Effect of functional foot orthoses on first metatarsophalangeal joint dorsiflexion in stance and gait.

Reduction in first metatarsophalangeal joint maximum degree of dorsiflexion with dorsiflexion of the first ray has been proposed to be the predominant cause of hallux abducto valgus and hallux rigidus. We sought to determine whether orthoses made from a cast with the first ray plantarflexed and a 4-mm medial skive could increase the maximum degree of dorsiflexion in patients with functional hallux limitus in stance and gait. Forty-eight feet of 27 subjects were casted for orthoses with the first ray plantarflexed and in the customary neutral rearfoot position with locked midtarsal joint. First

Supportive devices for preventing and treating subluxation of the shoulder after stroke.

BACKGROUND: Supportive devices such as slings, wheelchair attachments and orthoses have been used to treat subluxation of the shoulder after stroke. OBJECTIVES: To investigate the effect of supportive devices in preventing subluxation, re-positioning the head of humerus in the glenoid fossa, decreasing pain, increasing function and adversely increasing contracture in the shoulder after stroke. SEARCH STRATEGY: We searched the Cochrane Stroke Group Trials Register (last searched on 22 March 2004). In addition, we searched the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochr

A new walking orthosis for paraplegics: hip and ankle linkage system.

For paraplegics, two major disadvantages of hip-knee-ankle-foot orthotic systems that have a medial single hip joint are the short stride and horizontal rotation of the pelvis. The authors consider the pelvic rotation is caused by two factors; one is the lack of a mechanism to assist hip flexion, and the other is fixed ankle joints that cause instability when the step length becomes longer. Users must rotate their pelvis to initiate a swing in their legs and to achieve stability by making their two legs as parallel as possible in order to avoid losing balance. To overcome those disadvantages,

Secondary gait compensations in individuals without neuromuscular involvement following a unilateral imposed equinus constraint.

Ankle equinus is the most commonly identified impairment of individuals with spastic hemiplegia (SH). However, it is not clear how equinus at the ankle may contribute to gait deviations at other joints. The purpose of this study was to determine what compensatory gait deviations may occur as a result of an imposed, unilateral equinus constraint. Gait data were collected on 12 adult subjects with and without one ankle constrained in equinus using a unique taping method. Knee extension at initial contact, knee extension in mid stance, and hip extension at terminal stance were all found to be sig

Characteristic gait kinematics in persons with lumbosacral myelomeningocele.

Thirty self-ambulatory children with mid-lumbar to low-sacral myelomeningocele who walked without aids and 21 control children were evaluated by three-dimensional gait analysis. Characteristic kinematic patterns and parameters in the trunk, pelvis, hip, knee and ankle were analyzed with respect to groups with successive weakness of the ankle plantarflexor, ankle dorsiflexor, hip abductor, hip extensor and knee flexor muscles. Extensive weakness of the plantarflexors resulted in kinematic alterations in the trunk, pelvis, hip and knee and in all three planes seen as knee flexion, anterior pelvi

Comparison of a dynamic and a hinged ankle-foot orthosis by gait analysis in patients with hemiplegic cerebral palsy.

We studied the effect of a dynamic ankle-foot orthosis (d-AFO) on gait in 12 hemiplegic cerebral palsy patients. Sagittal plane kinematic and kinetic data of walking with the d-AFO were compared with walking barefoot, walking in a hinged ankle-foot orthosis (h-AFO) with a plantarflexion block and normal values. All patients had excessive plantarflexion and initial toe contact when walking barefoot. The d-AFO did not improve gait significantly whereas the h-AFO did. The benefits of controlling plantarflexion by a longer lever arm fror the h-AFO to the proximal calf included a heel-toe gait patt

Calcitonin and spinal fusion.

The authors report on the efficacy of nonsurgical treatment of an older patient with a fractured odontoid process. The patient, an 85-year-old woman, had multiple medical problems that put her at an increased surgery-related risk. Therefore, an alternative approach was elected, including immobilization with a Philadelphia collar and the provision of calcitonin nasal spray. Bone union and clinical recovery were achieved within 8 weeks of initiating the nasal calcitonin therapy (12 weeks postinjury). Considering the patient's age, comorbidities, and the severity of the fracture, the recovery per

Use of ready-made insoles in the treatment of lesser metatarsalgia: a prospective randomized controlled trial.

Two insoles designed to treat primary lesser metatarsalgia were compared in terms of their effect on plantar pressures and the subjective symptom relief. A prospective single blind randomized trial of 8 weeks' treatment in 46 feet in 33 patients was performed. Subjective outcome measures were visual analogue pain scores and estimated compliance. Objective outcome measures were dynamic plantar pressures using the Musgrave Footprint System. In group 1 (Viscoped), 6 of 18 patients rated themselves much improved or somewhat improved, and in group 2 (Langer) the proportion was 12 of 15 (P = 0.02).

Rheumatoid arthritis. Hindfoot disease.

Rheumatoid arthritis frequently involves the hindfoot. It may produce a proliferative synovial hypertrophy around the tendons and a gradual destruction of the joints. Unfortunately, this part of the foot frequently is overlooked when caring for these patients who often have multiple areas of involvement giving pain and producing a deformity. Treatment consists of nonoperative measures such as proper footwear, physical modalities, and occasional injections of corticosteroids. It is important to preserve the function of the posterior tibial tendon and not allow the hindfoot to develop a valgus d

The VA-Cyberware lower limb prosthetics-orthotics optical laser digitizer.

Characterization of the residual limbs and limb segments of patients for prosthesis and orthosis design has principally been a subjective process, highly dependent upon the skill, level of training, and experience of the prosthetist/orthotist involved. Even with the application of computer-aided design (CAD) and computer-aided manufacturing (CAM) technologies in prosthetics and orthotics, residual limb/limb segment characterization has remained substantially subjective and dependent upon prosthetist/orthotist skill, training, and experience. To eliminate the variations and errors that frequent

[The lax juvenile flexible flatfoot--disease or normal variant?].

The spectrum of normal variations of children's feet is extremely broad and often difficult to separate from pathological conditions. Especially the flexible flatfoot normally disappears during growth, and even if it persists up to adult life, it hardly has any pathological significance. The natural course taken even by severe flexible flatfoot in children leads to results that are as good, if not even better than when surgery had been performed; therefore, more reticence with surgical treatment in such cases is advocated.

Elbow flexion contractures treated with serial casts and conservative therapy.

We report on the technique of serial casts to successfully reverse elbow flexion contractures in three patients. Patients considered for this technique had a traumatic injury to the elbow that resulted in residual soft tissue restrictions; their mean flexion deformity was 44 degrees, which had been present for an average of 6 months. Treatment consisted of traditional therapy methods to obtain initial stretch and elongation of tissue followed by application of a cylindrical fiberglass cast in the position of maximal elbow extension. The cast was worn for 3 to 5 days, then removed, and the enti

Conservative treatment of congenital dislocation of the hip in infants 1 to 9 months of age.

The results of conservative treatment of congenital dislocation of the hip (C.D.H.) in 630 neonates and infants aged 1 to 9 months with 1005 typical dislocations or subluxations are discussed. The 630 cases were divided into two groups. In group I, 120 neonates and infants aged 1 week to 2 months (195 hips) were treated by various methods of hip abduction (Von Rosen splint, clothes, pillows, plastic splints). The follow-up ranged from 1 to 13 years (mean 1.4 years). The results were excellent in 94.5%, good in 4%, fair in 1%, and poor in 0.5%. All 510 infants (810 hips) of Group II were treate

Biological factors and predictability of bracing in adolescent idiopathic scoliosis.

Biological factors that could have a predictive value in treating idiopathic scoliosis by brace were studied in 107 patients (102 girls and 5 boys). The mean age of the patients was 14 years 6 months, and the mean Cobb angle of the curves was 36 degrees at start of treatment. The Boston brace was used for an average of 1 year 6 months, and mean follow-up time after weaning was 3 years. Scoliosis with an apex of Th 10-12 proved to be the most favorable for the final result, with a mean correction of 2 degrees. All other curves remained unchanged. Patients who had a period of rapid growth just b

Biomechanical concepts in the treatment of ulcers in the diabetic foot.

The diabetic patient and the associated pedal abnormalities pose a particularly challenging problem for the doctor from both a management and a prevention standpoint. The sequellae of diabetes mellitus have both neurologic and vascular origins. Neurologically, the diabetic is predisposed to peripheral and autonomic neuropathy, while vascular aberrations may manifest anywhere in the arterial network. Secondary breakdown of cutaneous and osseous structures are the direct result of a chronic insensitivity to pain and a faulty healing mechanism. Healing is, at best, delayed due to this inborn erro

TEMPER: an acronym for ankle sprain rehabilitation.

As has been previously discussed, the incidence and resultant associated disabilities of ankle sprains have been well documented in the literature. The staggering statistics on long-term disability show that there is no such thing as a simple ankle sprain. The degree of disability is related to the extent of the initial injury as well as the follow-up medical care provided. It is this fact that requires a complete understanding of the injury as well as a proper treatment and rehabilitation program. One of the reasons cited for the long-term disability or lack of consistently good results in tr

Benign joint hypermobility with neuropathy: documentation and mechanism of tarsal tunnel syndrome.

Eleven consecutive patients with tarsal tunnel syndrome were reviewed. All had pes planus, 8 had calcaneal valgus hindfoot, and generalized benign joint hypermobility syndrome was evident in 9. Detailed enquiry was necessary to identify apparently trivial precipitating events. In 6 patients there was superimposed secondary reflex sympathetic dystrophy which tended to obscure symptoms of the causative tarsal tunnel syndrome. Wider recognition of this cause-effect mechanism and early management with orthotic footwear and intrinsic foot muscle exercises may obviate the need for surgical intervent

Plastic ankle-foot orthoses: evaluation of function.

Plastic ankle-foot orthoses (PAFOs) are worn by persons with hemiplegia to correct gait abnormalities such as foot drop during swing and insufficient pushoff during stance. A PAFO should resist plantarflexion sufficiently to provide toe clearance during the swing phase of gait without excessively increasing the knee bending moment during heelstrike. It should resist dorsiflexion during late stance to raise the heel to simulate gastrocnemiussoleus muscle group function. Five PAFOs were evaluated as to the amount of plantarflexion-dorsiflexion resistance that was provided when worn by hemiplegic

Tarsal tunnel syndrome.

In 14 patients tarsal tunnel syndrome was associated with varus heels and pronated, splayed forefeet. Review of the literature and the author's experience suggest that these conditions may be a common cause of the tarsal tunnel syndrome. Treatment of fixed varus deformities of the heel by outer heel wedges has been shown to be ineffective. Although outer heel wedges provide symptomatic relief in patients with relatively flexible hindfeet, surgical release of the flexor retinaculum behind the medial malleolus is the treatment of choice in patients who have the tarsal tunnel syndrome with planov
