[Villonodular synovitis of the knee in a 5-year-old child. Apropos of a case].

PURPOSE OF THE STUDY: Case report of a five-year-old boy presenting with a painless swelling of the left knee with decreased range of motion. METHOD: Joint aspiration: fluid rich in red blood cells. X-ray--CT-scan--MRI: pigmented villonodular synovitis suspected. Arthroscopy with synovectomy and biopsy confirmed diagnosis. Because of remaining swelling one month after arthroscopy, intra-articular injection of triamcinolone hexacetonide gave a good result which has been maintained after 2 years. DISCUSSION: Hypothesis on etiology and pathophysiology, differential diagnosis and treatment of Pi

Hypopigmented macules in acantholytic disorders.

BACKGROUND: Widespread hypopigmented macules are rarely seen in heavily pigmented patients with Darier's disease. Previous hypotheses concerning the cause of decreased pigmentation suggest it is a postinflammatory phenomenon or that the hypomelanosis is evidence of subclinical acantholysis. PATIENTS: This report presents 2 patients: a new case of disseminated guttate leukoderma in a black patient with Darier's disease and the first such case in a patient with transient acantholytic dermatosis (Grover's disease). Direct immunofluorescence and electron-microscopic studies were carried out on le

Evaluation of the Efficacy of a 2-MNG-Containing Serum on Improving Skin Tone and Hyperpigmentation.

INTRODUCTION: Hyperpigmentation disorders and skin tone unevenness are a very frequent concern worldwide, especially in China; 2-mercaptonicotinoyl glycine (2-MNG, Melasyl®) is an innovative anti-pigmentation ingredient which binds with melanin precursors, being an alternative to tyrosinase inhibitors. Our goal was to evaluate the effects of a 2-MNG-containing serum on improving facial skin tone evenness and hyperpigmentation. METHODS: Two clinical studies were conducted in China. Clinical study 1, a randomised controlled intra-individual study, included 32 healthy individuals (individual typ…

Lack of association among typical congenital hypertrophy of the retinal pigment epithelium, adenomatous polyposis, and Gardner syndrome.

BACKGROUND: It has been recently documented that multiple bilateral pigmented lesions at the level of the retinal pigment epithelium may be an indicator of patients with familial adenomatous polyposis who are prone to develop intestinal cancer, particularly if there is a positive family history of these intestinal disorders. Although atypical, such lesions have been called congenital hypertrophy of the retinal pigment epithelium (CHRPE). This study was undertaken to determine whether the typical lesions of CHRPE, seen frequently by ophthalmologists, also were indicators of familial adenomatous

[AIDS--mucous membrane manifestations].

In the course of the infection with the human immunodeficiency virus (HIV), we frequently observe disorders of the mucous membranes and, occasionally, they present the first manifestation of HIV-induced immunodeficiency. Like in other organs, opportunistic infections and malignant tumors prevail as a result of the impaired immune system. Opportunistic infections are characterized by frequency (candidiasis), aggressive expansion, persistence, frequent recurrences, and resistance to therapy (gingivitis, parodontitis, herpes simplex, warts). Oral hairy leucoplakia is considered a specific lesion

N-tert-Butylhydroxylamine promotes melanin production in oxidative stress conditions through the MITF signaling pathway.

During the aging process, melanogenesis is downregulated, leading to hair graying. Various agents, including α-melanocyte-stimulating hormon … e robust antioxidant properties of NtBHA underscore its promise as a therapeutic agent for managing pigmentation disorders and mitigating oxidative stress-related skin aging. This study establishes a foundation for further research into the clinical application of NtBHA in anti-aging and dermatological treatments.

The microphthalmia transcription factor (Mitf) controls expression of the ocular albinism type 1 gene: link between melanin synthesis and melanosome biogenesis.

Melanogenesis is the process that regulates skin and eye pigmentation. Albinism, a genetic disease causing pigmentation defects and visual disorders, is caused by mutations in genes controlling either melanin synthesis or melanosome biogenesis. Here we show that a common transcriptional control regulates both of these processes. We performed an analysis of the regulatory region of Oa1, the murine homolog of the gene that is mutated in the X-linked form of ocular albinism, as Oa1's function affects melanosome biogenesis. We demonstrated that Oa1 is a target of Mitf and that this regulatory mech

Key points in dermoscopic differentiation between lentigo maligna and solar lentigo.

A clinical diagnosis of lentigo maligna at an early stage is often difficult even for experienced dermatologists. Differential diagnoses would include solar lentigo, early lesions of seborrheic keratosis, lichen planus-like keratosis, pigmented actinic keratosis and melanocytic nevus. Dermoscopy has been shown to have higher diagnostic accuracy, especially in the diagnosis of pigmented skin lesions, in the past two decades. To aim of the present study was to review the diagnostic key points on dermoscopy in the published work to differentiate lentigo maligna from other differential diagnoses a

Excited state energy fluctuations in the Fenna-Matthews-Olson complex from molecular dynamics simulations with interpolated chromophore potentials.

We analyze the environment-induced fluctuation of pigment excitation energies in the Fenna-Matthews-Olson (FMO) complex from various perspectives, by employing an interpolation-based all-atom potential energy model for describing realistic pigment vibrations. We conduct molecular dynamics simulations on a 100 ns timescale, which is an extent that can enclose the effect of static disorder, and demonstrate its timescale separation from fast dynamic disorder. We extract the spectral densities of the complex by considering both the site and the exciton bases. We show that exciton delocalization re

Pigmented villonodular synovitis in the hand.

The related disorders of pigmented villonodular synovitis and pigmented villonodular tenosynovitis have very different clinical presentations. The synovial form occurs most frequently in the knee although other lower extremity joints including the hip, ankle and subtalar joints can be involved. Rarely, the shoulder, elbow and wrist have been the joint of origin. In contrast, the tenosynovial form is one of the most common masses in the hand and is found very much less frequently elsewhere. This report documents the synovial origin of four cases involving the small joints of the hands.

Acute Posterior Multifocal Placoid Pigment Epitheliopathy: Clinical and Iconographical Aspects of Two Cases.

Acute posterior multifocal placoid pigment epitheliopathy (APMPPE) is a rare inflammatory disorder of the choriocapillaris and retinal pigment epithelium. We report two atypical cases: one with unilateral involvement complicated by serous retinal detachment requiring corticosteroid therapy; the second with bilateral involvement that resolved spontaneously. These observations underscore the clinical heterogeneity of APMPPE and the pivotal role of multimodal imaging, including optical coherence tomography (OCT) and OCT-angiography (OCT-A), in elucidating its pathophysiology, guiding diagnosis, m

Hairy nevus spilus: a case series.

Nevus spilus, also known as speckled lentiginous nevus, is a nevoid disorder characterized by hyperpigmented macules or papules scattered over a background of tan pigmentation. Although nevus spilus is mainly of cosmetic concern, malignant melanoma may rarely develop in the lesions. Although classically not mentioned as a hairy nevus, a few reports in literature mention overlying hypertrichosis in lesions of nevus spilus. We hereby report four cases of nevus spilus without malignant change with overlying terminal hairs that arose mainly from the background pigmented area.

Demonstration of residual perifollicular pigmentation in localized vitiligo--a reverse and novel application of digital epiluminescence dermoscopy.

Digital epiluminescence dermoscopy (microscopy) is usually employed to examine melanomas and other pigmented lesions. We report its reverse application in assisting the early diagnosis of a depigmentation condition-localized vitiligo. A pattern of depigmentation with residual reservoirs of perifollicular pigments is clearly visualized. This pattern is not seen in other disorders of depigmentation. Such pattern signifies focally active or repigmenting vitiligo and thus, clearly serves as a useful guide for the cases wherein there exists a doubt about the possible diagnosis. Further studies are

[Functional and morphological features of vitreoretinal proliferation after penetrating wounds of eye complicated by hemophthalmos (immunohistochemical and electron microscopy study)].

Comprehensive (optic, electron, and fluorescent microscopy) morphological analysis of vitreous fragments removed in vitrectomy after penetrating wounds of the eye complicated by hemophthalmia showed that blood and damaged cells of pigment-free ciliary epithelium and the glia play an important role in the development of proliferative processes in the ophthalmic cavity. Immunohistochemical studies revealed active expression of immunoglobulins A and G and of HLA-DR antigen on immunocompetent cells and abnormal expression of HLA-DR in pigment-free ciliary epithelium and the glia, confirming the au

Polypoidal choroidal vasculopathy.

Polypoidal choroidal vasculopathy was first described as a peculiar hemorrhagic disorder of the macula, characterized by recurrent sub-retinal and sub-retinal pigment epithelium bleeding in middle aged black women. The use of indocyanine green angiography and subsequently of optical coherent tomography has widened our ability to study and understand the pathophysiology of this disorder. The primary abnormality involves the choroidal circulation, and the characteristic lesion is an inner choroidal vascular network of vessels ending in an aneurysmal bulge or outward projection, visible clinicall

Pigmented paravenous chorioretinal atrophy: Updated scenario.

Pigmented paravenous chorioretinal atrophy (PPCRA) is an uncommon form of chorioretinal atrophy characterized by perivenous aggregations of pigment clumps associated with peripapillary and radial zones of retinal pigment epithelial atrophy that are distributed along the retinal veins. Most patients are asymptomatic, and evidence suggest that PPCRA is slowly progressing. Unless macular involvement is present, the majority of patients usually retain a normal visual function. Our ability to diagnose PPCRA has recently improved thanks to multimodal imaging, especially with the advent of ultra-wide

Mitochondrial NCKX5 regulates melanosomal biogenesis and pigment production.

Oculocutaneous albinism (OCA) is a heterogeneous and autosomal recessive hypopigmentation disorder, which is caused by mutations of genes involved in pigment biosynthesis or melanosome biogenesis. We have previously identified NCKX5 (also known as SLC24A5) as a causative gene for OCA type 6 (OCA6). However, the pathogenesis of OCA6 is unknown. We found that NCKX5 is localized to mitochondria, not to melanosomes. Pharmacological inhibition of mitochondrial function or NCKX exchanger activity reduced pigment production. Loss of NCKX5 attenuated Ca2+ enrichment in melanosomes, which compromised P
