Divided nevus, also known as kissing nevus, is a rare variant of congenital melanocytic nevi in which there are two adjacent nevi in areas of the body that undergo embryonic cleavage. The original description of this type of lesion was on the eyelid. The location on the penis is even rarer, with only 17 case reports in the literature so far, and only one of them described the dermoscopic findings. We report the case of a patient with divided nevus of the penis and its clinical, dermoscopic and histopathological features.

Epidermal nevus syndrome (ENS) is a term that encompasses several phenotypes defined by the association of an epidermal nevus with one or more congenital systemic anomalies, mainly ocular, osseous and cerebral. The two most frequent, keratinocytic nevus syndrome and linear sebaceous nevus syndrome, also correspond to the neurological phenotypes. The term "dysplastic nevus" is a misnomer and should be abandoned. Dysplastic nevus is not just a name, it is the root of the concept that histomorphology (or any morphologic examination including dermatoscopy) is able to predict the fate of a benign melanocytic proliferation. There is no evidence that this hypothesis is true but there are observations that falsify it. Preferably a specific diagnosis should be made based on dermatoscopic pattern and, if this is not possible, on clinical or dermatoscopic grounds alone the term "nevus, not otherwise specified" should be used.

BACKGROUND: Divided nevus of eyelid is a rare form of congenital nevus. It can cause cosmetic and psychological problems. The aim of the treatment is to reliably remove the lesion with minimal functional impairment, to improve the cosmetics, and to prevent canceration.
METHODS: The paired nevi were treated with CO2 laser by local anesthesia. Ablation was stopped after all the visible pigment tissues were removed and normal tissues became visible, shaping the basilar part to get good cosmetic results, avoiding thermally damaging the lower dermis as far as possible to minimize the side effect.
