Delayed cerebral infarction in pneumococcal meningitis due to postinfectious vasculitis: A case report.

RATIONALE: Delayed cerebral infarction (DCI) is a rare but devastating complication of bacterial meningitis that is characterized by an initial good recovery followed by sudden deterioration several days after presentation. The potential mechanisms underlying DCI encompass postinfectious vasculitis, vasospasm, and cerebral thrombosis. Sequential magnetic resonance angiography and characteristic high-resolution magnetic resonance patterns are helpful in discerning the cause of DCI. PATIENT CONCERNS: A right-handed 45-year-old man with nasal leakage of cerebrospinal fluid from a car accident wa

First report of a patient meeting criteria for both multisystem inflammatory syndrome in children and adult onset Still's disease.

BACKGROUND: COVID-19 is associated with a postinfectious hyperinflammatory disorder, multisystem inflammatory syndrome in children (MIS-C), that shares characteristics with still's disease, known as systemic juvenile idiopathic arthritis (SJIA) in children younger than 16, and adult onset Still's disease (AOSD) in children 16 and older. Both MIS-C and SJIA/AOSD can be complicated by macrophage activation syndrome (MAS), a potentially fatal condition of cytokine storm. CASE PRESENTATION: We present a 16 year-old male who developed quotidian fever, headache, conjunctival injection, sore throat,

Five-year mortality of severely malnourished patients with chronic anorexia nervosa admitted to a medical unit.

OBJECTIVE: Anorexia nervosa (AN) is associated with one of the highest mortality rates of any psychiatric disorder but limited mortality data were reported for those with extremely severe malnutrition. This study aimed to estimate standardized mortality ratio (SMR), investigate predictive factors of mortality and causes of death among a sample of patients with AN admitted to a specialized clinical nutrition unit (CNU) because of extremely severe malnutrition. METHODS: Between 11/27/1997 and 01/15/2014, vital status was determined for 384 patients admitted for AN at the first time in the CNU.

Demographics, aetiology and outcome of paediatric acute liver failure in Singapore.

INTRODUCTION: The aetiology of paediatric acute liver failure (PALF) varies widely according to age, and geographic and socioeconomic factors. This study aimed to examine the epidemiology, aetiology and outcome of PALF in Singapore at a single centre. METHODS: A retrospective review was performed of patients aged 0-18 years who were diagnosed with PALF from 2007 to 2019. PALF was defined by: absence of chronic liver disease; biochemical evidence of acute liver injury; and coagulopathy, non-correctible by vitamin K, defined as prothrombin time (PT) ≥20 seconds or international normalised ratio

Rabbits immunized with Epstein-Barr virus gH/gL or gB recombinant proteins elicit higher serum virus neutralizing activity than gp350.

Epstein-Barr virus (EBV) is the primary cause of infectious mononucleosis and has been strongly implicated in the etiology of multiple epithelial and lymphoid cancers, such as nasopharyngeal carcinoma, gastric carcinoma, Hodgkin lymphoma, Burkitt lymphoma, non-Hodgkin lymphoma and post-transplant lymphoproliferative disorder. There is currently no licensed prophylactic vaccine for EBV. Most efforts to develop prophylactic vaccines have focused on EBV gp350, which binds to CD21/CD35 to gain entry into B cells, and is a major target of serum neutralizing antibody in EBV seropositive humans. Inhibition of mTORC1 inhibits lytic replication of Epstein-Barr virus in a cell-type specific manner.

BACKGROUND: Epstein-Barr virus is a human herpesvirus that infects a majority of the human population. Primary infection of Epstein-Barr virus (EBV) causes the syndrome infectious mononucleosis. This virus is also associated with several cancers, including Burkitt's lymphoma, post-transplant lymphoproliferative disorder and nasopharyngeal carcinoma. As all herpesvirus family members, EBV initially replicates lytically to produce abundant virus particles, then enters a latent state to remain within the host indefinitely. METHODS: Through a genetic screen in Drosophila, we determined that reduc

Association between elevated whole blood Epstein-Barr virus (EBV)-encoded RNA EBV polymerase chain reaction and reduced incidence of acute lung allograft rejection.

BACKGROUND: Accurate functional assessment of patient immunosuppression after solid-organ transplantation remains elusive. Despite therapeutic serum immunosuppressive drug levels many lung transplant recipients still develop allograft rejection. We investigated the hypothesis that detection of latent Epstein-Barr virus (EBV) in peripheral blood may be a functional marker for the net effects of administered immunosuppression. METHODS: A retrospective analysis was performed on data obtained from a prospective trial investigating the ability of a novel EBV polymerase chain reaction (PCR) panel f

Rabbit antithymocyte globulin as induction immunotherapy for pediatric deceased donor kidney transplantation.

PURPOSE: There is scant literature describing the long-term outcome of the use of antithymocyte globulin induction immunotherapy in pediatric deceased donor kidney transplants. We retrospectively studied the long-term results and safety of antithymocyte globulin as induction immunotherapy in all children undergoing transplantation at our institution since 1991. MATERIALS AND METHODS: A total of 120 kidney transplants were performed in 95 patients 18 years or younger between January 1986 and December 1998. Patients were divided into 2 groups. The control group (63 patients) received cyclospori

Infectious Causation of Abnormal Host Behavior: Toxoplasma gondii and Its Potential Association With Dopey Fox Syndrome.

The apicomplexan parasite Toxoplasma gondii, the causative agent of toxoplasmosis, can infect all warm-blooded animals. T. gondii can subtly alter host behaviors-either through manipulation to enhance transmission to the feline definitive host or as a side-effect, or "constraint," of infection. In humans, T. gondii infection, either alone or in association with other co-infecting neurotropic agents, has been reliably associated with both subtle behavioral changes and, in some cases, severe neuropsychiatric disorders, including schizophrenia. Research on the potential impact of T. gondii on the

The Impact of Government Lockdowns on the Mental Health of the General Population: A Systematic Review and Meta-analysis.

Since December 2019, the COVID-19 pandemic has spread globally, prompting governments in many countries to implement lockdowns to control the transmission of the virus. Outbreaks of emerging infectious diseases, such as COVID-19, and the associated government lockdowns may have significant negative impacts on mental health. A comprehensive review of the available evidence on this topic can provide useful information for policymakers. This review aimed to assess the effects of government lockdowns on the mental health of the general population during emerging infectious disease outbreaks. On Ap

Strongyle egg shedding and egg reappearance periods in horses with pituitary pars intermedia dysfunction.

Pituitary pars intermedia dysfunction (PPID) is the most common endocrine disorder of older horses. Immune dysfunction in horses with PPID could increase susceptibility to infectious diseases, including strongyle infections; however, few data are available. The aim of this study was to determine if horses with PPID had increased strongyle faecal egg counts (FEC) compared with control horses, over a fourteen-week period in Victoria, Australia. Clinical signs and plasma adrenocorticotropic hormone (ACTH) concentrations were used to categorise horses into PPID (n=14) or control (n=31) groups. Fae

A novel limited-dose C5 inhibitor add-on as rescue therapy for refractory NMOSD attacks: a practical alternative to conventional multi-dose regimens.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) patients with refractory attacks, despite standard therapy with high-dose intravenous methylprednisolone (IVMP) and plasma exchange (PLEX), often experience significant disability. While complement C5 inhibition has established efficacy in relapse prevention, its role in acute-phase management remains unclear. METHODS: In this single-center, retrospective case series, eight patients (6 AQP4-IgG seropositive, 2 seronegative) with acute NMOSD and suboptimal responses to IVMP ± PLEX were treated with 1-4 weekly doses of ECU (900 mg per d

Decoding the bilateral vestibulopathy spectrum: etiology-based phenotypes, clinical profiles and pathways to implant candidacy.

INTRODUCTION: Bilateral vestibulopathy (BVP) is a chronic, disabling disorder characterized by bilateral loss of vestibular function, leading to imbalance, oscillopsia, and falls. Despite established diagnostic criteria, clinical expression and rehabilitation potential vary widely across etiologies. Understanding these functional signatures is crucial for tailoring future neurostimulation strategies. The main objective of this study is to decode the etiological and functional heterogeneity of BVP and define clinical-functional profiles guiding vestibular or cochleo-vestibular implant candidacy
