Shortening the time to the diagnosis of amyotrophic lateral sclerosis: role of the neurologist.

Early diagnosis is particularly relevant in the diagnosis of amyotrophic lateral sclerosis (ALS) as new therapies, which may have a direct impact on the outcome of the disease, are being developed. Many ALS patients go to non-neurological specialists rather than neurologists for their first consultation. Neurologists specialized in the field of neuromuscular diseases therefore have a duty to help general neurologists and other non-neurological specialists to recognize the disease. This may be achieved by disseminating the El Escorial/Airlie House clinical and electrophysiological criteria for

[Non-epileptic paroxysmal movement disorders].

Non-epileptic paroxysmal dyskinesias present with different forms of extrapyramidal hyperkinesias (dystonia, chorea, athetosis, ballism) in variable combinations and with cerebellar signs, respectively. They may be classified as: 1. paroxysmal dystonias/choreoathetoses (paroxysmal dystonic choreoathetosis = PDC), paroxysmal kinesigenic choreoathetosis = PKC, intermediate form) and 2. paroxysmal ataxias (PA) (PA with myokymia and neuromyotonia, azetazolamide-responsive PA). Nocturnal paroxysmal dystonia is now regarded as one form of nocturnal frontal lobe epilepsy. Research in molecular geneti

Neurophysiologic evaluation of lower motor neuron damage in tetraplegia.

We quantitatively investigated the extent of damage to motor neurons in tetraplegic subjects. Numbers of motor units in the patients were significantly lower for thenar, wrist extensor, and biceps brachii as compared to controls. Reduction in counts occurred even when M-response amplitudes were normal. Standard electromyography suggested a surprising frequency of lower motor neuron dysfunction below the level of injury. These results confirm previous reports and add data on motor units in the biceps brachii.

Sclerosteosis: report of a case in a black African man.

Sclerosteosis is a rare genetic disorder of bone modelling, similar to, but distinct from, van Buchem disease; it has been described almost exclusively in Afrikaners of South Africa, a white population of Dutch ancestry. Isolated cases have been reported in a girl in Japan, a boy in Spain, and in multiracial families in Brazil and USA. Here we report a case of sclerosteosis in a black man born in Senegal. He presented with the full features of the disease: tall stature; syndactyly: nail dysplasia; massive sclerosis of the long tubular bones, the ribs, the pelvis and the skull; multiple cranial

Protective coating of cranial nerves with fibrin glue (Tissucol) during cranial base surgery: technical note.

OBJECTIVE: Cranial nerve deficit, either transient or permanent, is a common postoperative complication after cranial base surgery. Frequently, this occurs because intracranial nerves are directly involved in the cranial base lesion. However, any cranial nerve adjacent to the lesion can be damaged because of direct or indirect manipulation during surgery, leading to severe morbidity. We describe a new technique in which the adjacent intracranial nerves are protected from surgical trauma by coating the nerves with a biological two-component fibrin glue (Tissucol; Immuno A.G., Vienna, Austria).

[Examination of the unconscious patient].

The unconscious patient presents an challenging emergency in the intensive care unit. Patient evaluation and treatment must occur simultaneously. After checking vital signs, deciding about cardiopulmonary resuscitation or intubation, an intravenous line must be established, blood samples collected and glucose and thiamine should be given. The physical and neurological examination should be performed in a efficient way and must consider the following eight points: best reaction to loud commands, best motor reaction to noxious stimuli, respiratory pattern, size and reactions of pupils, eye movem

Ectasia and fenestration of the anterior cerebral artery associated with persistent trigeminal artery: case report.

Multiple vascular anomalies in a 22-year-old patient who had signs and symptoms of vestibulocochlear dysfunction are reported. Angiography revealed a fenestration of the proximal segment of the left anterior cerebral artery, an ectasia of the left anterior and posterior cerebral arteries, and a persistence of the right trigeminal artery. The rarity of the association is documented, and the clinical significance is discussed.

Autonomic nervous system disorders in Parkinson's disease.

This paper gives a review on the clinical features of autonomic failure which occur in idiopathic Parkinson's disease (PD), mainly based on the recent publications, especially from Japan. In 1817, James Parkinson already mentioned bowel and bladder dysfunctions in his original article. Since then, a number of clinical studies have disclosed the occurrence of autonomic failure in this disease. It is known that relatively minor degrees of orthostatic hypotension are not rare, but other features of autonomic failure are rather uncommon, generally mild in severity and nonprogressive. Orthostatic h

[The Marinescu-Sjögren syndrome].

The patient, 54 years old, has an evolutive bilateral congenital cataract, with lens opacities, initially placed in posterior cortex and which developed to a total cataract. The patient also present oligophrenia, spinocerebral ataxia and trophic disturbances with kyphoscoliosis and extremities deforms. We insist on the research of the trophic and neurological disturbances at the patients with congenital cataract for the elucidation of some etiopathogenic aspects in the degenerative syndrome described by Marinescu.

Complex partial status epilepticus of extratemporal origin: report of a case.

A 28-year-old woman with no history of seizure was 7 months pregnant when she developed a prolonged complex partial status epilepticus (CPSE) organized in recurrent complex partial seizures of occipital origin, which was ascertained by the presence of elementary visual hallucinations and nystagmus heralding the attacks. EEG demonstrated recurrent seizures starting from the right occipital area. This especially refractory case of CPSE resolved after treatment with antiepileptic drugs and termination of pregnancy by cesarean section.

Congenital vertical ocular motor apraxia.

The case of a 4 1/2-year-old boy with congenital vertical ocular motor apraxia who was otherwise developmentally and neurologically normal is reported. The presence of perinatal hypoxia in this patient may have been etiologic. While the presence of a supranuclear vertical ocular motor abnormality usually suggests a serious, acquired neurologic or systemic disease it may rarely occur as an isolated congenital finding, as demonstrated in this case.
