Cost-conscious outpatient rehabilitation of 210 patients in a neuromuscular disease clinic was examined. The patients followed over a one-year period included: Duchenne Muscular Dystrophy, [50], Charcot-Marie-Tooth [42], Limb-Girdle Dystrophy [37], Spinal Muscular Atrophies [28], "Congenital" Myopathies [12], Friedreich's Ataxia [7], Polymyositis [7] and other related diseases. Nitrous oxide analgesia is a safe alternative to parenteral narcotics for outpatient surgical procedures. This type of analgesia is effective, well tolerated by most age groups, easily administered, rapidly reversible and associated with a low incidence of serious side effects. Hypoxia can be prevented by using fail-safe equipment, by observing prescribed limitations on nitrous oxide concentrations and by administering 100 percent oxygen on termination of nitrous oxide administration.

Despite several studies advocating the routine use of outpatient breast biopsy, most surgeons have been reluctant to adopt this practice. A procedure for the outpatient management of acute hemorrhoidal disease is presented. Treatment, using local anesthesia with hyaluronidase, is directed to (1) relieve pain, (2) decrease edema, (3) treat appropriately the internal hemorrhoidal disease, and (4) treat the external thrombosis if present. Adequate treatment of the acute disease will, in a significant proportion of patients, avoid further surgical intervention. Eperience with results of this modality of treatment in patients is presented.

Minoxidil is a potent orally administered vasodilator under investigation for use in severe hypertension. Fifteen patients with moderate to severe hypertension refractory to conventional antihypertensive drugs were treated with minoxidil on an outpatient basis. Propranolol and furosemide were administered concomitantly to control reflex tachycardia and fluid retention. We evaluated the effectiveness of a routine outpatient urinalysis screening program on a sample population of 2600 patients. The 189 abnormal urine results found in 182 patients were followed up by study of any new clinical and laboratory investigations or therapeutic modifications initiated on the basis of any abnormal test result. The urinalysis screening program appeared to have significant bearing on diagnosis or treatment in only 13 patients. BACKGROUND: Fentanyl-involved overdose deaths have surged, yet buprenorphine remains underutilized for opioid use disorder (OUD) treatment, partly due to the risk of precipitated withdrawal during buprenorphine initiation. Direct-to-inject (DTI) buprenorphine using weekly long-acting injectable formulations may reduce this risk with a gradual and sustained rise in serum buprenorphine levels.
