The authors report on indications and results of percutaneous surgical treatment of trigeminal neuralgia based on own experiences in 1124 cases. In 912 cases percutaneous thermorhizotomy and in 212 cases percutaneous microcompression have been used. A rare neurovascular complication of a antebrachial arteriovenous fistula in a chronic hemodialysis patient is reported. A large brachial venous pseudoaneurysm caused median neuralgia by direct compression of the nerve. Surgical resection of the pseudoaneurysm resulted in complete relief of neuralgia.

Management of established postherpetic neuralgia should proceed on several fronts simultaneously and may include pain-oriented treatments that are pharmacologic, neuroaugmentative, anesthetic, physiatric, psychological, and surgical. Many patients develop a pattern of abnormal illness behavior, manifesting loss of interest in work or avocations, social withdrawal, and disturbance of family roles. Improvement in these impairments must be a goal that is as important as amelioration of the pain itself.

140 patients suffering from trigeminal neuralgia are evaluated. 22 patients were treated by symphatholytic and parasympatholytic drugs and the remaining 118 were operated on by peripheral neurectomy and nerve avulsion. In case of involvement of the inferior alveolar nerve, Thoma's, Poppen's and Ginwalla's method were tried. All cases were critically evaluated and followed for a period of 6 months to 5 years. Patients with trigeminal neuralgia may be treated in many ways before referral for definitive surgery. This aspect has been looked at in a group of 140 consecutive patients. Patients on prolonged medication have been studied with respect to the period over which the pain occurred and the amount of carbamazepine taken. Earlier referral for surgery is recommended for some of these patients and guidelines are given.

A concept for the treatment of trigeminal neuralgia is presented. On the basis of distinctive pain patterns, localized areas of pathosis within the jawbones are detected and obilterated. To date, we have treated eight patients with idiopathic trigeminal neuralgia. All patients have experienced total or near total abatement of pain.

Forty otherwise healthy patients over 50 years of age with early, severe painful herpes zoster were randomly allocated to two groups for treatment. Twenty patients received prednisolone 40 mg daily with gradual reduction over a period of 4 weeks, whilst the other twenty patients received carbamazepine 100 mg four times daily. Facial neuralgia appears in a variety of forms which have different fundamental pathophysiological mechanisms. Of decisive importance are neuralgias with sensitive trigeminal, intermediate (sensory root), glossopharyngeal and vagus nerves which are caused by functional disturbances or damage to the nerve.
