Social inequality in onset of mobility disability among older Danes: the mediation effect of social relations.

OBJECTIVE: This article investigates social inequality in onset of mobility disability and in measures of social relations and whether social relations mediated the effect of socioeconomic status on mobility. METHOD: A total of 2,825 nondisabled older men and women, enrolled in the Danish Intervention Study on Preventive Home Visits, constituted the study population. Data were obtained by mailed questionnaires in 1998-1999 and 2001-2002 and by merging analyses to registers at Statistics Denmark. RESULTS: There was a social gradient in onset of mobility disability, with odds ratio of 1.11 (1.

Tai Chi Chuan in the management of Parkinson's disease and Alzheimer's disease.

BACKGROUND: Parkinson's disease (PD) and Alzheimer's disease (ALZ) represent later-life onset neurodegenerative disorders that gradually rob those afflicted of their quality of life. PURPOSE: This chapter offers practice-based recommendations on how instruction and practice of Tai Chi Chuan (TCC) can be adapted for individuals with PD and those with ALZ. RESEARCH EVIDENCE: Practice of TCC is widely advocated as an exercise option in PD; however, little validating research exists. Even less is known about feasibility of applications of TCC for individuals with ALZ. CLINICAL IMPRESSIONS: The sl

A patient with worsening mobility: a diagnostic challenge.

Worsening mobility and "off legs" are common reasons to seek a medical opinion. A 62-year-old gentleman, previously fit and healthy, was referred to a district general hospital in the UK with worsening mobility. The initial clinical features were suggestive of Miller Fisher syndrome (MFS). Initial investigations were unremarkable apart from mild hypokalemia. Despite correction of the hypokalemia and immunoglobulin infusion, the patient's condition deteriorated. The presence of hypokalemia raised the possibility of underlying malignancy with ectopic ACTH secretion. A CT of the chest/abdomen sho

Mapping the adoption trajectory, mobility independence, and social participation-wellbeing relationship of assistive technologies for persons with disabilities: a systematic literature review towards a conceptual framework.

PURPOSE: This study analyse the adoption trajectory of assistive technology (AT) and its impacts on mobility independence and social participation-well-being among persons with disabilities. MATERIALS AND METHODS: Following the PRISMA 2020 guideline, 80 empirical articles (2015-2025) from Scopus and Web of Science were synthesised using a three-dimensional scoring system: (1) the adoption trajectory, (2) mobility independence and (3) social participation-well-being. RESULTS: AT adoption correlates strongly with functional mobility (ρ = 0.70), but only moderately with well-being (ρ = 0.58). T

Association of gut microbiome with mobility impairment in the Hispanic Community Health Study/Study of Latinos.

Aging-related declines in mobility are more common in women than men. The gut microbiome may play a role in physical function, but sex-specific roles are unknown. In adults ≥50 years old (n = 1187 women, 585 men), we examined associations of self-reported mobility impairment with gut microbiome assessed by stool shotgun sequencing, and examined heterogeneity by sex. Gut microbiome α-diversity was lower and overall composition (β-diversity) altered in women with mobility impairment compared to without, but this was not the case in men. Fifteen microbiome species were associated with mobility im

Physical activity positively impacts disability outcomes during transition from midlife to early older age irrespective of body mass index.

We examined the effects of physical activity (PA) and body mass index (BMI) longitudinal patterns (trajectories) on subjective measures of mobility, function, and disability in adults and assessed whether effects of PA trajectories on function varied due to BMI. Group-based trajectory analyses were used to determine patterns of change in PA and BMI using data from the Health and Retirement Study 1931-1941 birth cohort (n = 10,507). Physical function was assessed by Mobility Limitations (0-5 scale) and Large Muscle Function (0-4 scale) Indexes, as well as with score for activities of daily livi…

Validity of the 30-Second Sit-to-Stand test as a measure of lower extremity function in persons with multiple sclerosis: Preliminary evidence.

BACKGROUND: The 30-Second Sit-To-Stand (30SSTS) is a quick, inexpensive, safe, and widely used clinical measure of lower extremity function. To date, there is limited evidence regarding the use of 30SSTS in multiple sclerosis (MS). The purpose of this study was to examine the construct validity of the 30SSTS test in persons with MS compared with non-MS healthy controls. METHODS: Twenty ambulatory persons with MS and twenty age- and sex-matched healthy controls completed the 30SSTS, Timed 25-Foot Walk (T25FW), Timed Up and Go (TUG), Six-Minute Walk (6MW), and Godin Leisure-Time Exercise Questi

Motor and Physical Function Impairments as Contributors to Slow Gait Speed and Mobility Difficulty in Middle-Aged and Older Adults.

BACKGROUND: Motor function affects ability to perform daily activities and maintain independence. Yet, the interrelatedness of upper and lower extremity motor impairments and the magnitude of their contribution to slow gait and mobility difficulty are not well investigated. METHODS: Participants in the Baltimore Longitudinal Study of Aging (N = 728, aged 50-99) completed motor and physical function tests including grip and knee extension strength, pegboard, finger tapping, standing balance, chair stands, fast-paced 400-m walk, and usual gait speed. Slow gait was defined as usual gait speed <1

Prevalence of US Adults With Cardiovascular Disease Reporting Barriers to Clinician Office Visits: IMPLICATIONS FOR ACCESSIBLE MODELS FOR CARDIAC REHABILITATION.

PURPOSE: Despite known health benefits of cardiac rehabilitation (CR) for patients with cardiovascular disease (CVD), only a quarter of eligible patients attend. Among CR barriers are physical (eg, walking) and in-person attendance limitations. The purpose of this study was to determine the prevalence of difficulty walking and dependence on another person to attend medical appointments among people with and without CVD using national survey data. METHODS: We compared the prevalence of difficulty walking and difficulty attending medical appointments alone among adults with and without CVD usin

Improvement in walking impairment following surgical and endovascular revascularization: Insights from VOYAGER PAD.

BACKGROUND: Peripheral artery disease (PAD) affects 200 million people worldwide and is associated with impaired quality of life, increased morbidity, and mortality. Supervised exercise therapy (SET) and lower-extremity revascularization (LER) are both proven strategies to improve patient symptoms. Short and long-term functional outcomes after LER for symptomatic PAD in a large, international cohort have not previously been described. METHODS: The VOYAGER PAD trial (ClinicalTrials.gov identifier: NCT02504216) enrolled subjects after LER for symptomatic PAD (Rutherford category 2-6). Participa

Handgrip strength asymmetry is associated with slow gait speed and poorer standing balance in older Americans.

INTRODUCTION: Handgrip strength (HGS) asymmetry may help identify the functional asymmetries that contribute to mobility limitations. We sought to determine the associations of HGS asymmetry on gait speed and standing balance in older Americans. MATERIALS AND METHODS: The analytic sample included 8,396 adults aged ≥65-years for the last wave in which they participated in the 2006-2016 Health and Retirement Study. Participants were categorized into asymmetry groups based on the degree of HGS asymmetry. Persons with gait speed <0.8 meters/second were slow. Balance scores ranged from 0-4 with lo…
