We report a fatal case of disseminated infection due to Mycobacterium bovis occurring in an elderly patient. The greatest incidence of tuberculosis is in the aged, who were likely to have been infected in childhood when the disease was more prevalent. However, the clinical or radiographic presentation of tuberculosis in the older patient may be atypical or nonspecific, contributing to delay in diagnosis, and greater morbidity and mortality.

We report an unusual organism, Mycobacterium chelonei, which causes digital tenosynovitis. Atypical mycobacterial infections of the hand are uncommon. The diagnosis should be suspected when there is persistent swelling or tenosynovitis, inconsistent with other inflammatory processes such as rheumatoid arthritis. Synovial biopsy specimen and appropriate cultures are necessary to confirm the diagnosis.

Glutamine synthetase from a Gram-positive acid-fast bacterium, Mycobacterium smegmatis, was purified to homogeneity from cells grown with glycerol-bouillon medium. Electron micrographs of the enzyme revealed a dodecameric arrangement of its subunits in two superimposed hexagonal rings, similar to the structure of glutamine synthetase of Escherichia coli. Disc electrophoresis in the presence of sodium dodecyl sulfate indicated a subunit molecular weight of 56,000. The susceptibility of congenitally athymic rats to Mycobacterium leprae infection has been investigated. The association of mast cells and lepra bacilli was studied in the foot pad skin of immunosuppressed mice inoculated with 10(5) lepra bacilli. The mice were killed at intervals varying from 2 1/2-14 months postinoculation. M. leprae are found in large numbers in nerves, blood vessels, muscle fibres, hair follicles, sweat and sebaceous glands. Mastocytosis and extensive degranulation are seen at similar sites. The sensitivity of Mycobacterium lepraemurium for isoniazide, dapsone, ethionamide, pyrazinamide, rifampicin, p-aminosalicylic acid, ethambutol and various sulfonamides was determined using Ogawa medium. The sensitivities of M. lepraemurium for these drugs are different from those obtained with M. leprae. Our results illustrate that M. lepraemurium is not a model for the drug sensitivities of M. leprae.

The differences in susceptibility among C57Bl/6, DBA/2 mice and their F1 hybrids to infections with M. lepraemurium were shown to depend upon the route of infection and size of the inoculum. It is suggested that continuous leakage of bacilli into the circulation from a primary focus of intraneural infection may simultaneously initiate bacillary dissemination and the suppression of cell-mediated immunity. Both these features are essential for the development of lepromatous leprosy. Nerve involvement in leprosy, previously thought of as a diagnostic feature of the disease and as a complication of therapy, may represent an essential phase in the cycle of infection and reinfection by Mycobacterium leprae.

A combined infection with Mycobacterium intracellulare and histoplasmosis in a patient with end-stage sarcoidosis is reviewed. A healthy skepticism should be applied to all but the most classic cases of sarcoidosis. Prolonged follow-up with repeated attempts to demonstrate a specific causative agent will decrease the possibility of overlooking treatable infections. An initial work-up of a patient with sarcoidosis includes fungal immunologic tests, which if positive can lead to further investigation and the correct diagnosis.
