[APPLICATION OF RECONSTRUCTIVE SURGICAL TECHNIQUES FOR THE PERIPHERAL NERVE TO INJURED PHRENIC NERVE TO RESTORE THE PARALYZED DIAPHRAGM].

Phrenic nerve injury often causes diaphragmatic dysfunction. Damage to the phrenic nerve may be caused by iatrogenic injury such as transection or crush during thoracic or neck surgery to treat bronchogenic, mediastinal, or neck tumors. Plication of the diaphragm is a procedure in which the flaccid hemidiaphragm is tautened by oversuturing it. Although it has been offered to patients with unilateral diaphragmatic paralysis who have severe dyspnea and other symptoms, the essential treatment should be restoration of the function to the paralyzed diaphragm. Established reconstructive techniques f

The Rib Cartilage Concept in Microtia.

There is a long history of rib cartilage use in reconstruction for microtia. This article aims to describe the modern techniques of autologous auricular reconstruction in detail, to elucidate the modifications for certain anatomical or disease-related conditions, and to contrast the different strategies used by other surgeons. The two mainstays of successful reconstruction with excellent results are the fabrication of the framework and soft tissue management. A three-dimensional framework mimics the curves, indentations, and peaks of a normal ear with maximal stability. Recently, the creation

[The super extended sub-mental flap or combo sub-mental flap].

The author presents a technical variation of the sub-mental flap including in a conventional pedicled flap both sub-mental axes and their anastomoses on the midline. The assessment of the first flaps raised according to this method confirms the improvement of the distal blood supply. It allows the possibility to harvest "super extended" flaps reaching the contralateral auricular lobula. This variation can be considered as an axial flap which only the tip, located beyond the mandibular angle, is at random. The evolution of the sub-mental flap from its original description to this variation call

Soft-tissue refinements in the treatment of unicoronal craniosynostosis: the rotational forehead flap.

Unicoronal craniosynostosis is characterized by ipsilateral superior and posterior displacement of the supraorbital rim and frontal bone, ipsilateral widening of the palpebral fissure, ipsilateral superior displacement of the brow, and contralateral brow depression. In the literature, surgical treatment has focused on bony anatomy, with little written about soft-tissue correction. Over the last 25 years, the senior author (L.A.W.) has incorporated soft-tissue refinements, including a rotational flap of the forehead, elevating the contralateral brow, depressing the ipsilateral brow, and equilib

Simultaneous labia minora and majora reduction: a case report.

Labia minora enlargement may constitute a cosmetic or functional problem for a woman, potentially leading to surgery. However, more rarely, the labia majora may also be problematic for women. This case describes the rare situation of a woman presenting with enlargement of both the labia minora and majora and her subsequent simultaneous surgical treatment.

Nasal papilloma, a rare late complication of tongue flap repair of palatal fistula.

The use of a tongue flap for treatment of palatal fistulae is a well-established procedure, but it is not without complication. We report on a patient who underwent palatal fistula closure with tongue flap as a child, who then developed a nasal papilloma from the mucosa of the tongue flap 40 years later. This case is the first of its kind to be reported and emphasizes the need for long-term follow-up because the tongue mucosa is vulnerable to many diseases.

Meta-analysis of therapeutic procedures for acquired subglottic stenosis in adults.

BACKGROUND: Treatment for adult subglottic stenosis is technically demanding and no therapeutic algorithm exists. We performed the present meta-analysis of treatment for this condition in an attempt to compare efficacy on the basis of type of procedure. METHODS: We identified 24 eligible retrospective studies reporting the therapeutic results for inclusion criteria. Meta-analysis was performed by combining the results of the reported success rates; success is defined as a condition requiring no further treatment. The relative risk was used as a summary statistic. RESULTS: Pooled success rate

Ex-vivo ureteral spatulation during laparoscopic pyeloplasty: a novel approach to a difficult problem.

Ureteral spatulation and performance of the anastamosis to the renal pelvis are the most important and often technically demanding and time consuming steps in performing a laparoscopic pyeloplasty. We describe a novel ex-vivo technique of ureteral spatulation and placement of the apical anastamotic sutures. This technique is especially helpful in challenging cases where optimal angles may not be viable via pure laparoscopic technique. The technique was performed on six patients, five pediatric and one adult without any immediate complications or long-term stricture or anastamotic disruption.

Reconstruction of Wassel type IV thumb duplication.

The most common thumb duplication is a Wassel type IV with duplication of the proximal and distal phalanges. The term "split-thumb" seems more appropriate as neither element is as robust as a normal thumb. The split parts may share bony elements, tendons, ligaments, joints, neurovascular structures, and nails. The surgical principles are preservation and augmentation of the dominant thumb and ablation of the lesser counterpart. The soft tissue(s) from the ablated thumb are used to augment the retained thumb. This article will discuss the indications, timing, and technique of surgical reconstru

[Deformity-associated treatment of the hallux valgus complex].

Hallux valgus represents a combined deformity with malpositioning of the big toe in the metatarsophalangeal joint and metatarsal splaying due to metatarsus primus varus formation. It is defined on the basis of joint condition of the metatarsophalangeal and tarsometatarsal (TMT) joints, the extent and congruence or incongruence of malposition, mobility of the metatarsophalangeal joint and TMT stability. Basic resection appears to be indicated only in exceptional cases. Depending on the degree of severity, deformities can be corrected by means of distal, diaphyseal or proximal osteotomies and TM

An unusual staged labial rejuvenation.

INTRODUCTION: There have been concerns about the safety and effectiveness of publicized procedures in elective female genital rejuvenation. AIM: To present an interesting case in which a staged approach to rejuvenation of both the Labia majora and minora was performed safely and effectively. METHODS: A patient with an unusual redundancy of both the Labia minora and majora presented interested in reduction. The procedure was performed in two parts. RESULTS: Despite her complication following the first stage, 6 weeks after her second stage, the patient is satisfied. CONCLUSIONS: Cosmetic red

A simple surgical technique for correction of macrotia with poorly defined helical fold.

Macrotia is a relatively rare auricular deformity and few techniques for treatment have been described, most of which include skin and cartilage excisions. Effacement of the helical fold may accompany this deformity and should be reconstructed for aesthetic integrity. The authors present their ear reduction technique that achieves ear reduction and helical fold reconstruction through a posterior approach. The method is a simple procedure performed by incising and overlapping the cartilage and should be indicated for selected mild macrotia cases.
