Surgical Extrusion of Anterior Teeth with Intrusion Traumatic Injury: A Report of two Cases.

Intrusion is the most severe luxation injury type, which results in both soft and hard tissue damage. In severe intrusions, the crown must be re-positioned in the arch to avoid periapical pathology and marginal bone loss. There is minimal information about the effect of treatment delay on pulpal and periodontal healing in intrusion trauma. The present paper reports on two cases of severe intrusive luxation applied late at different times treated with surgical extrusion. The first patient, an 11-year-old female, referred to Tokat GOP pediatric dentistry clinic three days after the intrusion too

Alternative approach for carious tissue removal in primary teeth.

BACKGROUND: Minimally-invasive techniques for the management of carious dental tissue has been well documented. This study aimed to report on the clinical and radiographic response after partial caries removal on left mandibular primary second molar. CASE REPORT: A 6-year-old boy sought treatment because of pain in the left mandibular primary second molar. The tooth had a deep caries lesion on the occlusal surface, with loss of enamel structure and dentine proximity with the pulp confirmed by periapical radiograph. According to the clinical and radiographic diagnosis, partial caries removal w

Wound healing following curettement of bifurcation abscesses of human primary molars.

Forty-five abscessed human mandibular primary molars with bifurcation radiolucency were subjected to a two-stage formocresol treatment of the pulp chamber. On the first appointment, an elliptical patch of buccal mucosa was removed. Bony defects varied from 2 mm. to more than 4 mm. in diameter. Periapical radiographs were used to record changes in bone development. Punch biopsy specimens were taken from the bifurcation area of some subjects from 2 weeks up to a year after surgical curettement. Biopsied tissue was studied by electron and light microscopy to examine wound healing of the bifurcati

Histologic examination of teeth with necrotic pulps and periapical lesions treated with 2 scaffolds: an animal investigation.

INTRODUCTION: Traditional pulp regeneration procedures that use a blood clot as a scaffold have produced histologic evidence of bone, cementum, and connective tissue growth within the root. Platelet-rich plasma (PRP) is a bioactive scaffold containing growth factors that enhance wound healing. AIM: The aim of this study was to histologically compare the tissues generated when PRP or a blood clot is placed into teeth with preexisting necrotic pulps and periapical lesions. METHODS: Twenty-four canine teeth from 6 immature ferrets were used. Two ferrets served as positive controls. Sixteen expe

Oral Reactive Lesions Associated to Incomplete Removal of Natal Teeth: Case Report.

Natal teeth are infrequent at birth. Dental extraction is the most recommended treatment to avoid complications. Reactive soft tissue lesions associated to natal tooth or its incomplete removal are rare. A five-month-old female presented natal teeth #71 and #81 which were removed the day after the birth. The baby evolved with two nodules in the region of #71 and #81, pedunculated, 10 and 9 mm, respectively, covered by a pink mucosa, firm in consistency. Periapical radiography showed two radiopaque areas, suggestive of root fragments. After 30 days the nodule located in the region of #81 became

Intraosseous lipoma in the periapical region of a maxillary third molar.

INTRODUCTION: Images of oral lesions can look so exceptionally similar that a diagnosis hypothesis could be difficult. Lesions mimicking apical periodontitis must be considered when making a differential diagnosis. Intraosseous lipoma is a rare benign tumor of low incidence among all bone tumors, and therefore few cases have been reported in the literature. METHODS AND RESULTS: A case of intraosseous lipoma associated with the distobuccal root of tooth #1 in a 39-year-old woman is described. The dental general practitioner did not perceive anything of note in terms of the radiographic aspect,

Corticosteroids in dentistry.

The application of corticosteroids to cut dentine following cavity preparation, to carious dentine in indirect pulp capping, to the exposed pulp or to pulp remnants and periapical tissues during root canal therapy has been claimed to eliminate post operative pain and inflammation. Other investigators have thrown doubt on the validity of these claims and have reported continued inflammation progressing to necrosis following such applications. This paper reports detailed clinical and histological findings in a series of over 500 teeth some of which received conventional treatment while others we…

Implant Apicoectomy as a Minimally Invasive Alternative to Explantation in Retrograde Peri-Implantitis: A Case Report with a 12-Month Clinical and Radiological Follow-Up.

Background/Objectives: Implant explantation is often considered the standard treatment for severe peri-implant inflammatory lesions, including cases with apical involvement. However, this approach is relatively invasive and aggressive, which can lead to significant loss of bone and soft tissues, potentially compromising future prosthetic rehabilitation, particularly in the esthetic zone. This study aimed to present implant apicoectomy as a conservative alternative that would allow preservation of implant stability while effectively treating localized inflammation. Methods: This case report pre

Milk EVs Promote Apical Periodontitis Bone Repair via Osteoblast Targeting.

Chronic apical periodontitis (CAP), an inflammatory disease of periapical tissues, leads to alveolar bone destruction. Current therapies lack osteoblast-targeting specificity and fail to effectively promote bone repair. Extracellular vesicles (EVs), particularly milk-derived EVs (MEVs), show potential for bone regeneration and have anti-inflammatory effects. We investigated the therapeutic potential of engineered MEVs modified with the osteoblast-targeting peptide DSPE-PEG-Mal-Cys-SDSSD (DPS) in CAP. DPS-MEVs enhanced osteogenic capacity and exhibited greater osteoblast targeting compared with…

Sclerosing epithelioid fibrosarcoma of the jaw: a case report and literature review.

INTRODUCTION: Sclerosing epithelioid fibrosarcoma (SEF) is an extremely rare form of bone and soft tissue sarcoma. It occurs mainly in the deep soft tissue of the lower extremities, with few cases reported in the head and neck region. Tumors involving the oral and maxillofacial region (OMFR) and intraosseous examples are rare. CASE PRESENTATION: We present a 52-year-old male with a radiolucent lesion at the apex of the left mandibular second molar tooth with the clinical impression of a periapical granuloma. The patient has a history of SEF of the skull, which was treated by chemotherapy and

Comparison of the Treatment Efficacy of Endo-Perio Lesions Using a Standard Treatment Protocol and Extended by Using a Diode Laser (940 nm).

Marginal and periapical periodontal diseases cause massive destruction of tooth tissues and surrounding tissues, such as alveolar bone and maxillary sinus floor, visible on radiographs. Lesions involving the apical and marginal periodontium are endo-perio (EPL) lesions. This study aimed to compare the treatment efficacy of endo-perio lesions using a standard treatment protocol and a standard diode laser-assisted treatment protocol. The 12 patients were divided into the study (a) and control (b) group. Periodontal indices, tooth vitality and mobility, occlusal status, and radiographic diagnosis

Use of colorimetry as a diagnostic tool for early detection of peri-implant diseases.

INTRODUCTION: The increasing aesthetic demand in dentistry and the limitations of visual colour assessment have encouraged the development of objective methods for evaluating dental and peri-implant tissues. Although colourimeters were originally designed for tooth shade matching, their application has recently expanded to peri-implant soft-tissue analysis, providing quantitative and reproducible measurements capable of detecting early inflammatory changes. OBJECTIVES: This study aimed to determine whether colour analysis using a colourimeter is a valid diagnostic tool for the preliminary det
