Treatment of neuro-ophthalmologic manifestations of tuberculosis.

In the absence of obvious pulmonary or disseminated tuberculosis, ocular and central nervous system (CNS) tuberculosis may represent a significant diagnostic challenge. Refinements in polymerase chain reaction techniques and neuroimaging have strengthened the battery of tests used to diagnose CNS and ocular tuberculosis, yet in many cases, the diagnosis remains one of exclusion; it may ultimately be determined through exacerbation by anti-inflammatory therapy with subsequent improvement by antitubercular medication treatment. Because of emerging drug resistance, at least a two-drug regimen is

Multiple sclerosis and neuro-ophthalmologic manifestations.

The authors report clinical features of ocular manifestations in patients with multiple sclerosis (MS), those that affect the visual sensory system and those that affect the ocular motor system. Disturbances of visual sensory function may precede, manifest coincidentally or follow the neurologic manifestations. Visual disturbances are common in MS and often a result of acute demyelinating optic neuropathy. Careful examination of MS patients, who have never suffered optic neuritis, may also reveal asymptomatic visual loss. Asymptomatic visual loss seems to be a universal feature of MS. Patients

Ophthalmologic findings in patients with nonsyndromic plagiocephaly.

The purpose of this article was to study the prevalence of strabismus and astigmatism in children with nonsyndromic plagiocephaly. Children with deformational plagiocephaly do not have an increased prevalence of strabismus compared with the general population but do have an increased prevalence of astigmatism, whereas children with nonsyndromic craniosynostotic plagiocephaly have an increased prevalence of strabismus and astigmatism. This observational cohort study retrospectively reviewed patients in a multidisciplinary craniofacial clinic between 1993 and 2001. Patients were referred for eva

[Thyroid orbital pathologies in ophthalmological settings].

OBJECTIVE: To illustrate the originality and diversity of an ophthalmologic pathology associated with a thyroid disease (OATD), we report 42 cases of patients hospitalised or having consulted an ophthalmologic department for this disease over a period of 4 years. METHOD: Each patient was assessed by an endocrinologist and an ophthalmologist. Data included the OATD severity score, date of occurrence compared with the thyroid disease and determination of thyroid function and immune abnormalities. RESULTS: Patients were aged a mean of 48.9 + 15.3 years; 65% were women and 39% smokers. OATD prec

Neuro-ophthalmologic complications of cataract surgery.

Neuro-ophthalmologic complications from cataract surgery are uncommon and include central nervous system toxicity, binocular diplopia, traumatic optic neuropathy and ischemic optic neuropathy. Retrobulbar blocks may be accidentally injected into the subarachnoid space with diffusion to the brainstem. This leads to cardiovascular, respiratory, and mental status compromise. Most patients have complete recovery with adequate support. Post-operative, binocular diplopia may occur secondary to anisometropia or previously unrecognized misalignment. Periocular injection may cause paresis or fibrosis o

Cyclophotocoagulation: a report by the American Academy of Ophthalmology.

OBJECTIVE: This document describes cyclophotocoagulation procedures for glaucoma and examines the evidence to answer key questions about patient selection, and efficacy of transscleral and endoscopic techniques. METHODS: A literature search conducted for the years 1968 to 2000 retrieved 130 citations. The author reviewed 34 of these articles and selected 19 for the panel methodologist to review and rate according to the strength of evidence. A Level I rating is assigned to properly conducted, well-designed, randomized clinical trials; a Level II rating is assigned to well-designed cohort and

[Ophthalmological manifestations of cavernous sinus thrombosis].

Thrombosis of cavernous sinus is one of the most grave diseases of cerebral venous system; its first signs most often manifest in the organ of vision. Pain in the orbit and the respective half of the head develop in the presence of general fatigue, chill, and fever. In parallel with this, venous dyscirculatory disorders in the eye and its appendages involve all oculomotor nerves in succession. Further progress of the disease leads to development of congestive optic disk. Involvement of the other eye is one of the most severe complications of cavernous sinus thrombosis. Further course and manif

Ophthalmologic screening of deaf students in Oregon.

PURPOSE: To determine the frequency of Usher's syndrome and other ocular disease in students receiving special education services for the deaf in Oregon and to assess the amount of existing ophthalmologic surveillance of this population. A special emphasis was placed on screening for Usher's syndrome. METHODS: From 1980-1990, a prospective two-center visual screening program of 231 deaf students in schools throughout Oregon was conducted using an ophthalmic questionnaire, complete eye examinations, and electroretinography. Students were between the ages of 10 and 21 years and participated on

Abnormal head posture: an ophthalmological approach.

BACKGROUND: An abnormal head posture (AHP) is an important clinical sign; it may be due to non-ocular or ocular causes. The literature on the subject is disperse, however. OBJECTIVE: To present a comprehensive discussion on the subject. CLINICAL CONDITIONS: AHP due to an ocular cause can be observed in conditions both not related or related to the extraocular motility. In the latter group, comitant deviations, incomitant deviations, supranuclear palsies, A and V pattern, nystagmus, and central nervous system disorders are included. Facial asymmetry can be a feature of a long-standing congeni

The clinico-ophthalmological spectrum of antiphospholipid syndrome.

Antiphospholipid syndrome (APS) is a hypercoagulable disorder with highly variable symptomatology including ocular manifestations. Antiphospholipid antibodies (aPL) are the serologic markers of this clinical entity. Our aim was to note the main associated ocular features of APS and elucidate which pathogenic mechanisms may participate. Retinal vascular thrombosis and different neuro-ophthalmologic manifestations, such as optic neuropathy and amaurosis fugax, may be considered as the ocular hallmarks of this syndrome. Ocular features due to aPL-induced thrombosis should be treated with anticoag

[Ophthalmologic findings in patients with sudden deafness].

UNLABELLED: The etiology of sudden hearing loss is still not known. Among other pathomechanisms, vascular disorders have been proposed as an underlying cause. We investigated patients with sudden hearing loss for signs of microcirculation disorders. PATIENTS: Thirty-one patients (15 males and 16 females, mean age 37.5 years) were admitted because of sudden hearing loss. In addition, all patients underwent thorough ophthalmologic examination. RESULTS: Refraction, vision, intraocular pressure and excavation of the optic nerve were normal. In 81% of all cases abnormal conjunctival vascular ecta

Ophthalmological significance of HLA associated uveitis.

The association between HLA-A29 and birdshot chorioretinopathy is the strongest association between HLA and disease ever described. The determination of HLA-A29 is therefore of diagnostic significance. The association between HLA-B27 and acute anterior uveitis (AAU) is much weaker. However, it is evident that B27+ AAU is clinically different from B27-AAU. Half of the B27+AAU patients have or will have ankylosing spondylitis or Reiter's syndrome. The determination of HLA-B27 is therefore of clinical significance and should be determined in each case of AAU. The B27+ patients should be referred

[Use of Pulfrich's phenomenon in ophthalmological diagnosis].

The Pulfrich effect is a simply observable visual stereo phenomenon arising when the visual latency for one eye is longer than for the another. In the paper the review of the literature devoted to the application of this phenomenon for the ophthalmological diagnosis is presented. The authors come to the conclusion that the Pulfrich effect can be successfully used in clinical practice as an indicator of many ophthalmological disorders especially of optic neuritis.

Neuro-ophthalmologic manifestations of Lyme disease.

Lyme disease is a tick-borne spirochetal infection characterized by skin rash, neurologic, cardiac, and arthritic findings. The authors report six patients with Lyme disease who had neuro-ophthalmologic manifestations. One patient had meningitis with papilledema, two had optic neuritis, and one had neuroretinitis. Three patients had sixth nerve paresis, two of whom cleared quickly, whereas multiple cranial nerve palsies and subsequent optic neuropathy developed in another. Early recognition of neuro-ophthalmologic findings can help in the diagnosis and treatment of Lyme disease.
