The incidence of complications related to Caesarean section was studied among 643 patients operated in four hospitals in Zimbabwe. Major complications were recorded in 15% of the patients and minor complications in 27%. Among these complications, infection morbidity played the leading role. Only 53% of Caesarean section were devoid of complication or need of blood transfusion. It is concluded that in a Third World environment, Caesarean section carries a substantial risk of operative and post-operative complications.

Main complication of different diseases is the paralytic ileus. The basis of all pathophysiological mechanisms is a decrease of the immune deficiency in the intestinal wall. At first the intestinal water-, electrolyte- and protein metabolism is disturbed. Secondly we can measure an endotoxinemia and a bacteriemia with all complications. In the end a complete septic shock and an insufficiency of the organ "gut" can be observed. The patients die - in absence of treatment - because of intraabdominal sepsis and peritonitis.

While there have been several overseas studies on the quality of life after proctocolectomy, data concerning patients who have undergone ileostomy and proctocolectomy in Australia are limited. For this reason, a questionnaire was sent to all members of the New South Wales Ileostomy Association. Of the 354 who replied, the indication for ileostomy had been ulcerative colitis in 82%, Crohn's disease in 10%, familial polyposis in 4%; and 4% had undergone proctocolectomy and ileostomy for some other reason. A Caucasian with sickle cell trait (54% A, 43% S hemoglobin) had symptoms and signs of chronic hemolytic anemia as well as splenic infarction while living at very high altitude (in excess of 2,800 m). Trips from sea level back to high altitude repeatedly precipitated pulmonary infarctions. All symptoms and evidence of hemolysis resolved at low altitude.

Hospital admission histories were compared for groups of Navy enlisted men identified as alcohol abusers or controls during the later years of their service careers. The alcohol abuse group had a higher mean number of hospital admissions per year and a higher mean number of days hospitalized per year than did the control group. The factors influencing morbidiy and mortality after liver injury were monitored prospectively in 637 patients. Death correlated with the number of associated injuries, severity of injury, presence of great vessel injury, active bleeding from liver at laparotomy, and severe shock on admission. Four patients who had received silicone injections had the following complications: migration, hepatic disease manifested as granulomatous hepatitis (previously undescribed, to our knowledge), hypopigmentation, and death.
