Free tissue transfer for the treatment of facial paralysis.

Long-standing facial paralysis requires the introduction of viable, innervated dynamic muscle to restore facial movement. The options include regional muscle transfer and microvascular free tissue transfer. There are advantages and disadvantages of each. Briefly, the regional muscle transfer procedures are reliable and provide immediate return of movement. However, the movement is not of a spontaneous mimetic nature. Free tissue transfer, in contrast, offers the possibility of synchronous, mimetic movement. It does, however, require a prolonged healing time in comparison with that of regional

Vulnerability of different nerves to intrafascicular injection by different needle types and at different approach angles: a mathematical model.

BACKGROUND AND OBJECTIVES: We assume that intrafascicular spread of a solution can only occur if a large enough portion of the distal needle orifice is placed inside the fascicle. Our aim is to present and evaluate a mathematical model that can calculate the theoretical vulnerability of fascicles, analyzing the degree of occupancy of the needle orifice in fascicular tissue by performing simulations of multiple positions that a needle orifice can take inside a cross-sectional nerve area. METHODS: We superimposed microscopic images of two routinely used nerve block needles (22-gauge, 15° needle

Brachial artery perforator-based propeller flap coverage for prevention of readhesion after ulnar nerve neurolysis.

It is difficult for most plastic and orthopaedic surgeons to treat nerve dysfunction related to neural adhesion because the pathophysiology and suitable treatment have not been clarified. In the current report, we describe our experience of surgical treatment for adhesive ulnar neuropathy. A 58-year-old male complained of pain radiating to the ulnar nerve-innervated area during elbow and wrist motion caused by adhesive ulnar neuropathy after complex open trauma of the elbow joint. The patient obtained a good clinical outcome by surgical neurolysis of the ulnar nerve combined with a brachial ar

[On the CT-diagnosis of optic nerve lesions. Differential diagnostic criteria (author's transl)].

Computed tomograms of 166 optic nerve lesions were analyzed: 97 were mainly orbital and 69 mainly intracranial. The criteria were clinical course, size, density and delineation of the optic nerve shadow, orbital and cerebral soft tissue abnormalities, and bony changes in the optic canal. Characteristic CT features are described of individual disease entities such as optic gliomas, optic nerve sheath meningiomas, neoplastic and inflammatory infiltrations. The differential diagnostic importance of individual CT criteria is evaluated and discussed. Simultaneous visualization of orbital and intrac

Progression of regeneration after nerve infarction.

We performed morphologic studies on the progression of regeneration in cat hind limb nerves after acute ischemic injury. None of the damaged nerve trunks showed a major increase in endoneurial connective tissue. Despite this fact, regeneration of nerve with transfascicular infarct was far from complete even 16 months after injury as manifested by a striking increase in the small myelinated fibers (MF) number and decrease in large MF number. Restoration of infarcted nerves was less complete than that previously reported after a nerve crush. Changes in the necrotic nerve segment and difficulty i

Sciatic nerve injury following analgesic drug injection in rats: A histopathological examination.

OBJECTIVE: Sciatic nerve neuropathy can be observed following intramuscular gluteal injections. The histopathological examination of sciatic nerve damage following intramuscular injection in the gluteal region for acute pain treatment is not feasible in humans due to the inability to dissect and examine the nerve tissue. To overcome this issue, we used a rat model for demonstrating damage to the sciatic nerve tissue after the application of commonly used drug injections. METHODS: We investigated possible damage following the intramuscular injection of diclofenac, lornoxicam, morphine, and pet…

Bilateral fibrolipomatous hamartoma of the median nerve with macrocheiria and late-onset nerve entrapment syndrome.

The case of a 46-year-old man is reported who presented with an extraordinary constellation of fibrolipomatous hamartomas of the right ulnar and both median nerves, with a right-sided giant hand ("macrocheiria") due to enlarged bones and subcutaneous tissue, and unusual late manifestation of nerve entrapment at the right wrist. Magnetic resonance imaging of the hypertrophied nerves disclosed large nerve fascicles surrounded by fibrous tissue, which allowed preoperative diagnosis.

[Mucoid pseudo-cysts of the sheath of the external popliteal sciatic nerve. Apropos of 2 cases].

Two cases of paralysis of the etxernal popliteal sciatic nerve are described. They are secondary to the development of a pseudo-cyst containing mucoid matter within the connective tissue sheath of the nerve itself. The authors describe the pathological anatomy of these lesions and discuss their aetiology, favouring the theory of mucoid degeneration of the connective tissues of the nerve sheath. They give brief indication of the clinical symptoms and emphasize the need for early but simplified surgery, ruling out any nerve resection.

Quantitative regional structure of the normal human lamina cribrosa. A racial comparison.

We digitized magnified photographs of cross-sections of the laminae cribrosae of 16 normal subjects 50 or more years of age. The sample included 7 blacks and 9 age-matched whites. Connective tissue area as a proportion of the lamina cribrosa area was significantly less and pores were significantly larger in the superior and inferior than in the nasal and temporal quadrants of the lamina cribrosa. These differences were more pronounced for peripheral as compared with central regions of the lamina cribrosa. Less connective tissue support for nerve fibers in the superior and inferior quadrants of

Highly aligned electroactive ultrafine fibers promote the differentiation of mesenchymal stem cells into Schwann-like cells for nerve regeneration.

This study investigates the efficacy of a novel tissue-engineered scaffold for nerve repair and functional reconstruction following injury. Utilizing stable jet electrospinning, we fabricated aligned ultrafine fibers from dopamine and poly(L-lactic acid) (PLLA), further developing a biomimetic, oriented, and electroactive scaffold comprising poly(pyrrole) (PPy), polydopamine (PDA), and PLLA through dual in situ polymerizations. The scaffold demonstrated enhanced cell adhesion and reactive oxygen species (ROS) scavenging capabilities and promoted the differentiation of mesenchymal stem cells (M

Quantitative analysis of neural distribution in human coracoacromial ligaments.

This study investigated sensory nerve distribution in 27 human coracoacromial ligaments by immunohistochemical methods using antiprotein gene product 9.5 antibody and anticalcitonin gene related peptide antibody. Mean nerve densities were compared among three areas (acromion side, center, and coracoid side) in two groups (patients with rotator cuff tears and patients with shoulder dislocations). In all three areas of both groups, many nerve fibers immunoreactive to antiprotein gene product 9.5 antibody were observed in the periligamentous bursal tissue. However, in the ligament parenchyma, ner

[Nerve biopsy].

Nerve biopsy, if undertaken at centres with appropriate expertise, may be a valuable diagnostic procedure in selected patients with peripheral neuropathy. Sural nerve biopsy and skin biopsy for evaluation of epidermal nerves are described. Indications, surgical procedure, preparation and transport are reviewed. Usually the patient has to be admitted to a hospital with the necessary resources for a comprehensive battery of nerve tissue processing, including teasing and electron microscopical examination, and other investigative procedures. Another option, when practically and economically feasi…

Neurological recovery across a 2-cm gap of radial nerve repair using end to end suture following supracondylar humerus fracture: Case report.

INTRODUCTION AND IMPORTANCE: Traumatic laceration of the radial nerve following supracondylar humerus fracture in the pediatric population is highly uncommon. Transection of the radial nerve is rare, and approximately 12.7% of all nerve injuries are categorized as neurapraxia that will resolve spontaneously. CASE PRESENTATION: We report a rare case of a 9-year-old boy presented with a right wrist drop after fell on an outstretched hand two weeks before. The radiological finding revealed a posteriorly displaced supracondylar humerus fracture. Intraoperatively, we found the radial nerve had los

Mimosa-Inspired Stimuli-Responsive Curling Bioadhesive Tape Promotes Peripheral Nerve Regeneration.

Trauma often results in peripheral nerve injuries (PNIs). These injuries are particularly challenging therapeutically because of variable nerve diameters, slow axonal regeneration, infection of severed ends, fragility of the nerve tissue, and the intricacy of surgical intervention. Surgical suturing is likely to cause additional damage to peripheral nerves. Therefore, an ideal nerve scaffold should possess good biocompatibility, diameter adaptability, and a stable biological interface for seamless biointegration with tissues. Inspired by the curl of Mimosa pudica, this study aimed to design an…

Facial Nerve Abnormalities in Congenital Middle Ear Malformations With Comments on Preoperative Detectability: A Report of Two Cases.

Facial nerve abnormalities and congenital middle ear malformations originate in similar developmental stages and are often concomitant. Preoperative recognition of such a condition is desirable to prevent any damage, but it is often difficult. Here, we report two cases of middle ear malformations associated with facial nerve abnormalities, describe the computed tomography (CT) appearance, and discuss its preoperative detectability. Case 1 was a 40-year-old female with a facial nerve abnormality associated with stapes ankylosis. She underwent stapes surgery. The facial nerve was hanging over th

Effect of bleeding on nerve regeneration and epineural scar formation in rat sciatic nerves: an experimental study.

OBJECTIVE: Epineural scar formation is one of the most significant negative factors affecting surgical repair after peripheral nerve injury. The scar tissue mechanically hinders axonal regeneration and causes adhesions between nerves and surrounding tissues. A hemostatic agent Ankaferd Blood Stopper (ABS; İmmun Gıda İlaç Kozmetik San. ve Tic. Ltd. Şti., Istanbul, Turkey) has not been previously used. Decreasing the postoperative bleeding and adhesions between nerve and surrounding tissues will prevent the formation of scar tissue, as well as corresponding compressive neuropathy and/or decelera

Biodegradable cell-laden starch foams for the rapid fabrication of 3D tissue constructs and the application in neural tissue engineering.

Cells encapsulation by biomaterials has been widely studied as a strategy of building tissue construct in tissue engineering. Conventional encapsulation of cells using hydrogels often needs the polymerization process or relatively complex molding process. In this study, we developed a facile strategy for the in situ fabrication of biodegradable cell-laden starch foams. By utilizing the unique gelatinization property of starch, cell-laden starch foams with tunable architecture were rapidly prepared in a green and biological-friendly process. The bubble size and stiffness of starch foams could b
