Neurophysiological markers that predict and track treatment outcomes in childhood anxiety.

The present study examined the cortical processes that mediate cognitive regulation in response to emotion-eliciting stimuli, before and after anxious children participated in a cognitive behavioral therapy program. Electroencephalographic activity was recorded from anxious children (n = 24, 8 males) and comparison children (n = 16, 7 males) at pre-and post-treatment sessions. The change in anxiety T-scores from pre- to post-treatment was used to signify clinical improvement among anxious children (Improvers: n = 11 vs. Non-improvers: n = 13). Event-related potential components were recorded w

Clinical and neurophysiological aspects of anatomical variants in dorsomedial hand innervation.

OBJECTIVE: Three patients presented with painful paraesthesias in the dorsomedial aspect of the hand after removal of a ganglion cyst of the dorsal lower third of the forearm. This hand territory is usually innervated by the dorsal cutaneous branch of the ulnar nerve (DCU) but here it was innervated by a branch of the radial superficial nerve (RS). Consequently, we aimed to examine the frequency of this variant in the general population. METHODS: Both RS and DCU were stimulated in the forearm and sensory response recorded from the dorsomedial skin area of the hand in the three patients and 10

[Present status and future development of neurophysiological examination in laboratory medicine].

In 2011, nine brain and six nerve and muscle physiological examinations were covered by the national health insurance in Japan. The greatest recent change was the digitalization of electroencephalography in the neurophysiological laboratory. This change is good for hospital affairs both economically and ecologically. Board certified doctors or technologists by the Academic Society in Neurophysiology are increasing each year and are improving the clinical level in daily laboratory work. One problem is that the level of neurophysiological laboratory examinations differs among hospitals in Japan,…

The neurophysiological basis of the integration of written and heard syllables in dyslexic adults.

OBJECTIVE: Letter-speech sound integration in fluent readers takes place automatically and is dependent on temporal synchrony between letters and sounds. In developmental dyslexia, however, letter-speech sound associations are hard to learn, compromising accurate and fluent reading. We studied the effect of printed text on processing speech sounds in dyslexic and fluent adult readers. METHODS: Visual stimuli were presented with sequences of spoken syllables including vowel or consonant changes, or changes in syllable intensity, frequency, or vowel duration. As visual material, written syllabl

Ataxic hemiparesis: neurophysiological analysis by cerebellar transcranial magnetic stimulation.

The aim of this study was to investigate physiological mechanisms underlying ataxia in patients with ataxic hemiparesis. Subjects were three patients with ataxic hemiparesis, whose responsible lesion was located at the posterior limb of internal capsule (case 1), thalamus (case 2), or pre- and post-central gyri (case 3). Paired-pulse transcranial magnetic stimulation (TMS) technique was used to evaluate connectivity between the cerebellum and contralateral motor cortex. The conditioning cerebellar stimulus was given over the cerebellum and the test stimulus over the primary motor cortex. We st

Decoding ensemble activity from neurophysiological recordings in the temporal cortex.

We study subjects with pharmacologically intractable epilepsy who undergo semi-chronic implantation of electrodes for clinical purposes. We record physiological activity from tens to more than one hundred electrodes implanted in different parts of neocortex. These recordings provide higher spatial and temporal resolution than non-invasive measures of human brain activity. Here we discuss our efforts to develop hardware and algorithms to interact with the human brain by decoding ensemble activity in single trials. We focus our discussion on decoding visual information during a variety of visual

An evaluation of neurophysiological criteria used in the diagnosis of motor neuron disease.

BACKGROUND: New criteria for the neurophysiological diagnosis of amyotrophic lateral sclerosis/motor neuron disease (ALS/MND) were recently proposed at an international symposium in Awaji-shima, Japan. They differ from the accepted revised El-Escorial criteria by considering fasciculation potentials to be evidence of acute denervation. In addition, when assessing diagnostic certainty, the Awaji-shima criteria equate electrodiagnostic evidence of lower motor neuron dysfunction with clinical examination findings. METHODS: A retrospective review of 205 consecutive sets of notes was performed, fr

Sonography as an aid to neurophysiological studies in diagnosing tarsal tunnel syndrome.

Diagnosing tarsal tunnel syndrome can be difficult because of varying clinical diagnostic criteria and equivocal physical signs. We present a case of tarsal tunnel syndrome where nerve conduction identified distal tibial neuropathy and high-resolution sonography was able to show nerve swelling within the tarsal tunnel.

A neurophysiological study of myoclonus in patients with DYT11 myoclonus-dystonia syndrome.

Mutations in the epsilon-sarcoglycan (SGCE) gene have been associated with DYT11 myoclonus-dystonia syndrome (MDS). The aim of this study was to characterize myoclonus in 9 patients with DYT11-MDS presenting with predominant myoclonus and mild dystonia by means of neurophysiological techniques. Variously severe multifocal myoclonus occurred in all of the patients, and included short (mean 89.1 +/- 13.3 milliseconds) electromyographic bursts without any electroencephalographic correlate, sometimes presenting a pseudo-rhythmic course. Massive jerks could be evoked by sudden stimuli in 5 patients

Neurophysiological pathways to obesity: below awareness and beyond individual control.

A global obesity epidemic is occurring simultaneously with ongoing increases in the availability and salience of food in the environment. Obesity is increasing across all socioeconomic groups and educational levels and occurs even among individuals with the highest levels of education and expertise in nutrition and related fields. Given these circumstances, it is plausible that excessive food consumption occurs in ways that defy personal insight or are below individual awareness. The current food environment stimulates automatic reflexive responses that enhance the desire to eat and increase c

Carpal tunnel syndrome in young adults--an ultrasonographic and neurophysiological study.

BACKGROUND: The carpal tunnel syndrome (CTS) is by far the most frequent compression neuropathy and encompasses 45% of all non-traumatic nerve lesions. Women are affected twice as often as men and manifestation usually occurs over the age of 30; 76% of all patients become symptomatic between the age of 40-70 years. In young adults typical diagnostic clues suggestive of a CTS, may be absent leading to misdiagnosis and late treatment. PATIENTS AND METHODS: 30 patients suffering from CTS were subdivided in 2 groups according to age. Patients under the age of 35 were allocated in group A whereas

Creutzfeldt-Jakob disease presenting as corticobasal degeneration: a neurophysiological study.

Creutzfeldt-Jakob disease (CJD) can occasionally present with a clinical picture resembling a corticobasal degeneration (CBD). Transcallosal inhibition, as tested by focal transcranial magnetic stimulation, is frequently absent or highly disrupted in CBD patients. We report a case of CJD presenting at the beginning of the disease as a CBD in which the ipsilateral silent period (iSP) was present and well detectable. This brief report shows that study of the iSP may be a useful diagnostic tool in order to differentiate CBD from syndromes presenting with similar clinical features.

Neurophysiological evaluation of the optimum target in gamma thalamotomy: indirect evidence.

Gamma thalamotomy has been useful for the treatment of Parkinson's disease and other movement disorders, but it has disadvantages, such as a delayed clinical effect after irradiation and the inaccuracy of targeting because depth recording is not available. Therefore, we sought to determine the optimum target in gamma thalamotomy based on the results of conventional selective thalamotomy with reference to the AC-PC line. To obtain indirect support for the appropriateness of the optimum target, we performed depth recording around the region of the estimated optimum target based on the results of

Neurophysiological evidence for cortical discrimination impairment of prosody in Asperger syndrome.

Asperger syndrome (AS), belonging to the autism spectrum of disorders, is one of the pervasive developmental disorders. Individuals with AS usually have normal development of formal speech but pronounced problems in perceiving and producing speech prosody. The present study addressed the discrimination of speech prosody in AS by recording the mismatch negativity (MMN) and behavioural responses to natural utterances with different emotional connotations. MMN responses were abnormal in the adults with AS in several ways. In these subjects, fewer significantly elicited MMNs, diminished MMN amplit

Neurophysiological correlates of age-related changes in working memory capacity.

Cognitive abilities such as working memory (WM) capacity decrease with age. To determine the neurophysiological correlates of age-related reduction in working memory capacity, we studied 10 young subjects (<35 years of age; mean age=29) and twelve older subjects (>55 years of age; mean age=59) with whole brain blood oxygenation-level dependent (BOLD) fMRI on a 1.5 T GE MR scanner using a SPIRAL FLASH pulse sequence (TE=24 ms, TR=56 ms, FA=60 degrees , voxel dimensions=3.75 mm(3)). Subjects performed a modified version of the "n" back working memory task at different levels of increasing workin

[Clinical and neurophysiological aspects of cognitive disturbances in patients with panic disorders].

Auditory event-related potentials P300 were studied in 44 patients with panic disorders (PD) and 23 normal controls. Compared to the controls, the patients differed significantly by the reduced P300 wave amplitude and alteration of its habituation. PD patients also demonstrated more pronounced anxiety, depression, autonomic dysfunction, hyperventilation and attention insufficiency. The patients with atypical PD differed from those with typical PD by reduced P300 wave amplitude, higher depression and lower personality anxiety levels. The data obtained demonstrate that cognitive function disturb
