Carpal tunnel syndrome secondary to ganglion cyst in a child.

Carpal tunnel syndrome is uncommon in children and is associated with an underlying etiology in the majority of cases. The diagnosis of the condition in childhood is dependent on a high degree of clinical suspicion, careful clinical examination, and judicious use of confirmatory investigations. The authors report a novel cause of carpal tunnel syndrome in a child, and discuss the investigation and management strategies in childhood carpal tunnel syndrome.

Sensitivity and specificity of the 'Awaji' electrodiagnostic criteria for amyotrophic lateral sclerosis: retrospective comparison of the Awaji and revised El Escorial criteria for ALS.

The Awaji Commission recently proposed a modification of the electrodiagnostic criteria for ALS. We assessed whether the Awaji recommendations improve the sensitivity of the early diagnosis of ALS. In a retrospective study we reviewed clinical and neurophysiological data for 213 patients who visited our motor neuron disease outpatient clinic between October 2006 and December 2008. Using the El Escorial criteria, 51 patients were diagnosed with definite or probable ALS, 14 with probable laboratory-supported ALS, and 28 with possible ALS. An alternative diagnosis was present in 120 patients. App

Orthostatic hypotension as a presenting symptom of the Guillain-Barré syndrome.

Dysautonomia is commonly observed in Guillain-Barré syndrome (GBS). However, hypotension is rarely a presenting and persisting symptom. A man with orthostatic hypotension progressively suffered with distal sensory disturbance, weakness in limbs, and persisting paralytic ileus. Dr Liu et al. made the diagnosis of GBS and successfully treated him with intravenous immunoglobulin. Chronic alcoholism might underlie this atypical course.

Organotypic cultures as tools for testing neuroactive drugs - link between in-vitro and in-vivo experiments.

The development of neuroactive drugs is a time consuming procedure. Candidate drugs must be run through a battery of tests, including receptor studies and behavioural tests on animals. As a rule, numerous substances with promising properties as assessed in receptor studies must be eliminated from the development pipeline in advanced test phases because of unforeseen problems like intolerable side-effects or unsatisfactory performance in the whole organism. Clearly, test systems of intermediate complexity would alleviate this inefficiency. In this review, we propose cultured organotypic brain s

Acute sensory ataxic neuropathy with antibodies to GD1b and GQ1b gangliosides and prompt recovery.

Three patients developed acute pure sensory ataxic neuropathy. Two of the three patients had a recent Campylobacter jejuni infection. Patient 1 had monospecific IgG anti-GD1b. Patients 2 and 3 had cross-reactive IgG anti-GQ1b and anti-GD1b and patient 2 also had IgG anti-GT1a. Motor nerve conduction studies were completely normal. Sensory conductions showed reduced amplitude or absent sensory nerve action potentials with normal or slightly slowed conduction velocities. In patient 2, serial electrophysiological studies showed reappearance and improvement of sensory nerve potential amplitudes in

[Bilateral idiopathic carpal tunnel syndrome in a child under 10 years of age].

INTRODUCTION: The carpal tunnel syndrome is the most frequent compression neuropathy in the adult population, being very uncommon in children, particularly under the age of 10 years. The most frequent etiology is the lisosomal storage diseases, followed by anatomical disorders, trauma, intensive sports practice and some idiopathic cases among others. In this paper we are presenting a case of an idiopathic and bilateral carpal tunnel syndrome in a 9 years old boy. CASE REPORT: A 9 year-old patient suffering daily paresthaesias in both hands for a year, producing limitations in hand mobility wh

[Basic neurography and its diagnostic importance].

Nerve conduction studies are fundamental elements of the neurophysiological investigation of neuromuscular diseases. They provide information on peripheral nerve function. Knowledge of the biological and technical basis of the method is essential for the clinician to understand the place of nerve conduction testing in the diagnostic process. A characteristic feature of the nerve fibers is their ability to conduct electrical potentials. This conductivity changes in pathologic circumstances; therefore, the patient's nerve conduction data may be important if a neuromuscular disorder is suspected.

Neurophysiological investigations in restless legs syndrome and other disorders of movement during sleep.

Besides polysomnographic techniques, other neurophysiological methods have been utilized in order to understand the pathophysiology of restless legs and periodic limb movement syndromes. By using electromyography with nerve conduction velocity (EMG-CV) and somatosensory evoked potentials (SEPs) it has been possible to determine how frequently each muscle was involved in periodic limb movements, how frequently EMG activity started in a given muscle and the time delay and pattern of activation between the first and the other activated muscles. The etiology of symptoms does not involve structural

An introduction to electromyography: an invited review.

Electromyography is a complex diagnostic test that is useful in diagnosing many neuromuscular and musculoskeletal conditions. Physiatrists and neurologists become familiar with this test during their training, while other physicians have minimal or no exposure, outside of perhaps some basic physiology lectures. A review of the anatomy, physiology, and physics upon which electrodiagnostic testing is based and of the various techniques used during a study provides the necessary foundation for understanding and using study results. A discussion of the conditions for which electrodiagnostic testin

Therapy of CIDP and related immune-mediated neuropathies.

This study assesses the data supporting the current use of immunotherapy for management of neuropathies with a presumed autoimmune basis. Immune or inflammatory mechanisms are implicated in an increasing number of disorders that involve damage to peripheral nerves and sensory ganglia. The most prevalent of these is chronic inflammatory demyelinating polyradiculoneuritis (CIDP). CIDP is clinically distinguishable from a series of other immune- or inflammation-mediated neuropathies. These include the following: multifocal acquired demyelinating sensory and motor (MADSAM) neuropathy associated wi

Compound nerve action potential of common peroneal nerve recorded at fibular neck: its clinical usefulness.

OBJECTIVE: To determine the clinical usefulness of the compound nerve action potential of the common peroneal nerve recorded immediately distal to or at the fibular neck, at the same time stimulating the common peroneal nerve laterally at the posterior knee. DESIGN: For recording the compound nerve action potential of the common peroneal nerve, the active recording electrode was secured just distal to or at the fibular neck area, whereas the stimulating electrode was placed on the medial border of the lateral hamstring tendon at the level of the popliteal crease. To reduce shock artifacts, a

Proximal median neuropathy secondary to humeral neck fracture.

Median neuropathies proximal to the wrist are uncommon and usually result from penetrating injuries, fracture dislocation of the distal humerus, or compression by fibrous bands. A 66-year-old man suffered a comminuted fracture of the proximal humerus after a fall. Electrodiagnostic studies revealed a severe proximal median neuropathy and a mild distal radial mononeuropathy. Proximal median neuropathy rarely occurs in humeral neck fracture, mostly because the median nerve is not in close contact with the humerus proximally.

Facilitation of rhythmic events in progressive myoclonus epilepsy: a transcranial magnetic stimulation study.

Twelve subjects with progressive myoclonus epilepsy (PME) were studied with transcranial magnetic stimulation (TMS), using single and paired magnetic stimuli at different interstimulus intervals (ISIs), and polygraphic recording. Motor threshold (T) and silent period (SP) were normal. Paired TMS showed a loss of inhibition at 100-150 ms ISI and a marked facilitation at 50 ms ISI of conditioned motor evoked potential (MEP). Polygraphic analysis showed 20 Hz oscillatory activity over the sensorimotor area coupled to contralateral myoclonic jerks. These findings suggest a condition of increased s

[True neurogenic outlet syndrome: report of 2 cases].

True neurogenic thoracic outlet syndrome is caused by compression of the lower trunk of the brachial plexus usually by a cervical rib, fibrous band or an elongated transverse process of C7. We describe two cases of female patients (23 and 19 years old) with pain in the right superior limb and progressive muscular weakness and atrophy of the intrinsic muscles of hand. Electrodiagnostic studies showed reduced amplitude of compound muscle action potential of median nerve and decreased amplitude of ulnar sensory nerve action potential. Motor and sensory nerve conduction velocities were normal in b

Proximal Martin-Gruber anastomosis mimicking ulnar neuropathy at the elbow.

We present a case of Martin-Gruber anastomosis (MGA) mimicking conduction block between the above- and below-elbow sites of ulnar nerve stimulation. We review the anatomical and electrophysiological literature on this subject and discuss its clinical implications. The potential for a MGA to occur very proximally in the forearm and thus mimic ulnar neuropathy at the elbow is underrecognized. We recommend that a check for MGA be performed on all patients with an apparent conduction block at the elbow, and suggest that 3 cm distal to the medial epicondyle may be an optimal below-elbow ulnar nerve
