Case report: Dorsal root ganglion (DRG) stimulation for acute neuropathic pain from acute herpes zoster infection.

There is little evidence regarding the effectiveness of procedural interventions for treatment of pain after an acute herpes zoster (AHZ) infection before the development of postherpetic neuralgia (PHN). In our case, a 61-year-old male presented with 1 month of left foot pain following an AHZ infection. After 1 month of pain refractory to treatment and admission to our hospital for acute pain management, a nerve stimulator was placed at the left first sacral (S1) dorsal root ganglion (DRG), which significantly decreased his pain despite his ongoing dermatologic manifestations of AHZ. In conclu

Pain Trajectories and Predictors Following Uterine Artery Embolization Using a Nitroglycerin-Lidocaine Protocol: A Prospective Study.

BACKGROUND AND AIMS: Uterine artery embolization (UAE) is an effective minimally invasive treatment for symptomatic fibroids; however, post-procedural pain remains a major limitation. Intra-arterial lidocaine administration has shown inconsistent results with concerns regarding vasospasm and incomplete embolization. We addressed this by introducing a sequenced protocol utilizing prophylactic intra-arterial nitroglycerin. This study aimed to characterize post-procedural pain trajectories, identify predictors of pain severity, and evaluate the safety of the regimen. METHODS: In this prospective…

Pain Assessment During Less Invasive Surfactant Administration Using Skin Conductance: A Prospective Cohort Study.

BACKGROUND: Less Invasive Surfactant Administration (LISA) is an established treatment for surfactant deficiency. However, analgosedation fo … actant Administration (LISA). Our findings suggest that SC alone may not be a reliable indicator of procedural pain in preterm infants shortly after birth. There remains a critical need for objective methods to assess pain in preterm infants. Further research is required to validate SC for neonatal pain assessment.

Virtual hypnosis with HypnoVR system during prostate biopsy: a new tool to increase patients' comfort.

Prostate biopsy is often perceived as an invasive and painful procedure, frequently associated with significant emotional distress. Despite the use of local anesthesia, a considerable proportion of patients still report pain and anxiety. Hypnosis has been demonstrated to reduce procedural discomfort; however, its clinical use is limited by the requirement of trained professionals and time constraints. This preliminary study evaluates the feasibility and potential benefits of digital sedation using immersive virtual reality (VR) with the HypnoVR system in five patients undergoing prostate biops

Dream of the Endless: Updates in Agents for Procedural Sedation.

Procedural sedation and analgesia is an essential activity in the emergency department for managing pain and anxiety during a variety of medical procedures. Various pharmacotherapy options, including opioid analgesics, antiemetics, anticholinergics, sedatives, and ketamine have been utilized, all with their unique efficacy and safety profiles. This review highlights the challenges associated with using certain agents and discusses emerging trends such as the use of newer synthetic opioids and the expanding use of dexmedetomidine. Overall, the selection of the optimal agents for procedural seda

Assessment of Pain in the Newborn: An Update.

Preterm and term neonate pain assessment in neonatal intensive care units is vitally important because of the prevalence of procedural and postoperative pain. Of the 40 plus tools available, a few should be chosen for different populations and contexts (2 have been validated in premature infants). Preterm neonates do not display pain behaviors and physiologic indicators as reliably and specifically as full-term infants, and are vulnerable to long-term sequelae of painful experiences. Brain-oriented approaches may become available in the future; meanwhile, neonatal pain assessment tools must be

Ketamine/midazolam versus etomidate/fentanyl: procedural sedation for pediatric orthopedic reductions.

BACKGROUND: Orthopedic reductions are commonly performed procedures requiring sedation in the pediatric emergency department (PED). Ketamine is a widely used agent for pediatric procedural sedation, but its use may present difficulties in select populations, such as those with psychiatric diagnoses. In such a case, alternative agents that are safe and effective are needed. Etomidate is a commonly used induction agent for rapid-sequence intubation in the PED. Several retrospective and few prospective studies support etomidate's safety and efficacy in pediatric procedural sedation. OBJECTIVE: T
