surgery was required. The outcome of 18 is evaluated. There were 11 women, 7 men with a mean age of 33.7 years, all having stages I or II of osteoarthritis. The serratus anterior fascia was used as a free-tissue transfer in four patients for the reconstruction of dorsal hand defects. All patients had multiple open metacarpal fractures with extensor tendon injuries. The fascia was used to "sandwich" the extensor tendons in a bed of areolar gliding tissue to avoid adhesions. Fractures of the fifth metacarpal can be treated non-operatively by plaster immobilisation or functional taping. In order to evaluate the better treatment policy, the two methods were compared in a prospective randomised series. The hemicallotasis technique was used to treat the fifth metacarpal in a hand with congenital synostosis of the fourth and fifth metacarpals. This study was planned to search for the presence of possible tubular bone abnormalities in familial short stature (FSS) as has been previously noted in some reports.
