Vertebral artery dissection (VAD) can cause posterior-circulation ischemia after cervical manipulation; postpartum physiological changes may increase vascular vulnerability. A 30-year-old woman, nine months postpartum, developed an abrupt occipital headache, vertigo, nausea, and gait imbalance during a commercial neck massage. On examination, she had truncal ataxia and gait instability without focal limb weakness. We obtained 1.5-T MRI (diffusion-weighted imaging (DWI)/apparent diffusion coefficient (ADC), T1-weighted imaging (T1), T2-weighted imaging (T2), fluid-attenuated inversion recovery

Central neck compartment metastases are most commonly found in papillary thyroid cancer(PTC) patients. Central neck dissection (CND), encompassing lymph node levels Ⅵ and Ⅶ, remains controversial when performed prophylactically CND (pCND) in clinically node-negative (cN0) PTC patients. Current guidelines, consensus statements, and literatures lack a unified, objective criterion for determining cN0 status. The reasons for the controversy surrounding pCND lie in the varying oncological benefits and complication rates of pCND reported in different studies for cN0 PTC patients, as well as the inco…

OBJECTIVE: To identify T1N0M0 oral tongue squamous cell carcinoma (OTSCC) patient subgroups benefiting most from neck dissection (ND) based on the SEER database.
METHODS: Patients with T1N0M0 OTSCC were identified from the SEER database. Propensity score-matching (PSM) was utilized to balance baseline characteristics between ND and non-ND groups. Survival differences between these groups were compared using Kaplan-Meier (K-M) analysis and log-rank tests. Additionally, a multivariable Cox regression model was applied to the pre-matched cohort to identify independent prognostic factors.

BACKGROUND: The indication and extent of selective lateral neck dissection (LND) for cN1a papillary thyroid carcinoma (PTC) remain uncertain. The present study aimed to identify potential predictors and distribution pattern of lateral lymph node recurrence (LLNR) after central neck dissection in cN1a PTC patients.
METHODS: The cN1a PTC patients who underwent initial central neck dissection at our centre were retrospectively reviewed, and the median follow-up period was 6.8 years. Reoperation with LND was performed when LLNR was confirmed. Risk factors for LLNR were identified, and the metasta…

BACKGROUND: The neck management of clinical-nodal negative (cN0) oral squamous cell carcinoma (OSCC) remains controversial. Elective neck dissection (END) and observation are the main strategies, but it is still not clear who could benefit the most from END. The purpose of this study was to clarify the potential clinical factors that affect the therapeutic value of END and to explore the actual characteristics associated with benefit from END.
METHODS: Patients with cN0 OSCC were identified in the SEER database from 2000 to 2019. 5-year Overall survival (OS) and disease-specific survival (DSS…

BACKGROUND: Cervical lymph node metastases in papillary thyroid cancer are common. Although central neck dissection is indicated in clinically nodal-positive disease, it remains controversial in patients with no clinical evidence of nodal metastasis. The aim of this retrospective study was to determine the outcomes of clinically lymph node-negative patients with papillary thyroid cancer who underwent total thyroidectomy without a central neck dissection, in order to determine the rates of recurrence and reoperation in these patients compared with a group of patients submitted to total thyroide…

BACKGROUND: The pattern of lateral cervical metastases from papillary thyroid carcinoma (PTC) has been reported without a clear understanding of the distribution of central nodes at risk. The present study evaluated the pattern of central and lateral cervical metastases from PTC with respect to recently defined neck sublevels and subsites.
METHODS: Between 2003 and 2006, 52 consecutive patients with lateral cervical metastases from previously untreated PTC underwent total thyroidectomy and therapeutic comprehensive neck dissection of the central and lateral compartments, including five bilate…

Since 1983, 421 patients have been treated for lung cancer at this institute. Since 1988, neck lymph node dissection (11 cases) and new extended lymphadenectomy through a collar incision and median sternotomy (22 cases) have been conducted. Indications for this new radical operation are scalene, supraclavicular or highest mediastinal node involvement, or superior pulmonary sulcus carcinoma, in patients aged 70 or less without distant metastasis and NSCLC. No major complications and operative mortality were encountered in this study. Patients with scalene or supraclavicular node involvement sho

BACKGROUND: Neck dissections (ND) are a routine procedure in head and neck oncology. Given the postoperative functional impact that some patients experience, it is imperative to identify and track quality of life (QoL) symptomatology to tailor each patient's therapeutic needs. To date, there is no validated French-Canadian questionnaire for this patient-population. We therefore sought to translate and validate the Neck Dissection Impairment Index (NDII) in Canadian French.
METHODS: A 3-phased approach was used. Phase 1: The NDII was translated from English to Canadian French using a "forward …

Necrotizing soft tissue infection (NSTI) is a rare, but life-threatening, complication of head and neck surgery. We present a 70-year-old male with a history of immunosuppression who presented with polymicrobial NSTI following parotidectomy and neck dissection for cutaneous squamous cell carcinoma. The objective of this report was to promote awareness for NSTI following parotidectomy and selective neck dissection and highlight the management measures that can optimize survival outcomes. We performed a database search that identified 1,025 citations, of which 5 articles described classified as …

Fifty-nine patients with head and neck carcinoma were examined with 67Ga scintigraphy. All patients had undergone lymph node dissection of the neck. They were followed for a minimum of 2 years after the examination. The primary tumor, treated prior/contemporaneously to the lymph node dissection, did not evolve in this interval. Metastatic involvement of the lymph node capsule was observed in all 44 cases with metastatic lymph nodes; macroscopic radicality was surgically obtained since involvement of the capsule was only microscopic. Nevertheless, complementary radiotherapy was given. The whole
