Lifestyle interventions in type 2 diabetes (DM2) prevention implementation studies can be effective and lasting. Long-term weight loss maintenance enhances the intervention effect through a significant decrease in diabetes incidence over time. Our objective was to identify factors predicting long-term successful weight reduction maintenance achieved during a DM2 prevention program in patients with high DM2 risk in primary health care. Study participants (n = 263), middle-aged, slightly obese with baseline increased DM2 risk (Finnish Diabetes Risk Score (FINDRISC)>14), but no diabetes were invi

Although guidelines recommend strict blood pressure (BP) control in patients with type 2 diabetes mellitus (T2DM) and elevated cardiovascular risk, the long-term effects of this approach are unknown. We investigated the effect of intensive BP control on clinical outcomes in patients with T2DM over 9 years of follow-up. We included Action to Control Cardiovascular Risk in Diabetes - Blood Pressure participants in the standard glucose control arm who had established cardiovascular disease, chronic kidney disease, were ≥75 years of age or who had a 10-year coronary heart risk ≥15%. Participants w

BACKGROUND: Little data is available concerning the prognostic implications of renal function abnormalities, their evolution over time and the effects of nifedipine on such abnormalities in patients with stable angina pectoris.
METHODS: The previously published ACTION trial compared long-acting nifedipine GITS 60 mg once daily to placebo among 7,665 patients. Standard laboratory tests including creatinine and uric acid were assessed at baseline, after 6 months, 2 and 4 years, and at the end of follow-up. We assessed the impact of nifedipine on markers of renal dysfunction and determined wheth

PURPOSE: The oral complications associated with radiotherapy to the head and neck are a significant dose-limiting factor. The goals of this study were to determine whether oropharyngeal rinsing and ingestion of misoprostol protect mucous membranes from the acute effects of irradiation, and to evaluate the quality-of-life (QOL) outcomes of patients receiving misoprostol. We report the results of the QOL outcomes of patients in this study.
METHODS AND MATERIALS: A total of 33 patients with resected or intact cancer of the oral cavity, oropharynx, supraglottic larynx, or hypopharynx were registe

BACKGROUND: Consumer advocacies emphasize embedding lived experience into palliative care provision. However, tokenistic consumer involvement often hinder meaningful consumer participation in decision-making processes. This study explored stakeholder perception of patient and public involvement in palliative care, identifying barriers and enablers of effective consumer inclusion. METHODS: Using a qualitative story completion approach, we collected narratives written on a fictional consumer engagement scenario by six consumers, seven health professionals, and two palliative care researchers. INTRODUCTION: Treatment-resistant depression (TRD) exists when patient depression continues without remission or reduction despite treatment. There are no standardized guidelines for identifying TRD, but one failed antidepressant treatment at an adequate dose and duration can constitute TRD, especially in cases of severe depression or suicidality. TRD rates for depressed patients average approximately 50% to 60% of the general population. These numbers are higher in the military population and are often complicated by comorbidities.

BACKGROUND: Paediatric surgery is a stressful experience for patients and caregivers. While standardised protocols are the norm, patient-centred approaches are needed to empower patients/caregivers for an optimal perioperative pain experience. To address this gap, we employed a patient-centred approach using design thinking (DT) methodology to develop insights, map processes, identify opportunities and design solutions for individualised empowerment tools.
METHODS: In consultation with DT experts, a multidisciplinary team of stakeholders (healthcare providers, patients who underwent pectus ex

INTRODUCTION: Patient and Public Involvement (PPI) in mental health research is encouraged by funders in the United Kingdom, supported by guidance and best practice standards. This includes a commitment to learning and understanding impact. This paper explores the experiences of individuals who worked together for 7 years within an intervention study to understand differing perspectives and conceptualisations of how PPI shaped the research study.
METHODS: Using an instrumental case study design, qualitative data were collected in three workshops: university-based study team (n = 4), Lived Exp

The shared decision-making (SDM) tool developed by the World Federation of Hemophilia (WFH) provides a clear and concise overview of the process by which people with haemophilia (PwH) can collaborate with their healthcare professionals (HCPs) to engage and arrive at a therapeutic decision. This tool will be useful for all people with all bleeding disorders, not just PwH.

The present article critically analyzes the current approach to treatment outcome research and the uncritical use of treatment packages. Comparing various combinations of treatments with groups of clients randomly assigned to these conditions has led to the neglect of individual differences between persons falling into broad diagnostic categories. The inconclusive results of such studies say little about the efficacy of a particular treatment for an individual client and are also not very useful in helping clinicians to decide which specific treatment to use for a given client. Although treatm

INTRODUCTION: Treatment selection in haemophilia is increasingly challenging given evolving therapeutic options and the need for individualization. Shared decision-making (SDM) approaches have recently gained interest, though a synthesis of available studies is lacking.
AIM: A scoping review was conducted to summarize literature reporting on factors impacting treatment SDM in haemophilia and tools or models available to support such decisions.
METHODS: PubMed, Embase, the Cochrane Library, Web of Science and grey literature were searched for studies published through August 2023. Original st

BACKGROUND: Fibromyalgia Syndrome (FMS) is characterised by widespread and persistent pain, intrusive fatigue and cognitive issues, affecting approximately 5.4% of the UK population. Non-pharmacological therapies and education are current management recommendations, but these approaches rely on patients having an active role in their healthcare management. It is therefore important to identify the factors associated with FMS patients being active partners, as this could influence person-centred care provision.
AIM: The aim of this study is to explore the factors associated with patients being

The field of data analysis, preparation, and machine learning is rapidly expanding, offering numerous libraries and resources for exploration. Researchers gain knowledge through various channels, but few resources provide a comprehensive framework for building machine-learning models. We present a step-by-step framework for constructing a robust Random Forest classification model to fill this gap. Using the trained model, we predict if individuals visiting Sanoviv Medical Institute between 2020 and 2023 participated in the Lyme disease program based on age, symptoms, blood count, and chemistry

BACKGROUND: Many people deny science and reject health recommendations despite widely distributed facts and statistics. Didactic science and health communication is often dry, and relies on the false assumption that people make purely evidence-based decisions. Stories can be a powerful teaching tool by capturing attention and evoking emotion.
OBJECTIVE: We explore the impact and appeal of, and describe best practices for, using narrative (storytelling) versus didactic methods in science and health communication.
PATIENT INVOLVEMENT: No patients were involved in the review process.

INTRODUCTION: Meaningful patient involvement in health technology assessment (HTA) is essential in ensuring that the interests of the affected patient population, their families, and the general public are accurately reflected in coverage and reimbursement decisions. Central and Eastern European (CEE) countries are generally at less advanced stages of implementing HTA, which is particularly true for patient involvement activities. As part of the Horizon2020 HTx project, this research aimed to form recommendations for critical barriers to patient involvement in HTA in CEE countries.
