The authors present the results of complex drug and orthopaedic treatment of children with imperfect osteogenesis. 40 patients were treated with a somatotropic hormone, calcitrin and vitamin D metabolites (oxydevit and dihydrocholecalciferol). In 20 of these patients corrective osteotomies of the long bones of the lower extremities combined with metal osteosynthesis with rods and massive plates were performed. In osteogenesis imperfecta the bones are brittle but the teeth, whose dentine contains the same genetic collagen as bone (type I), may be clinically normal. To investigate this paradox we have measured the amino acid composition of insoluble dentine collagen from 16 deciduous and 18 permanent teeth in control subjects and in 59 patients with different forms of osteogenesis imperfecta. Osteogenesis imperfecta is not in itself a contraindication to stapedectomy. Thirty stapedectomies were performed on 24 patients with osteogenesis imperfecta. Thin ossicles, crural fractures, and thick, mushy, granular footplates predominate in this condition. Paraplegia occurred in an adolescent girl with osteogenesis imperfecta after chiropractic manipulation. The child had been able to walk freely out of doors. Complete motor paralysis with sensory sparing resulted due to anterior compression of the cord by spondyloptotic cervical vertebrae. Reconstructed computerised tomography was very helpful in demonstrating the abnormality. Anterior and then posterior decompression relieved the tethered spinal cord and were supplemented with bone grafting. Early diagnosis and surgical treatment will prevent similar neurological accidents.

Osteogenesis imperfecta is one of the commonest congenital systemic diseases. The treatment of the fractures caused by the diseases and the growth deficiencies which result is mainly conservative. Taking 22 operations on patient suffering from osteogenesis imperfecta, the results of the treatment are reported and the cases in which an operation is indicated are discussed. As the osteosynthesis material selected, the intramedullary Rush pin has proved its worth. No serious complications were observed.

Anesthetizing a patient with osteogenesis imperfecta is an uncommon and challenging event for most anesthesiologists. The manifestations of this disease directly affect anesthetic management. The choice of anesthetic technique must be carefully considered in view of the abnormalities in such patients. Local or regional anesthesia is very useful in patients with osteogenesis imperfecta, but when general anesthesia is mandatory ketamine hydrochloride is often a good choice. Close monitoring during anesthesia and surgery is essential.

Segmentation and intramedullary rod fixation of the tibia have been found to be safe and effective procedures in osteogenesis imperfecta. An extensible rod is more difficult to insert than a solid rod due to the problem of obtaining an adequate fit distally, so the practical advantage of one type over the other is not as great as it is for the femur. When the procedure is used for the proper indications and with sufficient attention to detail, severe deformities can be corrected, thus allowing many children to become or remain ambulatory.

Surgical treatment is a safe adjunct to the management of the child with osteogenesis imperfecta. Its objectives are to control fractures and correct deformities. It may significantly improve the functional status of the child. The long-range aim of bioscience is to determine and correct this genetic defect and thereby avoid the need for orthopedic methods of managements.

Osteogenesis imperfecta is often regarded as a form of osteoporosis. In many cases, particularly those in whom the first fracture occurs outside the neonatal period, bones that have not been fractured may appear radiologically normal. In a group of 24 adults with osteogenesis imperfecta the thickness of the metacarpal cortex was normal but their bones were often slender. Osteoporosis is probably not an inevitable feature of such cases, and some of the radiological abnormalities reported may be the results of previous fractures and their treatment.
