SIGNIFICANCE: The precise identification and preservation of functional brain areas during neurosurgery are crucial for optimizing surgical outcomes and minimizing postoperative deficits. Intraoperative imaging plays a vital role in this context, offering insights that guide surgeons in protecting critical cortical regions.
AIM: We aim to evaluate and compare the efficacy of intraoperative thermal imaging (ITI) and intraoperative optical imaging (IOI) in detecting the primary somatosensory cortex, providing a detailed assessment of their potential integration into surgical practice.

BACKGROUND: Fabry disease (FD) is a lysosomal storage disorder that causes glycosphingolipid deposition in the vascular endothelium. Early neurovascular involvement is difficult to detect because conventional magnetic resonance imaging (MRI) findings overlap with age-related small- and large-vessel changes. We hypothesized that integrating microvascular and macrovascular MRI markers could improve the detection of FD-related vasculopathy.
METHODS: In a prospective case-control study, 26 genetically confirmed FD patients and 26 age- and sex-matched healthy controls underwent three-Tesla MRI (3

BACKGROUND: Huntington's disease (HD) is marked by irreversible loss of neuronal function for which currently no availability for disease-modifying treatment exists. Advances in the understanding of disease progression can aid biomarker development, which in turn can accelerate therapeutic discovery.
METHODS: We characterised the progression of altered dynamics of whole-brain network states in the zQ175DN mouse model of HD using a dynamic functional connectivity (FC) approach to resting-state fMRI and identified quasi-periodic patterns (QPPs) of brain activity constituting the most prominent

Environmental hypoxia (fraction of inspired oxygen (FIO2) ∼ 0.120) is known to trigger a global increase in cerebral blood flow (CBF). However, regionally, a heterogeneous response is reported, particularly within the posterior cingulate cortex (PCC) where decreased CBF is found after two hours of hypoxic exposure. Furthermore, hypoxia reverses task-evoked BOLD signals within the PCC, and other regions of the default mode network, suggesting a reversal of neurovascular coupling. An alternative explanation is that the neural architecture supporting cognitive tasks is reorganised. Therefore, to

Transcranial direct current stimulation (tDCS) has been shown to modulate neural activity. Neural activity has been shown to be closely related, spatially and temporally, to cerebral blood flow (CBF) that supplies glucose via neurovascular coupling. Therefore, noninvasive and continuous monitoring of neural activity is possible with a measure of cerebral hemoglobin oxygenation using near-infrared spectroscopy (NIRS). In principal accordance, NIRS can capture the hemodynamic response to tDCS but the challenge remains in removing the systemic interference occurring in the superficial layers of t

The integration of signals from electro-encephalography (EEG) and functional magnetic resonance imaging (fMRI), acquired simultaneously from the same observer, holds great potential for the elucidation of the neurobiological underpinnings of human brain function. However, the most appropriate way in which to combine the data in order to achieve this goal is not clear. Here, we apply a novel route to the integration of simultaneously acquired multimodal brain imaging data. We adopt a theoretical framework developed in the study of neuronal population codes which explicitly takes into account th

BACKGROUND: NADPH oxidase (NOX), a primary source of endothelial reactive oxygen species (ROS), is considered a key event in disrupting the integrity of the blood-retinal barrier. Abnormalities in neurovascular-coupled immune signaling herald the loss of ganglion cells in glaucoma. Persistent microglia-driven inflammation and cellular innate immune system dysregulation often lead to deteriorating retinal degeneration. However, the crosstalk between NOX and the retinal immune environment remains unresolved. Here, we investigate the interaction between oxidative stress and neuroinflammation in g

Since neuronal activity is coupled with neurovascular activity, we aimed to analyze the cerebral blood flow hemodynamics during and following high-definition transcranial direct current stimulation (HD-tDCS). We assessed the mean middle cerebral artery blood flow velocity (MCA-BFv) bilaterally using transcranial doppler ultrasound, during and after HD-tDCS, in eleven right-handed healthy adult participants (6 women, 5 men; mean age 31 ± 5.6 years old), with no evidence of brain or cardiovascular dysfunction. The HD-tDCS electrode montage was centered over the right temporo-parietal junction. T

PURPOSE OF REVIEW: In 2001, approximately 250 robotic-assisted laparoscopic prostatectomies were performed; in 2007, this number is expected to approach 50,000 surgeries. This surge has arguably been driven by the improved (real or perceived) clinical outcomes. In this review we assess 'long-term' experience based on reports recently published over the past 18 months.
RECENT FINDINGS: The short-term clinical outcomes for robotic-assisted laparoscopic prostatectomies such as deaths, transfusions, postoperative complications and bladder neck contractures are excellent, and appear to be improved

Flare and hyperalgesia after intradermal capsaicin injection in human skin. J. Neurophysiol. 80: 2801-2810, 1998. We investigated the neurovascular mechanisms that determine the flare response to intradermal capsaicin injection in humans and delineated the associated areas of mechanical and heat hyperalgesia. The flare response was monitored both visually and with infrared telethermography. The areas of mechanical and heat hyperalgesia were determined psychophysically. Thermography detected very large areas of flare. As an early event underlying the flare and before onset of the area of rubor

Post-stroke recovery remains limited despite advances in acute reperfusion therapies, underscoring the need to better understand underlying mechanisms that shape repair. Microglia, the resident immune cells of the central nervous system, orchestrate responses to ischemic injury and critically influence neurovascular remodeling, axonal reorganization, and functional recovery. Emerging evidence indicates that inflammatory preconditioning can reprogram microglial responses to subsequent insults, yet the exact intracellular signaling pathways mediating this adaptive state remain incompletely defin

Traumatic brain injury (TBI) remains a major unmet clinical challenge owing to its complex pathophysiology, profound heterogeneity, and limited regenerative capacity of the adult central nervous system (CNS). Existing therapeutic interventions are largely restricted to supportive management and fail to adequately address the secondary injury cascade characterized by persistent neuroinflammation, oxidative stress, vascular dysfunction, and progressive neurodegeneration. Against this backdrop, stem cell-based therapies have emerged as a rapidly expanding area of regenerative medicine with the po

BACKGROUND: Primary angle-closure glaucoma (PACG) induces widespread central nervous system remodeling. However, the molecular, cellular, and neurochemical architectures driving macroscopic functional synchronization abnormalities in PACG remain unexplored.
METHODS: We employed a multiscale imaging-transcriptomics framework. Resting-state functional MRI was used to evaluate regional homogeneity (ReHo) in 44 PACG patients and 57 healthy controls. Voxel-wise ReHo alterations were spatially linked to whole-brain gene expression profiles from the Allen Human Brain Atlas, canonical cell-type-speci

Ischemic stroke damages the blood-brain barrier (BBB), worsening neuronal injury. Treatments to protect the BBB are limited. We evaluated the neurovascular protective capacity of Kisspeptin-54 in ischemic stroke using in vivo and in vitro models. In vivo, mice underwent middle cerebral artery occlusion (MCAO), and cerebral infarct volume, neurological function, and blood-brain barrier (BBB) permeability were evaluated. In vitro, human brain microvascular endothelial cells (HBMVECs) were exposed to oxygen-glucose deprivation/reperfusion (OGD/R) to assess the effects of Kisspeptin-54 on paracell

Cerebral perfusion plays a crucial role in maintaining brain function and is tightly coupled with neuronal activity. While previous studies have examined cerebral perfusion trajectories across development and aging, precise characterization of its lifespan dynamics has been limited by small sample sizes and methodological inconsistencies. In this study, we construct the first comprehensive normative model of cerebral perfusion across the human lifespan (birth to 85 years) using a large multi-site dataset of over 12,000 high-quality arterial spin labeling (ASL) MRI scans. Leveraging generalized

Metabolic Dysfunction Associated Steatotic Liver Disease (MASLD) is the most common chronic liver disease in Western countries. It is becoming increasingly evident that peripheral organ-centered inflammatory diseases, including liver diseases, are linked with brain dysfunctions. Therefore, this study aims to unravel the effect of MASLD on brain histology, cognitive functions, and neurotransmitters. For this purpose, mice fed for 48 weeks on standard (SD) or Western diet (WD) were evaluated by behavioral tests, followed by sacrifice and analysis of the liver-brain axis including histopathology,

Cerebral cavernous malformations (CCMs) arise when capillaries within the brain enlarge abnormally, causing the blood-brain barrier (BBB) to break down. The BBB serves as a sophisticated interface that controls molecular interactions between the bloodstream and the central nervous system. The neurovascular unit (NVU) is a complex structure made up of neurons, astrocytes, endothelial cells (ECs), pericytes, microglia, and basement membranes, which work together to maintain blood-brain barrier (BBB) permeability. Within the NVU, tight junctions (TJs) and adherens junctions (AJs) between endothel

Cockayne syndrome (CS) is a rare, autosomal genetic disorder characterized by premature aging-like features, such as cachectic dwarfism, retinal atrophy, and progressive neurodegeneration. The molecular defect in CS lies in genes associated with the transcription-coupled branch of the nucleotide excision DNA repair (NER) pathway, though it is not yet clear how DNA repair deficiency leads to the multiorgan dysfunction symptoms of CS. In this work, we used a mouse model of severe CS with complete loss of NER (Csa-/-/Xpa-/-), which recapitulates several CS-related phenotypes, resulting in prematu

Bladder neck (BN) dissection is considered one of the most challenging steps during robot-assisted radical prostatectomy. Better understanding of the BN anatomy, coupled with a standardized approach may facilitate dissection while minimizing complications. We describe in this article the 4 anatomic spaces during standardized BN dissection, as well other technical maneuvers of managing difficult scenarios including treatment of a large median lobe or patients with previous transurethral resection of the prostate. The first step involves the proper identification of the BN followed by slow horiz
