Delta Storage Pool Deficiency: A Pediatric Case Report and Review of the Literature.

Platelet storage deficiencies are a heterogeneous group of bleeding disorders of variable severity caused by decreased number or content of platelet granules. We present the case of a 10-year-old patient with no personal history of previous bleeding who was admitted to the emergency department due to menorrhagia and mucocutaneous pallor. Common disorders of primary and secondary hemostasis were ruled out. Subsequently, a study of electron microscopy of platelets was performed, which reported the presence of alpha granules with a decreased number of dense granules. Currently, the patient receiv…

Left atrial myxoma complicated with multi-system embolization.

BACKGROUND: Atrial myxoma accounts for approximately 50% of all cardiac tumors. The majority of myxomas are located in the left atrium and present variable clinical manifestation. CASE PRESENTATION: A young man was transferred to our hospital with sudden onset of resting pain, pallor and numb in right leg. An atrial mobile mass was detected by transthoracic echocardiography. Anticoagulant and antithrombotic therapy were administered, a timely surgery was performed and the mass was confirmed as a myxoma. The patient did not discharge any discomfort post-operation. CONCLUSION: For patients wit…

Avascular necrosis of the jaws as initial presentation
of acute leukemia.

Leukemia is a malignant neoplasm of hematologic origin occurring from disorganized proliferation or increased life span of white cells. The initial manifestations of leukemia are generally signs of bone marrow failure, such as pallor, fatigue, fever, bleeding, and infection related to peripheral blood cytopenias. Few reports have described osteonecrosis, especially in the femoral head, as an initial manifestation of acute lymphoblastic leukemia. To the best of our knowledge, osteonecrosis of the jaw has not been reported as a first sign of leukemia. Here, two cases of osteonecrosis of the jaw …

[Traumatic optic neuropathy after fall with a bamboo stick].

Traumatic optic neuropathy (TON) is a rare and potentially vision-threatening condition caused by ocular or head trauma. The treatment of TON is controversial with no evidence-based guidelines. We describe a case of an eight-year-old boy, who lost his vision on the right eye after having had a trauma of his left lower eyelid when he fell with a bamboo stick. Vision loss was not detected until ten days after the accident, where there were no light perception and a relative afferent pupillary defect. We attributed the vision loss to TON and no treatment was given. Within months optic disc pallor…

Hepatic subcapsular hematoma: two neonates with disparate presentations.

Subcapsular hematoma of the liver rarely occurs in neonates and the diagnosis is often missed or delayed. We report two babies who had this uncommon condition in the early neonatal period. In the first baby, the hematoma was associated with ventouse delivery and presented with abdominal distension and worsening jaundice. In contrast, the other baby was relatively well, with progressive pallor as the only clinical finding. The former had no other identifiable risk factors, whereas the latter was confirmed as having classical hemophilia. The literature is briefly reviewed with regards to inciden…

Concussion or benign paroxysmal torticollis?

This case report describes a patient who presented to the trauma service on 3 occasions over the course of 2 years, each time with symptoms typical of concussion (e.g., crying, change in mentation, and vomiting). On more in-depth evaluation, it was discovered that the child had torticollis, pallor, and brief dizziness or vertigo with each episode. Benign paroxysmal torticollis is a periodic, paroxysmal syndrome that may be mistaken for the more common concussion. In addition to illustrating a uniquely pediatric neurological syndrome, this case demonstrates the importance of taking a careful hi…

Infantile choriocarcinoma in a neonate with massive liver involvement cured with chemotherapy and liver transplant.

A 6-week-old boy presented with fever, pallor, and hepatomegaly. Ultrasound showed a huge midline abdominal mass. β-human chorionic gonadotropin was markedly elevated, suggesting a diagnosis of infantile choriocarcinoma of the liver. A biopsy confirmed the diagnosis. The patient received 6 cycles of bleomycin, cisplatin, and etoposide with significant decrease in tumor size. However, the tumor remained unresectable. A donor liver became available, and the infant underwent successful liver transplantation. He received 2 posttransplant cycles of moderate dose of methotrexate. This case shows the…

[Benign paroxysmal torticollis of infancy: diagnosis and clinical evolution of six patients].

Benign paroxysmal torticollis of infancy is an episodic disorder that occurs suddenly and spontaneously in the first months of life. The patient's head tilts to one side for hours or days, and sometimes it can be associated with other symptoms like vomiting, sweating, pallor, irritability, headache and unstable gait. We report six patients with benign paroxysmal torticollis of infancy, whose symptoms began within the first six months of life and disappeared by five years of age. It is important that the physician be able to recognize this benign disorder to provide appropriate prognosis and no…

[A case of extra-adrenal pheochromocytoma with frostbite in extremities].

We describe a patient who presented frostbite in extremities in addition to characteristic symptoms, such as severe hypertension, sweating a … artery and loss of heat by frequent sweating attacks. Regarding cutaneous symptoms in this disease, pallor, acrocyanosis and cold extremities are commonly found. However, it seems that typical frostbite associated with pheochromocytoma has not been reported so far.

Therefore, it would seem worthwhile to investigate zinc in cases of disk pallor described as idiopathic or drug associated, and to investigate visual function in cases of severe malnourishment.

The treatment of uterine bleeding with vasopressin hormonogen (glypressin)--a pilot study.

In a pilot study 12 women with severe uterine bleeding received an intravenous injection of 0.2 mg of a vasopressin hormonogen, glypressin. In 11 of the patients a rapid reduction or cessation of blood loss were observed. Two patients had facial pallor following the injection but no other side effects occurred. Five of the older women with long histories of menorrhagia were placed on a regime of one injection of glypressin on the first day of menstruation for six months; in all of them the duration of bleeding was reduced to three days and the monthly blood loss became very slight. On the basi…

Control of scrotal colour in the vervet monkey.

The vervet monkey has a vivid blue scrotum which pales when the animal falls in social rank. Histology and electron microscopy showed dopa-positive melanocytes in the dermis each packed with fully melanised melanosomes. By transmitted light the scrotal skin was brown on a red background: by reflected light the colour was blue. Thus the blue colour is due to Tyndall scattering over a layer of melanin. Variation of scrotal colour was not due to changes in melanocyte number or dispersion of melanosomes. Pallor was induced by injecting fluid, and blueness could be restored by removing fluid. It is…

Burr Cell Hemolytic Anemia with Congenital Varicose Veins: a Distinct Case.

Burr cell hemolytic anemia is diagnosed through the identification of burr cells and thrombocytopenia on a peripheral blood smear, in conjunction with an increased reticulocyte count and elevated serum bilirubin levels. Burr cells, referred to as echinocytes, are a type of red blood cell that features short, evenly spaced spicules and a preserved central pallor. This condition is commonly associated with liver disorders or advanced kidney disease. Although the occurrence of burr cell hemolytic anemia in conjunction with varicose veins is rare, it may be indirectly related to liver damage. In t…

A Case Report of Timely Interventions in Postpartum Hemorrhage: The Role of Real-Time Surveillance, Uncrossmatched Blood Products, and Aortic Compression.

A 41-year-old G2P1 underwent a vaginal delivery complicated by a retained placenta, uterine atony, and a massive postpartum hemorrhage of 4000 mL. Despite hypotension, care was not escalated, possibly due to the absence of tachycardia, pallor, or mental status changes. While maternal early warning criteria failed to trigger an alert, an electronic real-time surveillance platform activated a team response. The patient became unresponsive with a systolic blood pressure of 47 mm Hg and severe hypoxemia. Aortic compression restored consciousness. She was resuscitated with uncrossmatched blood and …

Challenging diagnosis of spherocytosis in early infancy.

This case report describes an infant who presented to the emergency department with rhinorrhoea and severe mucocutaneous pallor. Laboratory evaluation revealed normocytic anaemia with reticulocytosis and a peripheral blood smear suggestive of peripheral red blood cell (RBC) destruction, although without the presence of spherocytes. Despite the absence of spherocytes on the peripheral blood smear, the clinical evaluation, including family history, led to the consideration of hereditary spherocytosis (HS). The diagnosis was confirmed through genetic testing, revealing a heterozygous pathogenic v…

Point-of-Care Ultrasound, an Unexpected Cardiac Mass in a Neonate.

Neonatal cardiac masses are uncommon and often not obvious based on physical examination or plain radiography. The objective of this clinical case report is to demonstrate how cardiac point-of-care ultrasound was pivotal in the clinical course of a seemingly well neonate with vague symptoms. A 6-week-old male infant presented to the emergency department with fatigue and pallor that was reported to have resolved before arrival. In the emergency department, he had a normal physical examination and stable vital signs. Cardiac point-of-care ultrasound was performed and demonstrated a mass near the…

An unusual case of chronic abdominal pain: an association between Celiac disease and Crohn's disease.

Inflammatory bowel diseases (IBD) and Celiac disease (CeD) are immune-mediated gastrointestinal diseases with incompletely understood etiology. Both diseases show a multifactorial origin with a complex interplay between genetic, environmental factors, and some components of the commensal microbiota. The coexistence of celiac disease with Crohn's disease is rarely reported in the literature. Here, we report a case of a 13-year-old Syrian male who presented with a history of abdominal pain, anorexia and pallor. CeD and Crohn's disease was documented on gastrointestinal endoscopy and histological…

I am blue: a toddler's Raynaud's phenomenon.

Raynaud's phenomenon (RP) is a well-known disorder of self-limiting paroxysmal vasospasms occurring in small arteries of the digits, in the order of skin pallor (white), followed by cyanosis (blue), ending with hyperemia (red). These designative triphasic colour changes with exposure to cold, or emotional response is diagnostic in adults. RP is a very rare phenomenon in the young paediatric population as noted by Nigrovic et al with 123 patients <19 year old in a large children's centre over 10 years and only 4 patients being <2 years old, with 69% of these being primary RP. To our knowledge, …
