BACKGROUND/PURPOSE: Pneumonectomy in children can be complicated by a severe mediastinal shift, which leads to bronchial stretching resulting in severe respiratory failure. This postpneumonectomy syndrome can be corrected by inserting a prosthesis in the empty side of the chest.
METHODS: Forty-two children, from 6 months to 15 years old, underwent a pneumonectomy. Seven of these patients were treated surgically for severe manifestations of postpneumonectomy syndrome. First insertion of an expandable prosthesis was followed up in 5 cases by its replacement with a breast prosthesis in adolescen…

BACKGROUND: Malignant pleural mesothelioma is uncommon, and presently, no standard treatment of this disease exists. The objective of our analysis was to study the patterns of failure for malignant pleural mesothelioma after trimodality treatment consisting of extrapleural pneumonectomy, chemotherapy, and radiation therapy.
METHODS: Between 1987 and 1993, 49 patients with malignant pleural mesothelioma underwent extrapleural pneumonectomy. There were two perioperative deaths, and 1 patient died 5 weeks after extrapleural pneumonectomy. Thirty-five of the surviving patients received adjuvant c…

OBJECTIVE: Acute respiratory distress syndrome (ARDS) is a major cause of death following lung resection. At this institution we reported an … re, consistent with findings in previous studies, the highest incidence and death from ARDS were in pneumonectomy patients (incidence 11.4%; mortality 50%). Although the incidence and mortality from ARDS following pneumonectomy were not significantly different between the two study periods (p=0.08, p=0.35), we found that fewer pneumonectomies were performed in …

Postpneumonectomy syndrome is characterized by postoperative bronchial obstruction caused by mediastinal shift. The syndrome is well documented in the medical literature as a late complication of right pneumonectomy; however, it rarely occurs following resection of the left lung, and only 10 cases have been published. The pathophysiology, clinical manifestations, prognosis, and treatment are similar for both sides of the lung. We present the case of an adult patient who underwent left pneumonectomy and developed postpneumonectomy syndrome 15 months later. Stenosis of the intermediate bronchus …

We observed improvements in two cases of chronic atelectasis through use of nasal continuous positive airway pressure (nCPAP). Case 1 suffered from middle lobe syndrome accompanied by chronic atelectasis resistant to medical treatment. Case 2 suffered from respiratory failure caused by chronic atelectasis and airway infection complications thereof following a total pneumonectomy and post-pneumonectomy syndrome. The patient was placed on artificial ventilation, and atelectasis was improved by maintaining PEEP and airflow to the atelectatic region. Following extubation we obtained good pneumatiz…

We report a 31-year-old man in whom initially localized lymphomatoid granulomatosis was diagnosed at left pneumonectomy. He had severe anemia which is rare and clubbing of the fingers and toes, a feature not previously described. Five months following surgery, disease spread to the right lung, and atypical lymphomatous transformation occurred in cervical and mediastinal lymph nodes, leading to a superior vena cava syndrome. Chemotherapy with cyclophosphamide and prednisone resulted in significant resolution initially, but ultimately, the patient had progressive axillary node enlargement and su…

We report the case of a 50-yr-old man who experienced exertional dyspnea 5 mo after a left pneumonectomy for carcinoma. As the clinical features pointed toward a pulmonary embolism, we performed a ventilation plus perfusion radionuclide lung scan. It showed no evidence of pulmonary embolism, but it did show a systemic uptake of the isotope, suggesting a right-to-left shunt that was confirmed by contrast echocardiography, which revealed an atrial septal defect. Right-to-left shunts after pneumonectomy have already been reported and can be diagnosed by lung scintigraphy. Usually, a patent forame…

We report a case of ileal metastasis of renal cell carcinoma (RCC) in a 58-year-old male. The patient had a history of radical nephrectomy for a right RCC, and 2 years later underwent bilateral partial pneumonectomy for metastatic disease of the lung. A period of 1 year after the partial pneumonectomy, he developed bloody stools. Colonoscopy revealed an ileocolic intussusception caused by a polypoid tumor in the ileum, and the tumor was observed to be protruding into the ascending colon. The histological features of the tumor biopsy specimen confirmed the diagnosis of metastatic RCC. Metastasi…

Multiple arteriovenous fistulae of the entire left pulmonary lobe were found to be the cause of increasing cyanosis on exertion and hour-glass nails in a 19-year-old patient. The right-to-left shunt amounted to 16%. The 2 possible forms of treatment were embolization of the arteriovenous fistulae or pneumonectomy. The first procedure was unsuitable because of the diffuse spread of the fistulae. In order to avoid all the early and late complications associated with a pneumonectomy, an unusual method was adopted: The left pulmonary artery was transected and the lung was left in place as a "physi…

BACKGROUND: Pneumonectomy and sleeve resection are routine operations for the treatment of central non-small cell lung cancer (NSCLC), but some patients suffered of central NSCLC, whose pulmonary function is too poor to tolerate pneumonectomy, or the tumor involves the bronchus and pulmonary artery extensively,it is hard to perform bronchovascular sleeve lobectomy. The aim of this study is to assess the feasibility of lung autotransplantation in the treatment of central NSCLC.
METHODS: The clinical data of 3 cases with central NSCLC treated by lung autotransplantation was reviewed from Decemb…

BACKGROUND: Postpneumonectomy syndrome (PPS) is a rare complication after pneumonectomy. It consists of an excessive mediastinal shift resulting in compression and stretching of the tracheobronchial tree and the esophagus. The aim of this study was to give a comprehensive overview of diagnosis, variety of symptoms and evaluation of surgical treatment of PPS.
METHODS: We retrospectively reviewed the charts of all our patients with PPS since 1994 with respect to symptomatology, treatment and outcome. Our results were compared with case reports and case series in the literature.
RESULTS: Six wo…

BACKGROUND: The development of a bronchopleural fistula (BPF) is a devastating complication after lung resection. Diabetic patients exhibit a high propensity for postpneumonectomy complications, particularly BPF. This study evaluated the use of an intercostal muscle flap to reinforce the bronchus in high-risk diabetic patients after pneumonectomy.
METHODS: From February 2002 to December 2005, 70 patients with established diabetes mellitus undergoing pneumonectomy were prospectively enrolled in this study. Patients were randomized to have their bronchial stump reinforced with an intercostal mu…

BACKGROUND: Late postpneumonectomy bronchopleural fistula (LBPF) is a serious complication. Surgical repair of the bronchial stump through a lateral thoracotomy is a dangerous attempt due to mediastinal fibrothorax and the risk of pulmonary artery stump damage.
OBJECTIVES: The goal of this study was to estimate the effectiveness of the transsternal, transpericardial approach for bronchial stump repair in case of LBPF.
METHODS: From 1996 to 2002, 1,294 lung resections for non-small cell lung cancer were performed at our department. Out of 412 pneumonectomies, 16 patients (3.8%) presented with…

BACKGROUND: Bronchopleural fistula is a serious complication after pneumonectomy. The aim of this retrospective study was to investigate the efficacy of bronchial stump reinforcement with a pedicled flap of whole pericardium.
METHODS: The bronchial stump of 93 consecutive patients who underwent pneumonectomy between July 1988 and March 2003 was covered with a pedicled pericardial flap. Pneumonectomy was performed for primary lung cancer in 89.2% of patients. The study patients received concomitant extensive mediastinal lymphadenectomy, resection of adjacent structures (aorta, vena cava, thora…

Locally advanced lung cancer invading central bronchi, carina, and trachea can be resected with bronchoplastic techniques including sleeve lobectomy and sleeve pneumonectomy. These procedures can be performed on selected patients with advanced lung cancer invading the root of lobar bronchus or carina. After the bronchial/tracheal resection, anastomosis of the severed bronchus/trachea is done to reconstruct the airway. The anastomosis should be done with great care, avoiding postoperative dehiscence, stenosis, or granulation formation. Some tips and pitfalls of the procedures, including determi…

Development of pleural empyema with or without bronchopleural fistula following pneumonectomy presents a major complication with a mortality of 10 to 20%. The surgical goals of bronchopleural fistula treatment are infection control, pus drainage, and lung re-expansion. The main goal is closure of the fistula. This can be achieved with various surgical methods, such as the Clagett or accelerated Weder procedure, omentoplasty, or a transpericardial approach, according to Abruzzini. We present the case of a 58-year-old patient with pleural empyema within the postpneumonectomy cavity in the presen…
