Presence of fetal microchimerisms in the heart and effect on cardiac repair.

Multiple complex biological processes take place during pregnancy, including the migration of fetal cells to maternal circulation and their subsequent engraftment in maternal tissues, where they form microchimerisms. Fetal microchimerisms have been identified in several tissues; nevertheless, their functional role remains largely unknown. Different reports suggest these cells contribute to tissue repair and modulate the immune response, but they have also been associated with pre-eclampsia and tumor formation. In the maternal heart, cells of fetal origin can contribute to different cell lineag

[Perioperative Management of Cardiac Surgery in Patients with Preoperative Heart Failure].

The lower preoperative left ventricular ejection fraction( LVEF), the more postoperative death. The perioperative management for cardiovascular patients with heart failure (LVEF<40%) is of great importance in cardiac surgery. The failing heart is characterized by intracellular Ca2+ handling abnormalities during excitation/contraction coupling( i.e., less amount of cytosolic Ca2+ recruitment in systole and insufficient cytosolic Ca2+ extrusion in diastole), which are caused by increased reverse-mode Na+/ Ca2+ exchange activity and abnormal sarcoplasmic reticular Ca2+ channels (ryanodine recepto

Combinatorial genetic replenishments in myocardial and outflow tract tissues restore heart function in tnnt2 mutant zebrafish.

Cardiac muscle troponin T (Tnnt2) mediates muscle contraction in response to calcium ion dynamics, and Tnnt2 mutations are associated with multiple types of cardiomyopathy. Here, we employed a zebrafish model to investigate the genetic replenishment strategies of using conditional and inducible promoters to rescue the deficiencies in the heart. tnnt2a mutations were induced in zebrafish via the CRISPR/Cas9 technique, and the mutants displayed heart arrest and dilated cardiomyopathy-like phenotypes. We first utilized the classic myocardial promoter of the myl7 and TetOn inducible system to rest

Case report: paradoxical ventricular septal motion in the setting of primary right ventricular myocardial failure.

PURPOSE: In this report, a case of right ventricular (RV) failure, hemodynamic instability, and systemic organ failure is described to highlight how paradoxical ventricular systolic septal motion (PVSM), or a rightward systolic displacement of the interventricular septum, may contribute to RV ejection. CLINICAL FEATURES: Multiple inotropic medications and vasopressors were administered to treat right heart failure and systemic hypotension in a patient following combined aortic and mitral valve replacement. In the early postoperative period, echocardiographic evaluation revealed adequate left

Three-dimensional architecture of pericytes with special reference to their topographical relationship to microvascular beds.

Microvascular beds and pericytes in the submandibular gland, thyroid gland and heart were studied by combined scanning electron microscopy and chemical digestion. The submandibular gland had a relatively loose network of blood capillaries, the thyroid gland possessed a close-meshed network of capillaries, and those in the myocardium ran parallel to the long axis of myocardial cells. The thyroid gland exhibited the largest numbers of pericytes. Three types of pericytes could be distinguished by their shape and localization. Type I pericytes, which were confined to true capillaries, had a fusifo

Strain analysis is superior to wall thickening in discriminating between infarcted myocardium with and without microvascular obstruction.

OBJECTIVES: The aim of the present study was to evaluate the diagnostic performances of strain and wall thickening analysis in discriminating among three types of myocardium after acute myocardial infarction: non-infarcted myocardium, infarcted myocardium without microvascular obstruction (MVO) and infarcted myocardium with MVO. METHODS: Seventy-one patients with a successfully treated ST-segment elevation myocardial infarction underwent cardiovascular magnetic resonance imaging at 2-6 days after reperfusion. The imaging protocol included conventional cine imaging, myocardial tissue tagging a

[Assessment of coronary heart diseases by echocardiography].

Echocardiography plays an important role in deciding the operative indication in patients with coronary artery diseases. On deciding the indication of coronary artery bypass graft, dobutamine stress echocardiography provides accurate information on presence or absence of myocardial ischemia and of myocardial viability in each coronary artery territory. Assessment of the coronary blood flow velocity using high-frequency Doppler enables us to examine the presence of coronary stenosis and the patency of bypass graft. Left ventricular remodeling is a cause of progressive heart failure after myocar

Inotropic interventions during myocardial "stunning" in the pig.

To study the effects of inotropic interventions on myocardial "stunning," we subjected pigs to 12 minutes of left anterior descendents (LAD) coronary artery occlusion followed by 2 hours of reperfusion. Measurements were made of segmental shortening (SS%), myocardial blood flow (MBF), adenine nucleotides, the extent of ultrastructural damage, and granulocyte accumulation. Three groups of pigs were compared: control (CON) (N = 6), isoproterenol infused (ISO) (N = 6), and pretreated with triiodothyronine (T3) (30 micrograms/kg ip) (N = 5). Inotropic reserve was tested with a bolus of Ca++. Durin

Endocardial fibroelastosis and hypoplasia of the left ventricle in neonates without significant aortic stenosis.

Endocardial fibroelastosis in neonates with hypoplasia of the left ventricle is usually associated with severe aortic stenosis or atresia. In this study three hearts were examined, in which severe hypoplasia of the left ventricular cavity with myocardial hypertrophy and endocardial fibroelastosis were associated with small but non-stenotic subaortic outflow tracts and aortic valves. These features were contrasted with those of neonatal left heart hypoplasia in aortic stenosis and atresia. The index cases were examples of the very rare contracted form of endocardial fibroelastosis.

Arrhythmia-Induced Cardiomyopathy: JACC State-of-the-Art Review.

Arrhythmias coexist in patients with heart failure (HF) and left ventricular (LV) dysfunction. Tachycardias, atrial fibrillation, and premature ventricular contractions are known to trigger a reversible dilated cardiomyopathy referred as arrhythmia-induced cardiomyopathy (AiCM). It remains unclear why some patients are more prone to develop AiCM despite similar arrhythmia burdens. The challenge is to determine whether arrhythmias are fully, partially, or at all responsible for an observed LV dysfunction. AiCM should be suspected in patients with mean heart rate >100 beats/min, atrial fibrillat

Myosin motor dynamics and cardiac function: Insights from in vivo small-angle X-ray scattering and pressure-volume analysis.

Regulation of myosin motor extension and conformation is central to cardiac muscle contraction-relaxation, with myosin playing a critical role in mechanosensing during the cardiac cycle. Direct assessment of in vivo dynamic interplay between myosin head position, cross-bridge cycling, sarcomere shortening (filament sliding), muscle stress-strain rates and pressure-volume (PV) relationships is key to understanding both normal cardiac function and ventricle pathological states. This work aims to demonstrate that in vivo temporal regulation of myosin head transfer to actin filaments in systole an

Comparison of acute changes in left ventricular volume, systolic and diastolic functions, and intraventricular synchronicity after biventricular and right ventricular pacing for heart failure.

BACKGROUND: Biventricular pacing (BiV) therapy has recently been shown to improve systolic function and cause reverse remodeling in patients with advanced heart failure with electromechanical delay. In these patients, the benefit of right ventricular (RV)-based pacing was controversial. We compared the acute changes in systolic and diastolic function, left ventricular (LV) volume, and intraventricular synchronicity in BiV pacing, RV pacing, and without pacing (No) by means of echocardiography and tissue Doppler imaging (TDI). METHODS: TDI was performed in 33 patients with heart failure after

Impact of bicarbonate on ionized calcium and cardiac function in albumin-free Langendorff perfusion in hearts of male rats.

The adult Langendorff methodology lacks standardization, with inconsistencies in the pH and calcium concentrations of Krebs-Henseleit (K-H) solution. The aim of this study was to clarify these inconsistencies. Adult male Wistar rat hearts were subjected to 40 or 80 min of global ischemia followed by reperfusion. Recovery of left ventricular developed pressure (LVDP) and triphenyltetrazolium (TTC) staining were evaluated. Ion analysis was performed using rat plasma. A calcium concentration of 1.75 mM was associated with increased contractile force and more severe ischemia-reperfusion injury. Th

Myocardial protection by nanomaterials formulated with CHIR99021 and FGF1.

The mortality of patients suffering from acute myocardial infarction is linearly related to the infarct size. As regeneration of cardiomyocytes from cardiac progenitor cells is minimal in the mammalian adult heart, we have explored a new therapeutic approach, which leverages the capacity of nanomaterials to release chemicals over time to promote myocardial protection and infarct size reduction. Initial screening identified 2 chemicals, FGF1 and CHIR99021 (a Wnt1 agonist/GSK-3β antagonist), which synergistically enhance cardiomyocyte cell cycle in vitro. Impact of Ischemic Heart Failure Etiology on Cardiac Recovery During Mechanical Unloading.

BACKGROUND: Small-scale studies focused mainly on nonischemic cardiomyopathy (NICM) have shown that a subset of left ventricular assist device (LVAD) patients can achieve significant improvement of their native heart function, but the impact of ischemic cardiomyopathy (ICM) has not been specifically investigated. Many patients with acute myocardial infarction are discharged from their index hospitalization without heart failure (HF), only to return much later with overt HF syndrome, mainly caused by chronic remodeling of the noninfarcted region of the myocardium. OBJECTIVES: This study sought

An approach to the three-dimensional display of left ventricular function and viability using MRI.

Cardiac MRI was performed in human volunteers to determine the magnitude of the misregistration (MSR) of cardiac landmarks due to variability in the diaphragm position for repeated breath-holds. Seven normal volunteers underwent MR imaging of the left ventricle (LV) to evaluate the magnitude of the endocardial centroid MSR. The MSR for a mid-ventricle short-axis image was 3.01 +/- 1.68 mm through-plane and 4.16 +/- 1.62 mm in-plane. A second order polynomial fit through the LV centroid coordinates minimized the in-plane component of the MSR error. Short-axis cine images, corrected for MSR, pro

Myocardial microdialysis of salicylic acid to detect hydroxyl radical generation during ischemia.

We examined in vivo monitoring of hydroxyl radical (.OH) generation during myocardial ischemia for 30 min by occluding the left anterior descending (LAD) in dog heart using a microdialysis technique. The hydroxyl radical reacts with salicylate and generates 2,3- and 2,5-dihydroxybenzoic acids (DHBA) which can be measured electrochemically in picomole quantity by a high pressure liquid chromatography-electrochemical (HPLC-EC) procedure. When the premature ventricular contraction (PVC) occurred at almost 25 sec intervals, marked elevation of the levels of 2,3- and 2,5-DHBA was observed in the he
