Anesthetic management of children with craniofacial anomalies.

Children with craniofacial anomalies, in particular those with facial clefts and the associated syndromes of Treacher Collins, Pierre Robin, and Klippel-Fiel and craniosynostosis and its related syndromes, present some of the most hazardous and difficult challenges that anesthetists may encounter within the entire practice of pediatric anesthesia. Through this review of the embryological development of the above-listed anomalies, the specific concerns of the airway and coexisting congenital defects, the reader will develop a better understanding of each anomaly in order to create a safe anesth…

Immune response: effects of operative stress in a pediatric model.

PURPOSE: The purpose of this study is to delineate the effect of different operative procedures on the cell-mediated immune response in a pediatric animal model using the delayed type hypersensitivity (DTH) skin test. METHODS: Sprague Dawley rats (1 week old) were sensitized against keyhole limpet hemocyanin (KLH). Animals were challenged 2 weeks later by an intradermal injection of KLH (0.3 mg) in sterile saline. Rats with positive DTH skin reactions at 24 and 48 hours after challenge (baseline) were divided randomly into five groups (n = 10 each): group I, unmanipulated control; group II, a

[Anesthesia with sevoflurane for tonsillectomy in a boy with Duchenne muscular dystrophy].

A 6-year-old boy with Duchenne muscular dystrophy (DMD) and foreseen difficult tracheal intubation underwent tonsillectomy under general inhaled anesthesia with sevoflurane. No neuromuscular blockers were administered and no perioperative complications emerged. In spite of advances in genetic diagnosis there continue to be patients with DMD because of spontaneous mutation of the dystrophin gene. Late detection leaves them vulnerable to administration of drugs like succinylcholine that can trigger fatal reactions involving hyperpotassemia, rhabdomyolysis, and malignant hyperthermia. Total intra

General Endotracheal Anesthesia vs Total Intravenous Anesthesia for Children Undergoing Low-Risk Cardiac Catheterization.

Historically, children undergoing cardiac catheterization have been managed with general endotracheal anesthesia (GETA). However, recent practice has favored general anesthesia with total intravenous anesthesia (TIVA). This study compares procedural outcomes in the pediatric cardiac catheterization laboratory (PCCL) for children undergoing low-risk cardiac catheterization with GETA vs. TIVA. We reviewed all patients aged 1-12 years, who underwent outpatient diagnostic or simple interventional catheterization from January 2016 to September 2023. Patients were divided into cohorts by type of ane

Standardized anesthetic protocol in pediatric Radiotherapy: A retrospective analysis of clinical efficacy and outcomes.

BACKGROUND AND PURPOSE: Radiotherapy plays a critical role in the treatment of pediatric cancers; however, the need for absolute immobilization during treatment necessitates the use of general anesthesia or deep sedation. MATERIALS AND METHODS: In this retrospective observational study conducted at Salerno University Hospital between 2022 and 2024, we evaluated a standardized anesthetic protocol based on the exclusive use of a laryngeal mask airway (LMA) in pediatric patients undergoing radiotherapy. Data were collected from 420 anesthesia sessions performed in 22 patients under 14 years of a

Laser gingivectomy for pediatrics. A case report.

Traditional gingivectomy procedures have been a challenge for pediatric dentists who confront issues of patient cooperation and discomfort. Treatment of pediatric patients must involve minimal operative and postoperative discomfort. Laser soft-tissue surgery has been shown to be well accepted by children. For the pediatric patient, the greatest advantage of the laser is the lack of local anesthesia injection and the associated pre- and postoperative discomfort. The following case report describes a gingivectomy procedure performed on a 14-year-old female.

Comparing the analgesic effects of ultrasound-guided caudal block and dorsal penile nerve block in pediatric concealed penis correction surgery: a randomized controlled trial.

BACKGROUND: Effective perioperative analgesia is critical for pediatric patients undergoing concealed penis correction surgery. Despite the utility of regional techniques like caudal block (CB) and dorsal penile nerve block (DPNB), evidence comparing their efficacy in this population remains limited. This study aimed to compare ultrasound-guided CB and DPNB for perioperative analgesia in pediatric concealed penis surgery. METHODS: In this prospective, double-blind, randomized controlled trial, 86 children (aged 5-12 years, ASA I-II) were allocated to CB (n = 44) or DPNB (n = 42) groups. All t

The impact of diurnal variations on emergence delirium following general anesthesia and surgery in children.

BACKGROUND: Emergence delirium (ED) is a widely recognized issue that prolongs mechanical ventilation and post-anesthesia care unit (PACU) resuscitation time, consequently increasing hospital costs and mortality. Postoperative disturbance in circadian rhythms, commonly leading to sleep disorders, has been identified as a significant risk factor for ED. However, the influence of surgery timing (morning vs. afternoon) on the incidence of ED in pediatric patients undergoing general anesthesia remains unknown. METHODS: Patients aged 2-6 years who were operated on under general anesthesia with a b

Short anesthesia without intravenous fluid therapy in children: Results of a prospective non-interventional multicenter observational study.

BACKGROUND: The German guidelines recommend that intravenous fluid therapy should not be mandatorily performed in children with short fasting times undergoing short anesthesia, but there is a lack of clinical studies including a large number of pediatric patients. Therefore, we performed a prospective non-interventional multicenter observational study to evaluate the perioperative hemodynamic and metabolic stability of children undergoing short anesthesia without intravenous fluid therapy. AIMS: The primary aim was to assess the incidence of hypotension and the secondary aim was to assess the

Bipolar hooked-wire electromyographic technique in the evaluation of pediatric vocal cord paralysis.

Laryngeal electromyography is a valuable test to assess vocal cord paralysis in adults. This technique can be applied and adapted to the pediatric patient. In the operating room under general anesthesia and endoscopic guidance, bipolar hooked-wire electrodes are passed percutaneously through the anterior neck skin into both thyroarytenoid muscles. Electromyographic signals are evaluated during light anesthesia and on awakening. A critical appraisal of this technique is presented, including an analysis of sources of electrical interference in the operating room.
