Efficacy of per-oral endoscopic myotomy for the treatment of non-achalasia esophageal motor disorders.

INTRODUCTION: Per-oral endoscopic myotomy (POEM) is effective in achalasia. The objective of this study was to evaluate the short-term clinical efficacy of POEM in non-achalasia esophageal motility disorders (NAEMD). PATIENTS AND METHODS: Patients with NAEMD diagnosed by high-resolution manometry were included in a retrospective multicentric study. For each individual case, two controls paired on gender and age were matched: one with type I/II achalasia and one with type III achalasia. The clinical response, defined by an Eckardt score ≤ 3, was assessed at 3 and 6 months. RESULTS: Ninety pat…

Peroral endoscopic shorter versus longer myotomy for the treatment of achalasia: a comparative retrospective study.

INTRODUCTION: Peroral esophageal myotomy (POEM) is a novel endoscopic treatment for achalasia. It has gained popularity worldwide among surgeons and endoscopists, but no studies have compared peroral endoscopic short with long myotomy for achalasia. We aimed to compare the clinical efficacy and safety between peroral endoscopic shorter and longer myotomy. METHODS: The retrospective study enrolled 129 achalasia patients who underwent POEM from July 2011 to September 2017. Based on the myotomy length (ML), patients were divided into shorter myotomy (SM) group (ML ≤ 7 cm, n = 36) and longer myot…

Modified arthroscopic anterior myotomy for internal derangement of the temporomandibular joint: clinical and radiological results.

The aim of this study was to present the results of a modification of the arthroscopic anterior myotomy for the treatment of internal derangement (ID) of the temporomandibular joint (TMJ): the minimally invasive arthroscopic anterior myotomy (MIAAM). Fifteen joints with Wilkes stages III-IV ID treated with this technique were studied. Clinical data evaluated were pain (visual analogue scale, VAS) and articular movements (preoperatively and at 1, 3, 6, 9, and 12 months postoperative). The position of the disc at 1 year after surgery was compared with the pre-surgical position, using magnetic re…

Per-oral endoscopic myotomy with septotomy for the treatment of distal esophageal diverticula (D-POEM).

BACKGROUND AND AIMS: Epinephric diverticula are frequently associated with esophageal motility disorder. Their management implies surgery, with 15% morbidity and 3% mortality rates. Flexible endoscopy could be an effective and safer approach for treating esophageal diverticulum with motility disorder. We report our experience of seven consecutive cases treated with per-oral endoscopic submucosal septotomy and myotomy (D-POEM). METHODS: Seven consecutive patients were referred for symptomatic non-zenker's esophageal diverticulum. The steps of the procedure were as follows: (i) analysis of the …

Paravertebral Block for a Patient With Achalasia Undergoing a Peroral Endoscopic Myotomy Procedure: A Case Report.

In 2016, the American Medical Association officially dismissed pain as a vital sign quoting the opioid epidemic as a major reason. Clinically, pain remains very relevant and we present the case of a patient with achalasia treated via peroral endoscopic myotomy procedure (POEM). Given that similar patients previously failed traditional pain management modalities, regional anesthesia was used in this patient's pain management. The positive outcomes yielded from this technique convinced our gastroenterological colleagues to request regional anesthesia for future patients, altering their approach …

Submucosal fibrosis in achalasia patients is a rare cause of aborted peroral endoscopic myotomy procedures.

Background and aims Peroral endoscopic myotomy (POEM) is now an established treatment for esophageal achalasia. The standard protocol ensures a smooth operation in most patients, but technical challenges and failures exist and little is known about the incidence, causes, and impact of aborted procedures. Here, using a large patient cohort, we attempted to answer these questions. Methods All patients admitted for planned POEM between August 2010 and July 2015 underwent chart review. Aborted POEM was defined as the inability to finish the procedure after submucosal injection. The cause of the fa…

Peroral Endoscopic Myotomy with EndoFLIP and Double-Endoscope: Novel Techniques for Achalasia in Pediatric Population.

BACKGROUND: Experience of peroral endoscopic myotomy (POEM) for treatment of achalasia in pediatric population is limited with varying techniques in different centers. The accurate extent of submucosal tunneling into the gastric cardia and the adequacy of myotomy are the important determining factors to success of POEM. A majority of studies in pediatric population have described using submucosal dye injection for assessing the adequacy of myotomy, however, this is a rather crude and inaccurate method. We herein describe the first case of pediatric achalasia managed with POEM incorporated with…

Pattern of esophageal eosinophilic infiltration in patients with achalasia and response to Heller myotomy and Dor fundoplication.

Eosinophilic esophagitis (EoE) is now recognized as a common cause of dysphagia. Eosinophilic infiltration of the esophagus has also been associated with other conditions, such as gastroesophageal reflux disease (GERD); however, the incidence, pattern, and clinical significance of eosinophilic infiltration in achalasia are poorly documented. We sought to characterize this histological finding in patients undergoing Heller myotomy (HM) for achalasia. Ninety-six patients undergoing laparoscopic HM for primary achalasia between 1999 and 2008 were identified from a prospective database. Serial mid…

Long-term results of conventional myotomy in patients with achalasia: a prospective 20-year analysis.

Myotomy has proved to be an efficient primary therapy in patients with achalasia, especially in younger patients (<40 years of age). The results of laparoscopic myotomy cannot be finally assessed, on account of the shorter postoperative follow-up. Thus, there are considerable data regarding intermediate-term outcomes after laparoscopic myotomy. The aim of our study was a 20-year analysis of the conventional cardiomyotomy as the underlying basis assessing the results of minimal-invasive surgery. Within 20 years (September 1985 through September 2005), 161 operations for achalasia were performed…

Importance of coronoidotomy and masticatory muscle myotomy in surgical release of trismus caused by submucous fibrosis.

Thirty patients with trismus resulting from betel nut chewing-induced oral submucous fibrosis were treated with either fibrotic tissue release only (group I) or fibrotic tissue release in combination with masticatory muscle myotomy and coronoidotomy (group II). The latter procedures were only performed in patients whose intraoperative interincisal distance remained less than 35 mm immediately after submucous fibrous tissue release. There were eight and 22 patients in groups I and II, respectively. In group I patients, the average intraoperative interincisal distance improved from 19.5 mm to 42…

[Efficacy of extramucosal myotomy in an experimental model of intramural colonic obstruction. An experimental study in rats].

An experimental study over the efficacy of myotomy in the treatment of intramural colon obstruction is presented. Forty Wistar rats were used. Four groups were made with ten animals each: Group I: control; Group II: myenteric denervation of four cm segment of transverse colon; Group III: myenteric denervation of 4 cm segment of transverse colon and myotomy; Group IV: myotomy of segment of transverse colon. Myenteric denervation was produced by serosal application of 0.1% benzalkonium chloride during 30 minutes. Animals were evaluated by clinical parameters (survival, weight change, food intake…

Pressure-flow analysis following peroral endoscopic myotomy in pediatric achalasia: Novel insights into esophageal emptying.

OBJECTIVE: Achalasia is a rare esophageal motility disorder in children. Peroral endoscopic myotomy (POEM) has emerged as a promising treatment yet standardized post-treatment evaluation remains lacking. While high-resolution manometry (HRM) is the diagnostic gold standard, its role post-treatment is limited. Pressure-flow analysis (PFA), derived from high-resolution impedance manometry (HRIM), offers a novel approach to assessing bolus transport and esophageal emptying. We aimed to evaluate PFA metrics in pediatric achalasia patients before and after POEM and assess its utility in monitoring …

Pneumatic Balloon Dilation versus Heller's Myotomy in Pediatric Achalasia: A Systematic Review and Meta-Analysis.

The optimal surgical treatment for achalasia in children remains controversial. Pneumatic balloon dilation (PBD) and Heller's myotomy (HM) are the most commonly used interventions, but comparative evidence is limited and based mainly on small observational studies. This study aimed to compare clinical outcomes of PBD and HM in children with achalasia through a systematic review and meta-analysis.PubMed, EMBASE, and Cochrane were searched through June 2025. Comparative studies evaluating PBD and HM in patients younger than 18 years were included. Outcomes of interest included symptom recurrence,…

Management of Peroral Endoscopic Myotomy Leak With an Esophageal Fully Covered Self-Expandable Metal Stents.

Achalasia type II is a rare esophageal motility disorder characterized by impaired relaxation of the lower esophageal sphincter. Peroral endoscopic myotomy (POEM) is a safe and minimally invasive procedure for achalasia. Although rare, complications such as esophageal perforation and pleural effusion can arise. This case study presents a 69-year-old woman with achalasia type II who developed esophageal perforation and empyema after POEM. Initial management with hemostatic clips was insufficient due to stent migration, necessitating a 135-mm fully covered self-expandable metal stent. Due to per…
