Basilar artery stenosis mimicking the lacunar syndrome of pure motor hemiparesis.

BACKGROUND: Stereotyped, repeated transient ischemic attacks manifesting as pure motor hemiparesis are most often attributed to ischemia of the internal capsule or ventral pons resulting from in situ disease of the small penetrating arteries. CASE DESCRIPTION: We report a 61-year-old man presenting with recurrent episodes of left-sided weakness consistent with the lacunar syndrome of pure motor hemiparesis. Subsequent neuroimaging revealed infarction of the right ventral pons and a critical basilar artery stenosis as the inciting lesion. Despite maximal antithrombotic therapy, he continued to…

[Diffusion-weighted magnetic resonance imaging (MRI) in acute brain stem infarction].

Diffusion-weighted magnetic resonance imaging (DWI) provides one of the earliest demonstrations of ischemic lesions. However, some lesions may be missed in the acute stage due to technical limitation of DWI. We therefore conducted the study to clarify the sensitivity of DWI to acute brain stem infarctions. Twenty-eight patients with the final diagnosis of brain stem infarction(midbrain 2, pons 9, medulla oblongata 17) who had been examined by DWI within 24 hours of onset were retrospectively analyzed for how sensitively the initial DWI demonstrated the final ischemic lesion. Only obvious(disti…

Changes of acetylcholinesterase activity in various parts of brain following nontreated and treated soman poisoning in rats.

Changes of acetylcholinesterase (AChE) activity in various parts of the brain (frontal cortex, medulla oblongata, pons Varoli, cerebellum, hypothalamus, and hippocampus), following im sublethal non-treated and treated soman poisoning were studied. As a treatment, two antidotal mixtures containing atropine and either obidoxime or oxime HI-6 were used. This antidotal treatment was administered im for 30 s following soman intoxication. The AChE activities in the various brain tissues were evaluated at 1 and 3 h following soman administration. As expected, the highly toxic organophosphorus compoun…

MR findings in pontocerebellar hypoplasia.

We present four cases with combined hypoplasia of the cerebellum and the ventral pons-pontocerebellar hypoplasia (PCH). PCH represents an autosomal recessive neurodegenerative disorder with fetal onset. The disease is rare, with less than 20 cases having been reported. The main findings of PCH and the inclusion criteria for our cases can be summarised as progressive microcephaly from birth, pontocerebellar hypoplasia documented by MRI and marked chorea, which may change, later in childhood, to more dystonic patterns. The cerebral cortex becomes progressively atrophic. Motor and mental developm…

Effects of acute and chronic ethanol administration on regional thiamin pyrophosphokinase activity of the rat brain.

Thiamin pyrophosphokinase (TPKase) activity was determined in supernatants of cerebral cortex, cerebellum, pons, medulla, hypothalamus and corpus callosum homogenates obtained from normal rats and from rats given ethanol acutely (a single dose of 4.7 g X kg-1 body wt) or chronically (4.7 g X kg-1 body wt daily for 35 days) by gastric gavage. Regional cell densities (derived from DNA content) and protein contents were also determined. TPKase was detected in all brain regions investigated, the highest activity being found in the cerebellum or in the pons depending on whether it was expressed per…

Thiamine content and turnover rates of some rat nervous regions, using labeled thiamine as a tracer.

The content of total thiamine radioactivity in some nervous structures and liver of the rat was determined in a steady state condition, using [thiazole-2-14C]thiamine as a tracer. The contents were analyzed by a mamillary type compartmental model which enabled us to calculate the influx and efflux fractional rate constants, turnover times, turnover rates and relative accuracy. Total thiamine turnover rates of the central nervous system regions were found to be ordered in the following sequence: cerebellum (0.55 microgram/g.h) greater than medullar and pons greater than spinal cord and hypothal…

Normal Correction of Sodium Leading to Central Pontine Demyelinosis: A Rare Occurrence.

Central pontine myelinolysis is a rare condition arising from the myelinolysis of white matter tracts in the pons, most commonly in response to iatrogenic hypertonic stress caused by the rapid correction of hyponatremia. Here, we present an interesting case of central pontine myelinolysis subsequent to normal saline infusion despite strict adherence to the guideline protocol.

A taxonomy for brainstem cavernous malformations: subtypes of pontine lesions. Part 1: basilar, peritrigeminal, and middle peduncular.

OBJECTIVE: Brainstem cavernous malformations (BSCMs) are complex, difficult to access, and highly variable in size, shape, and position. The authors have proposed a novel taxonomy for pontine cavernous malformations (CMs) based upon clinical presentation (syndromes) and anatomical location (findings on MRI). METHODS: The proposed taxonomy was applied to a 30-year (1990-2019), 2-surgeon experience. Of 601 patients who underwent microsurgical resection of BSCMs, 551 with appropriate data were classified on the basis of BSCM location: midbrain (151 [27%]), pons (323 [59%]), and medulla (77 [14%]…

Midbrain atrophy related to parkinsonism in a non-coding repeat expansion disorder: five cases of spinocerebellar ataxia type 31 with nigrostriatal dopaminergic dysfunction.

BACKGROUND: Spinocerebellar ataxia type 31 (SCA31) is caused by non-coding pentanucleotide repeat expansions in the BEAN1 gene. Clinically, SCA31 is characterized by late adult-onset, pure cerebellar ataxia. To explore the association between parkinsonism and SCA31, five patients with SCA31 with concomitant nigrostriatal dopaminergic dysfunction (NSDD) development, including three cases of L-DOPA responsive parkinsonism, were analyzed. METHODS: To assess regional brain atrophy, cross-sectional and longitudinal imaging analyses were retrospectively performed using magnetic resonance imaging (M…
