Battlefield neurosurgical care in the current conflict in southern Afghanistan.

An international military campaign involving large numbers of troops is ongoing in Afghanistan. To support the military efforts in the conflict zone, a network of military medical services of varying levels has been established. The largest and busiest multinational military hospital in southern Afghanistan is located at Kandahar Air Field where the only neurosurgeon is based. This report outlines the contribution of multinational military health services and the workload of the neurosurgical service in Kandahar.

A therapeutic update on PARP inhibitors: implications in the treatment of glioma.

Central nervous system (CNS) cancers are among the most aggressive and devastating. Further, due to unavailability of neuro-oncologists and neurosurgeons, the specialized treatment options of CNS cancers are still not completely available in most parts of the world. Among various strategies of inducing death in cancer cells, inhibition of poly(ADP-ribose) polymerase (PARP) has emerged as a beneficial therapy when combined with other anticancer agents. In this review, we provide a detailed therapeutic update of PARP inhibitors that have shown clinical activity against glioma.

Neurogenic stress cardiomyopathy following aneurysmal subarachnoid hemorrhage in a very elderly patient--case report.

A 90-year-old woman presented with aneurysmal subarachnoid hemorrhage (SAH) corresponding to Hunt and Hess grade II. Acute congestive heart failure and pulmonary edema developed following uneventful surgical clipping. Serial electrocardiography and echocardiography led to a diagnosis of neurogenic stress cardiomyopathy (NSC), also known as tako-tsubo cardiomyopathy. The outcome was favorable after supportive therapy with respiratory management and diuretic administration. Neurosurgeons treating SAH must take into account the various general treatment strategy options on a case by case basis af…

Metronidazole induced encephalopathy in a patient with brain abscess.

Metronidazole is commonly used for brain abscess but is not well known for its neurotoxic complications. Metronidazole-induced encephalopathy (MIEP) is toxic encephalopathy associated with the use of metronidazole. We experienced a case of brain abscess which developed reversible severe MIEP during treatment period. Although MIEP occurs in typical locations, it is not easy to differentiate from other conditions such as cerebral infarction, demyelinating diseases and metabolic diseases. Neurosurgeons should be aware that severe MIEP can occur during the use of metronidazole though it is not com…

Intraoperative uses of ultrasound in the pediatric neurosurgical patient.

Ultrasound imaging has become a common method for intraoperative evaluation of the central nervous system. Real-time monitoring aids in guidance for aspiration of fluid collections and placement of catheters as well as the localization and evaluation of masses, and confirmation of their complete removal. Color Doppler is useful in the evaluation of blood flow in arteriovenous malformations, aneurysms, and neoplasms. Consultation among the ultrasound staff, neuroradiologists, and neurosurgeons before the operative procedure maximizes the usefulness of ultrasound, thus aiding in the success of s…

Neurosurgical management of a painful subcutaneous nodule of the knee: angioleiomyoma.

BACKGROUND AND IMPORTANCE: Angioleiomyoma (ALM) is a rare, benign smooth muscle tumor that can occur anywhere in the body. Diagnosis is usua … ompletely resolved without any new neurological parasthesias or deficits on follow-up. CONCLUSION: Neurosurgeons should keep ALM in the differential diagnosis of painful extremity lesions. Fedor Krause: the first systematic use of X-rays in neurosurgery.

Within a few months of Wilhelm Conrad Röntgen's discovery of x-rays in 1895, Fedor Krause acquired an x-ray apparatus and began to use it in … al procedures and pathology. After the revolutionary discovery of x-rays by Röntgen, many prominent neurosurgeons seemed pessimistic about the use of x-rays for anything more than trauma or fractures. Krause immediately seized on its use to guide and monitor ventricular drainage and especially for the diagnosis of tumors of the skull base. The x-ray images cont …

Three-dimensional morphometry of the A1 segment of the anterior cerebral artery with neurosurgical relevance.

BACKGROUND: Despite research in the anatomical sciences for the last 200 years, some structures of the human body remain controversial or incompletely described. One of these structures is the A1 segment of the anterior cerebral artery (ACA). OBJECTIVE: To analyze the A1 segment of the ACA using novel stereoscopic methods because the 3-dimensional morphometry of the ACA is important to neurosurgeons. METHODS: A digital-image computer-based system was used to analyze the A1 segment of 230 ACAs derived from computed tomography. Data analysis included the inner diameter, length, and volume and …

The contributions of Otfrid Foerster (1873-1941) to neurology and neurosurgery.

BEST REMEMBERED FOR his description of the dermatomes in man, Otfrid Foerster was also an adept neurosurgeon and an innovative experimental neurophysiologist. As a neurologist, his contributions included conceptualizing rhizotomy as a cure for spasticity, anterolateral cordotomy for pain, the hyperventilation test in epilepsy, Foerster's syndrome, and the first electrocorticogram of a brain tumor. As a neurosurgeon, Foerster was able to excise intraventricular, hypophyseal, and quadrigeminal lesions and to perform epilepsy surgery under primitive conditions without clips, diathermy, or suction

Microelectrode Recording in DBS Surgery: An Educational Resource.

Microelectrode recording (MER) is a technique used during deep brain stimulation (DBS) surgery to guide electrode placement. After a stereotactic trajectory has been planned based on pre- and intraoperative imaging, a microelectrode capable of recording extracellular action potentials is inserted along the planned DBS electrode trajectory. By recognizing the characteristic features of the target structures, the intraoperative neurophysiologist and neurosurgeon work together to confirm that the trajectory is appropriate.

[Eye's traumatic enucleation : need for a multidisciplinary management].

We report a rare case of a 33 year-old man victim of a public assault resulting in bilateral enucleation of the orbits. The optic nerves were ripped or even pulled out with subsequent subarachnoïdal hemorrhage as well as chiasm lesions. Meaning castration, such an act may express a psychiatric or a neurologic disorder. The management must be multidisciplinary by a team that includes emergency doctors, radiologist, ophthalmologist, neurosurgeon as well as psychiatrist and neurologist.

Surgical approaches for resection of vestibular schwannomas: translabyrinthine, retrosigmoid, and middle fossa approaches.

Surgical removal remains one of the key treatment modalities for vestibular schwannomas. A team approach between a neurotologist and a neurosurgeon offers the patient the expertise of both specialties and maximizes the chances for an optimal outcome. Vestibular schwannomas can typically be resected through 1 of 3 main surgical approaches: the translabyrinthine, the retrosigmoid, or the middle fossa approaches. In this report and videos, the authors describe and illustrate the indications and surgical techniques for the removal of these tumors.

Anesthetic management for supratentorial tumor surgery.

The principles of the perioperative anesthetic management of patients with supratentorial tumors are reviewed. Emphasis is placed upon intracranial pressure (ICP) dynamics and the multiple causes of intracranial hypertension which may occur during the perioperative period. The author describes his experiences using routine ICP monitoring in these cases, arguing that this may help both the neurosurgeon and the neuroanesthetist to avoid many of the pitfalls currently associated with the perioperative care of these patients.

Advanced Surgical Techniques for Dural Venous Sinus Repair: A Comprehensive Literature Review.

The dural venous sinus (DVS) is a thin-walled blood channel composed of dura mater that is susceptible to injury during common neurosurgical approaches. DVS injuries are highly underreported, which is reflected by a lack of literature on the topic. Neurosurgeons should be familiar with appropriate techniques to successfully repair an injured DVS and prevent associated complications. This study presents a literature review on the surgical techniques for DVS repair after DVS injury during common neurosurgical approaches. The databases PubMed and Scopus were queried using the terms "cranial sinus…

A national survey on the management of foot drop secondary to lumbar degenerative disease in the United Kingdom.

PURPOSE: There is considerable variation in the management of foot drop secondary to lumbar degenerative disease (LDD) that occurs between centres and surgeons (spinal surgeons and neurosurgeons). The lack of standardised practice reflects the paucity in evidence base for management of this condition. In this survey, we aimed to assess current practice in the UK and identify the areas of variation. METHODS: A case-based survey was distributed to members of the Society of British Neurological Surgeons and British Association of Spine Surgeons through an online questionnaire. The survey consist…
