Biopsy of anterior mediastinal masses under local anesthesia.

Treatment of most primary mediastinal tumors is based on positive histological diagnosis; we describe a variation of anterior mediastinoscopy under local anesthesia. After infiltration with local anesthetic, a transverse skin incision is made in the second, third, or fourth interspace. The endothoracic fascia is incised vertically adjacent to the periosteum and a mediastinoscope is inserted in the mediastinum. Between 1995 and 2001, we have employed this technique in 46 patients with anterior mediastinal tumors. Histological diagnosis was obtained in all patients. Pneumothorax (2 patients) was…

Semiquantitative correction of posttraumatic enophthalmos with sliced cartilage grafts.

A simple surgical technique for correcting posttraumatic enophthalmos is described. The steps are as follows: (1) a plaster mold is obtained of the patient's face, (2) wax is added to the enophthalmic eye of the plaster mold until it becomes symmetrical, (3) the quantity of wax is measured, and (4) the same amount of sliced costal cartilage is implanted beneath the periosteum of the extended orbital wall behind the vertical axis of the globe. Using this technique, we have successfully treated six patients with traumatic orbital floor defects without complication. This approach is useful for de…

Severe perifolliculitis capitis with osteomyelitis.

This report describes a severe case of perifolliculitis capitis with the rare complication of skull osteomyelitis, a combination not previou … imentation, intravenous antibiotics, and multiple debridements of the scalp, including the involved periosteum and outer cortex of the exposed bone, followed by reconstruction with split-thickness skin grafts. A complete eradication of the disease with closure of all open wounds and clearance of osteomyelitis of the skull was achieved.

[The Sarmiento method as a conservative treatment of tibial fractures (author's transl)].

We have treated 77 tibial fractures according to Sarmiento's principles. This technic was applied two weeks after the initial time of the fracture, and allowed a total weight bearing with complete joint mobility. It needed a functional below-the-knee brace which ends on the knee like a PTB prosthesis. The role of soft tissues (periosteum and interosseous membrane) was important to be considered in the stabilisation of fractures. The results were satisfactory, healing times being comparable to those observed after other orthopaedic methods. Sarmiento's method seems worthwhile because of it's in…

Nonotosclerotic ankylosis of the stapedial footplate.

A woman with a long history of chronic otitis media had a stapedectomy after a surgical diagnosis of otosclerosis. In the histologic study of the temporal bones, done after her death four years later, the main portion of the stapes footplate was present but fractured and no otosclerosis was present. New celluluar bone which simulated otosclerosis filled the anterior oval window niche and was continuous with the middle ear surface of the footplate fragments. The new bone extended anteriorly to overlie a tympanosclerotic focus and inferiorly was continuous with the periosteum of the middle ear. …

Vertical Alveolar Ridge Regeneration by Means of Periosteal Activation-A Proof-of-Principle Study.

AIM: To assess the possibility of vertical alveolar ridge augmentation by means of activation of the periosteum. MATERIALS AND METHODS: Six adult male Beagle dogs were used for the study. All premolars and first molars were extracted, and one vertical saucer-shaped bony defect was created on each side of the mandible. After 3 months of healing, full-thickness muco-periosteal flaps were elevated, and one distraction device was placed on each side of the mandible. The distraction plate was left submerged, and the activation mechanism connected to the distraction rod was exposed intra-orally. Th…

The radical soft-tissue mobilization (Kelly repair) for bladder exstrophy.

UNLABELLED: The radical soft-tissue mobilization (RSTM) technique has been proposed as an alternative for staged reconstruction of bladder e … icles and corpora cavernosa from their attachment on the pubic bones. From the perineal approach, a periosteum incision was performed on the antero-medial aspect of the pubic bones, and the periosteum was peeled from the ischio-pubic ramus, lifting the corpora from the bone and providing control on the pudendal pedicles. The attachments of all striated muscu …

Human cranial stem cells: developmental, pathologic, and therapeutic implications.

Cranial skeletal stem cells are central to skull development, maintenance, and repair. These stem cell populations balance self-renewal with … ntal, molecular, and imaging data. We delineate their distinct niches within the sutures, dura, and periosteum, as well as the signaling pathways that regulate their function. We then highlight future avenues of investigation, including high-resolution profiling of human stem cell populations and development of mechanism-based regenerative strategies that in …

Spontaneous Bone Regeneration ("Phoenix Jaw" Phenomenon) Following Segmental Mandibulectomy for Stage 3 Medication-Related Osteonecrosis of the Jaw: A Case Report.

Segmental mandibulectomy is a valid treatment for advanced medication-related osteonecrosis of the jaw (MRONJ), but subsequent reconstruction is challenging. Spontaneous bone regeneration (SBR) following resection is exceedingly rare, particularly in the elderly. We present the case of a 63-year-old woman with stage 3 MRONJ who underwent a 60 mm segmental mandibulectomy. The defect was reconstructed with a titanium plate, with meticulous preservation of the periosteum. Over a three-year follow-up period, serial imaging demonstrated progressive and substantial bone regeneration along the plate,…

Iceberg lesion-A rare case of chronic osteomyelitis of the jaw with underlying radicular cyst: A case report.

Osteomyelitis is an inflammatory condition of the bones that often starts with an infection of the medullary cavity and swiftly extends to the haversian system and periosteum. If drainage and antimicrobials are ineffective, acute osteomyelitis may progress to chronic osteomyelitis. Here, we offer a unique case that analyzes the radiographic and clinical characteristics of osteomyelitis with underlying disease. Cysts and cyst-like lesions of the jaws must be diagnosed and evaluated using radiographic findings in conjunction with clinical complaints. This case study examines the circumstances th…

A rare case of orbital tuberculosis with cold abscess and frontal bone lesion.

Tuberculosis is a multisystem infectious disease caused by Mycobacterium tuberculosis and a leading infectious cause of morbidity and mortality worldwide. Orbital tuberculosis is a rare form of extra pulmonary tuberculosis, even in endemic areas. It may involve the soft tissue, lacrymal gland, periosteum, or bones of the orbital wall and can extend to adjacent paranasal sinuses or intracranial cavities. The delay in diagnosis can be due to the fact that the clinical signs simulate any inflammatory disease. The diagnosis is usually based on tissue examination in histopathological evidence prese…

Aggressive type of central giant cell granuloma in a woman on hormone replacement therapy: a histopathological insight.

Quite a few lesions of the oral cavity specifically of the gingiva have a greater inclination towards women and mostly occur during the first four decades of life, the cause of which may be credited to the changing levels of sex hormones. Out of all such lesions, one lesion whose aetiology is still unclear and which originats from the periosteum or periodontal ligament is central giant cell granuloma. Repeatedly, it is has been described as a reactive lesion, the cause of which may be secondary to local irritants or trauma, specifically plaque or calculus, which is not considered to be a true …

Zygomatic Arch Parosteal Osteosarcoma in Dogs and a Cat.

Parosteal osteosarcoma is a rare, slow-growing tumor most commonly arising from the surface of long bones. Tissue or histological sections from 5 dogs and 1 cat with zygomatic arch masses were examined. Clinical presentations varied from chronic sneezing to facial swelling. Imaging consistently demonstrated osseous proliferation in the area of the zygomatic arch. Histologically, the masses were characterized by well-differentiated fibro-osseous and chondroid components that radiated outward from the periosteum of the zygomatic bone. Cellular atypia and mitotic figures were uncommon. Parosteal …

[Dermatologic surgery on the scalp].

Soft tissue defects of the scalp can be closed with varying techniques depending on size and depth of the defect. While small and superficial defects can be closed primarily or be left open for secondary intention healing, larger and deeper defects may need flaps or skin grafts. Extensive defects may require combined flaps or vacuum assisted closure techniques. Defects of the periosteum with denuded skull bone must be treated immediately to avoid bony complications. Usually, the tabula externa is fenestrated with holes or abraded totally to create better healing conditions. Granulation tissue …

Carcinoma ex pleomorphic adenoma. Case report.

Carcinoma ex pleomorphic adenoma (CXPA) is a rare and aggressive malignancy that usually develops in primary or recurrent pleomorphic adenoma. It occurs in 6% of all pleomorphic adenomas, of which only 7% of cases occur in minor salivary gland tumors. The rate of occurrence and malignant change increases with time if a case is left untreated. Intraorally, it is mostly seen in the palate, followed by the upper lip and buccal mucosa. CXPA of the hard palate typically presents as a painless, slow-growing swelling that frequently involves the periosteum and underlying palatal bone. Imaging of CXPA…

The multifaceted role of periostin in tumorigenesis.

Periostin, also called osteoblast-specific factor 2 (OSF-2), is a member of the fasciclin family and a disulfide-linked cell adhesion protein that has been shown to be expressed preferentially in the periosteum and periodontal ligaments, where it acts as a critical regulator of bone and tooth formation and maintenance. Furthermore, periostin plays an important role in cardiac development. Recent clinical evidence has also revealed that periostin is involved in the development of various tumors, such as breast, lung, colon, pancreatic, and ovarian cancers. Periostin interacts with multiple cell…
