Observation or Otolaryngology Surveillance After Ventilation Tube Insertion in Children: The ConVenTu Noninferiority Randomized Clinical Trial.

IMPORTANCE: Middle-ear ventilation tube (VT) insertion is the most common ambulatory surgery in children. Follow-up for these patients may continue for 2 or more years and consume a great deal of health care resources. Research into the appropriate level of care for such controls is lacking. OBJECTIVE: To determine whether observation by general practitioners (GPs) yielded noninferior audiometric outcomes 2 years after VT insertion compared to otorhinolaryngology (ear, nose, and throat [ENT]) follow-up by specialist health care services. DESIGN, SETTING, AND PARTICIPANTS: In the multicenter

Should a pilot suffering from Ménière's disease be grounded or lifted off to the moon?

Recently, a pilot suffering from Ménière's disease was referred to our department. The decision concerning his ability to fly was made after an open discussion with him and his employer. He was allowed to fly during a period of remission. The relationship based on mutual trust proved to be wise since this pilot never tried to deny any recurrence of vertigo and spontaneously decided to obtain a waiver when vertigo reoccurred. In contrast, a law stating that pilots with Ménière's disease should be permanently grounded regardless of the treatment's result may lead to pilots flying with unconfesse

Sudden hearing loss in the elderly.

In elderly subjects suffering from presbycusis a clinical picture of sudden hearing loss, tinnitus and vertigo may be precipitated by a superimposed serous otitis media. Such an abrupt and dramatic sensory deficiency may be erroneously ascribed to an acute cochlear lesion and given grave prognostication. This situation was observed in 12 patients during a period of 2 years. Appropriate diagnostic evaluation revealed the true nature of the disease and simple therapeutic measures promptly restored the auditory function to its previous level.

Long-Term Preclinical Evaluation of a Permanent Stent Developed for the Human Eustachian Tube.

The Eustachian tube (ET) is a bottleneck when it comes to middle ear (ME) health. If its function is impaired, this can lead to serious consequences for the patient, such as hearing problems or deafness. Therefore, this study investigated a tapered nitinol stent (3-5 mm × 14 mm) for the human ET as a potential new permanent treatment for chronic Eustachian tube dysfunction (ETD) and thus ME ventilation disorders. The self-expanding stent was inserted unilaterally into the ET of 24 sheep with observation periods of 3, 6, and 12 months. Local tissue effects and the safety of the stent insertion

Imaging of the Eustachian tube and its function: a systematic review.

INTRODUCTION: The Eustachian tube is a complex and inaccessible structure, which maintains middle ear ventilation to facilitate transmission of sound from the tympanic membrane to the cochlea. A renewed interest in treatments for eustachian tube dysfunction has led to a demand for methods of imaging the Eustachian tube, and assessing tube opening non-invasively. This review aims to summarise the use of imaging in the anatomical assessment of the Eustachian tube, and to explore how radiological techniques can be used to assess tube function. METHODS: A systematic review of the literature was p…

The effect of tympanostomy tubes on otoacoustic emissions.

PURPOSE: Otoacoustic emissions (OAE) are a common screening tool to evaluate cochlear function. Middle ear dysfunction has been shown to impact results of otoacoustic emission testing, but there are limited data on the effect of tympanostomy tubes on OAE. The purpose of this study was to determine whether tympanostomy tube placement significantly improved OAE. METHODS: A retrospective review of charts was completed for patients younger than 18 years old who underwent tympanostomy tube placement from January 1, 2018 to September 1, 2023 and had preoperative and postoperative OAE testing within

Formula Otorrhea in Pediatric Patients With Cleft Palate.

The case series details 2 unusual cases of male newborns with cleft lip and palate (CLP) that later developed formula otorrhea. Both patients underwent bilateral myringotomies with the insertion of pressure equalizing (PE) tubes for chronic otitis media with effusion (OME). Chronic otorrhea associated with feeding occurred post-PE tube insertion and the otorrhea was later confirmed to be due to reflux of formula. Patients were treated with antibiotic ear drops, routine ear cleaning, anti-reflux medication, and reflux precautions. After definite cleft palate repair, formula otorrhea completely
