Central mechanisms of chronic pain (neuralgias and certain other neurogenic pain).

Almost all supratentorial intracranial lesions which may effectively control chronic pain in man are ineffective for acute pain. There are at least 10 sites at which lesions producing no somatic sensory loss have often stopped the peculiarly agonizing chronic pain of advanced cancer. They include: (a) inferior posteromedial or subcaudate (preinnominate) frontal white matter; (b) supracallosal portion of cingulum; (c) thalamotomy of centrum medianum and parafascicularis nuclei; (d) thalamotomy of linear parasagittal type separating connections between lateral specific and medial nonspecific sen…

Proposed Conditions for Pain Reprocessing Therapy Readiness: Beyond Phenotyping Neuroplastic Pain.

PURPOSE: This paper examines the clinical gap between identifying neuroplastic pain and achieving treatment success with Pain Reprocessing Therapy (PRT), proposing a structured readiness framework to guide clinicians in determining when patients are appropriately positioned to engage with this intervention for chronic primary pain. RECENT FINDINGS: Despite a randomized controlled trial demonstrating that approximately 66% of PRT participants achieved pain-free or nearly pain-free status at one year, no implementation guidelines exist for establishing patient readiness prior to initiating trea…

Neuroimmune crosstalk in osteoarthritis pain: from synovial inflammation to pain sensitization.

Pain in osteoarthritis (OA) is not explained adequately by cartilage loss or radiographic severity. Synovial inflammation, tissue damage, and neural plasticity interact across the joint, dorsal root ganglia, spinal cord, and brain to shape the intensity and persistence of symptoms. This mini review examines the synovium as an immune niche in OA pain and follows the signals that connect activated macrophages, fibroblast-like synoviocytes, mast cells, T cells, and damaged chondrocytes with nociceptor sensitization. Cytokines, chemokines, prostaglandins, nerve growth factor, complement fragments,…

Identification of high-impact chronic pain in adolescents with musculoskeletal pain.

High-impact chronic pain (HICP) describes pain that interferes substantially with daily functioning and is associated with diminished quality of life, yet brief tools to identify and track HICP in adolescents are lacking. The 3-item Pain, Enjoyment, and General Activity (PEG) scale derived from the Brief Pain Inventory offers a promising rapid measure, but its psychometric properties and an empirically derived HICP cut-off score based on PEG have not been established in pediatric chronic pain. The present study examined convergent validity, classification accuracy, and a PEG cut-off for HICP i…

Intact endogenous pain inhibition in complex regional pain syndrome type 1.

INTRODUCTION: Impaired endogenous pain modulation is a common characteristic of various chronic pain conditions and may be a key mechanism underlying the development of chronic pain in complex regional pain syndrome (CRPS). One way to assess endogenous pain modulation in humans is through conditioned pain modulation (CPM) paradigms. OBJECTIVES: This study aimed to assess CPM capacity in chronic CRPS type 1 and its potential association with the individual pain phenotype. METHODS: Sixteen individuals with chronic CRPS type 1 and 15 age- and sex-matched healthy controls (HC) were enrolled. Par…

PTSD symptoms, pain catastrophizing, and pain outcomes after acute orthopedic injury.

BACKGROUND: Posttraumatic stress disorder (PTSD) is associated with greater incidence of chronic pain. Pain catastrophizing often accounts for this association. Less is known about these relationships during the acute phase (1-2 months) following orthopedic traumatic injuries. We sought to understand which orthopedic traumatic injury-related PTSD symptoms were associated with acute pain and physical dysfunction and whether pain catastrophizing accounted for these associations. METHODS: This secondary analysis used baseline data from a multisite randomized controlled trial of an intervention f…

Acupuncture for pain and pain-related disability in deep infiltrating endometriosis.

OBJECTIVES: To evaluate the efficacy of acupuncture in relieving symptoms (dysmenorrhea, dyspareunia, pelvic pain and dyschezia) intensity, improving functional disability, reducing the number of days per months of dysmenorrhea, the frequency and the efficacy of analgesic use in deep infiltrating endometriosis (DIE). The safety profile was also evaluated. METHODS: The study sample was 34 patients with DIE; for 2 months (T-2, T-1) the women recorded diary notes on the numbers of days of menstruation, the presence, intensity, and disability related to dysmenorrhea, dyspareunia, pelvic pain, and…

[Comparison of pain intensity measurements among patients with low-back pain].

BACKGROUND AND PURPOSE:
Pain intensity is the most frequently assessed health domain in clinical studies among patients with low-back pain. Visual analogue scale (VAS) and Numeric rating scale (NRS) have been the mostly used measurement tools for pain intensity. We proposed to correlate these instruments to a generic health-related quality of life measurement tool in order to show the scale with superior clinical relevance.
. METHODS:
We used cross-sectional, convenience sampling. 120 patients with chronic low-back pain administered the 29-item Patient Reported Outcomes Measurement I…

Minimal Clinically Important Change of Movement Pain in Musculoskeletal Pain Conditions.

Movement pain, which is distinct from resting pain, is frequently reported by individuals with musculoskeletal pain. There is growing interest in measuring movement pain as a primary outcome in clinical trials, but no minimally clinically important change (MCIC) has been established, limiting interpretations. We analyzed data from 315 participants who participated in previous clinical trials (65 with chronic Achilles tendinopathy; 250 with fibromyalgia) to establish an MCIC for movement pain. A composite movement pain score was defined as the average pain (Numeric Rating Scale: 0-10) during 2 …

12. Vascular pain: Ischemic pain in the extremities and Raynaud's syndrome.

INTRODUCTION: Peripheral artery diseases (PAD) and Raynaud's syndrome are associated with substantial morbidity. PAD, through the restriction of blood flow to the extremities, may lead to critical limb ischemia with symptoms of pain at rest which may eventually progress to severe limb ischemia with gangrene. This serious and painful clinical condition requires extensive medical care, is limb-threatening and, in case of delayed or unsuccessful treatment, is associated with a high mortality rate. In Raynaud's syndrome, the blood supply to certain parts of the body, usually the fingers and toes a…

Caregiver Perspectives on Pain Sensitivity and Pain Experience in Rett Syndrome.

BACKGROUND: Although delayed or decreased responses to pain are commonly reported among caregivers of individuals with Rett syndrome (RTT), previous studies in relatively small samples have documented that caregivers are concerned about pain, particularly due to gastrointestinal and musculoskeletal conditions. AIMS: The purpose of the current study was to investigate in detail caregivers' perceptions of pain sensitivity, as well as the types, severity, and effect of pain experienced by individuals with RTT in a larger sample than previous studies. METHODS: A total of 51 caregivers (mostly mo…

Circulating polyunsaturated fatty acids, pressure pain thresholds, and nociplastic pain conditions.

OBJECTIVE: Polyunsaturated fatty acids (PUFAs) play a role in pain regulation. This study sought to determine whether free PUFAs found in red blood cells also play a role in nociceptive processing. We examined associations between circulating PUFAs and nociceptive thresholds to noxious mechanical stimuli. We also determined whether nociceptive thresholds were associated with nociplastic pain conditions. METHODS: This cross-sectional study used stored red bloods cells and data from 605 adult participants in the OPPERA-2 study of chronic overlapping pain conditions. In OPPERA-2 adults completed…
