A Rare Case of Laparoscopic Removal of a Giant Renal Pelvic Pus Clot Caused by Citrobacter Freundii Infection.

It is extremely rare for renal pelvic pus clot with a diameter of 3 cm that formed by purulent secretions due to bacterial infection. A 37-year-old female patient developed a giant renal pelvic pus clot with a diameter of 3 cm 5 months after percutaneous nephrolithotomy, caused by Citrobacter freundii infection, resulting in lower back pain and fever. Due to the lack of specialized tissue pulverizers for percutaneous nephrolithotomy, laparoscopic pyelotomy via intraperitoneal approach removed the giant renal pelvis pus clot. However, it was found that some small pus clots had formed around the

Micropercutaneous nephrolithotomy. A new therapeutic option for pediatric renal lithiasis.

INTRODUCTION: Micropercutaneous nephrolithotomy is an evolution from the conventional percutaneous surgery in which pyelocaliceal access is obtained through minimum bore holes. Its objective is the complete removal of the calculi, lowering the morbidity associated with larger bore percutaneous tracts. MATERIAL AND METHODS: We present the case of a micropercutaneous nephrolithotomy performed in a 14-year-old female patient with a 35 mm diameter kidney stone located in the renal pelvis. Surgery was performed in the Galdakao-modified supine Valdivia position. Puncture was done under ultrasound a

[Valdivia supine position as the best option for percutaneous surgery of renal calculi in morbidly obese patients].

INTRODUCTION: Treatment of the morbidly obese patient with symptomatic renal calculi is an interesting urological challenge. Extracorporeal shock wave lithotripsy is frequently not possible for several reasons, and many urological centers match these patients as one of the residual indications for open surgery of kidney lithiasis. MATERIAL AND METHOD: Two patients with body mass index more than 50 k/m2 and symptomatic kidney stones were treated with percutaneous nephrolithotomy in the supine position described by Valdivia (slightly lateralized supine decubitus). Standard instrumentation for p

Nephropleural Fistula as a Rare Complication of Nephrolithiasis: A Case Report and Literature Review.

Nephropleural fistula (NPF) is an exceptionally rare yet clinically significant complication that typically arises in the context of severe urinary tract infections or following percutaneous nephrolithotomy. We describe a case of NPF secondary to a complicated urinary tract infection, outlining its clinical course, diagnostic challenges, and therapeutic management. This report highlights the importance of early recognition and appropriate interventions to prevent the morbidity associated with this unusual entity.

Successful management of delayed post-PCNL renal pseudoaneurysm using a percutaneous endoscopic transurethral resection system: A case report.

Renal artery pseudoaneurysm is a rare but potentially life-threatening complication following percutaneous nephrolithotomy (PCNL). We report a female who developed massive gross hematuria after an uncomplicated right PCNL. Computed tomography angiography revealed a renal artery pseudoaneurysm, and selective angioembolization failed to achieve definitive hemostasis. As a salvage approach, percutaneous endoscopic intervention was performed. Under direct intrarenal visualization, the actively bleeding arterial branch was identified and successfully coagulated, achieving complete hemostasis. The p

Efficacy and safety of papillary vs non-papillary access to the calyces in patients undergoing percutaneous lithotripsy: a GRADE-assessed systematic review and meta-analysis.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) access is traditionally obtained via papillary puncture to minimize bleeding, though non-papillary access is frequently utilized in everyday clinical practice due to practical considerations such as anatomical variations and stone location. Because existing comparative studies have yielded inconsistent findings, this systematic review and meta-analysis aimed to synthesize existing evidence evaluating the safety and efficacy of papillary versus non-papillary access during PCNL. METHODS: A comprehensive systematic search of databases, including Pu

Ureteral stent encrustation: evaluation of available scores as predictors of a complex surgery.

BACKGROUND: Stent encrustation is an uncommon event (13%) with a significant impact in patients' management. Aim of our study was to evaluate the available grading systems for encrusted stents. METHODS: A retrospective analysis of encrusted stents was performed in four Italian centers between 2006 and 2020. Encrusted stents were classified according to four classifications: the Forgotten Encrusted Calcificated (FECal) Score, the Kidney Ureter Bladder (KUB) Score, the Visual Grading for Ureteral Encrusted Stent Classification and the Encrustation Burden Score (EBS). Classifications were evalua

Intravenous misplacement of the nephrostomy catheter into the inferior vena cava and secondary thrombosis following percutaneous nephrostolithotomy: A case report and literature review.

Nephrostomy catheter misplacement into the inferior vena cava after percutaneous nephrolithotomy is an extremely rare complication, and subsequent catheter-related thrombosis has been more rarely reported. Here, we report a rare case of nephrostomy catheter misplacement after percutaneous nephrolithotomy. During the procedure, due to bleeding upon establishing the puncture channel, a renal fistula catheter with a balloon was inserted to facilitate hemostasis. However, the catheter inadvertently migrated into the inferior vena cava, with the inflated balloon obstructing venous return, resulting

[Endoscopic and percutaneous treatment of purulent retention caused by obstructive calculi of the upper urinary tract. Observations apropos of 6 case reports].

On the basis of 6 cases of pyonephrosis due to renal stones successfully treated without surgery over a period of one year, the authors define their current therapeutic approach which consists of 2 phases: --1st phase: salvage of the kidney by ultrasound guided percutaneous nephrostomy under local anaesthesia. No hasty endoscopic procedures in an attempt to insert a ureteric catheter into the kidney; --2nd phase: elimination of the obstructing stone (on average, 8 days after the 1st phase), by percutaneous nephrolithotomy, descending ureteroscopy or ascending ureteroscopy.

Infection stones in patients with myelomeningocele and ileal conduit urinary tract diversion.

Infection renal calculi are serious complications of urinary tract diversion in patients with myelomeningocele. During a 12-month period, 10 renal units containing calculi were managed surgically. The kidneys were in patients with multiple previous operations and medical problems. Nine of the ten stones had a staghorn configuration, filling the renal pelvis with calyceal extensions. Nephrectomy was necessary for a nonfunctional kidney and a conventional nephrolithotomy was done in one case. Endourologic techniques were used for treatment of eight renal units. Endourologic methods depended on p
