The pediatrician's role in child abuse interviewing.

The history of sexual abuse that a child gives to the pediatrician may be the single most important factor in determining if a child has been abused. Most children have completely normal anogenital examinations; very rarely is definitive forensic evidence obtained or a sexually transmitted disease discovered. Physicians, in general, are given unique authority and privilege in reporting such hearsay evidence. The pediatrician often is the first professional with whom a child has contact when an allegation of abuse is made. Child protective issues then become paramount. It is important, therefor

The largely untold story of welfare reform and the human services.

Welfare reform has placed the lives of clients, the jobs of social workers, and the mission of agencies in jeopardy. Based on interviews with senior staff at 107 nonprofit human services agencies, this article documents the largely untold story of how nonprofit agencies' workers responded to the impact of welfare reform on their clients, their jobs, and the delivery of services. Workers reported less time for social services because of welfare-related regulations, penalties, work mandates, crises, and paper work. They also reported more service dilemmas including less control of the job, more

Promoting participation in healthcare situations for children with JIA: a grounded theory study.

Children's right to participate in their own healthcare has increasingly become highlighted in national and international research as well as in government regulations. Nevertheless, children's participation in healthcare is unsatisfactorily applied in praxis. There is a growing body of research regarding children's participation, but research from the children's own perspective is scarce. The aim of this study was thus to explore the experiences and preferences for participation in healthcare situations among children with juvenile idiopathic arthritis (JIA) as a foundation for creating strat

Therapeutic alliance in spinal cord injury rehabilitation: power, institutions, and disabling control.

BACKGROUND: The therapeutic alliance (TA) is fundamental to improving the quality of health and rehabilitation services. PURPOSE: This study critically explored patient perspectives on the influence of TAs with practitioners while participating in inpatient rehabilitation after spinal cord injury (SCI). METHODS: A qualitative design involving (n = 18) in-depth interviews with inpatients was employed. The verbatim transcripts were subjected to an iterative, reflective thematic analysis (RTA), guided by a social constructionist epistemology and deductively informed by theories of power and psy

Decision-Making Support for an Adult Patient With Severe Congenital Heart Disease: A Case Study.

In critical care, owing to the severity of the illness, it is often difficult to confirm the patient's treatment preferences because of issues with their level of consciousness. Supporting patient decision-making should prioritize confirming patients' wishes. This case study examines an instance in which the focus shifts from family- to patient-centred decision-making. During a multidisciplinary conference in which the medical and nursing team considered delegating treatment decisions to the family, an intervention advocating the necessity of patient-centred decision-making prompted a reconsid

Satisfaction with perinatal care providers and the childbirth experience: the moderating role of body mass index.

BACKGROUND: Satisfaction with birth and healthcare provider experiences have long-term effects for maternal health. Research has shown that mothers who report more trust, respect, and self-efficacy in their relationship with their healthcare providers are more likely to report positive birthing experiences. Further, individuals with obesity, including pregnant mothers, are more likely to experience weight-related stigma from healthcare providers which may negatively impact satisfaction with this relationship. Thus, the current study examines maternal pre-pregnancy body mass index (BMI) as a mo

"I think most people feel like healthcare professionals tell them to take their treatments and judge them for not taking them": reflexive thematic analysis of the views of adults with cystic fibrosis on how treatment adherence is discussed in healthcare.

OBJECTIVE: Previous research exploring patient-practitioner communication in relation to adherence in cystic fibrosis (CF) is limited. This UK study explored the views of adults with CF on how treatment adherence (related to all CF treatments) is discussed in routine CF care. METHODS: 12 White British adults (ten females; aged 20-37 years; mean 30.1 years) with CF participated in semi-structured interviews. RESULTS: Three overarching themes were developed through reflexive thematic analysis: (1) 'The power of language'; (2) 'Healthcare professionals do not recognise the importance of context

The Acute Aphasia IMplementation Study (AAIMS): a pilot cluster randomized controlled trial.

BACKGROUND: Effective implementation strategies to improve speech and language therapists' (SLTs) aphasia management practices are needed. Australian SLTs working in the acute setting have reported inconsistent implementation of post-stroke aphasia guideline recommendations. Therefore, implementation efforts to address these gaps are necessary. However, little is known about the effectiveness of behaviour-change strategies in SLTs providing acute aphasia management. AIMS: This study designed and tested the feasibility, acceptability and potential effectiveness of a tailored implementation str

Calling the GP surgery: patient burden, patient satisfaction, and implications for training.

BACKGROUND: Good communication is central to the effectiveness of GP service provision, as well as to patient satisfaction with surgeries, but very little is known about the actual communication that occurs between patients and surgeries. AIM: This study was carried out to examine, for the first time, how receptionists interact with patients on the telephone, to identify features of communication associated with efficacy and patient satisfaction. DESIGN AND SETTING: A qualitative conversation analysis of incoming patient telephone calls, recorded 'for training purposes', in three English GP

Association of empathy of nursing staff with reduction of seclusion and restraint in psychiatric inpatient care.

OBJECTIVE: Disruptive behavior leading to seclusion or restraint increases with patients in a high-acuity stage of mental illness who have histories of aggressive behavior. The study examined whether greater nursing staff empathy skills and motivation reduced use of seclusion and restraint and whether empathy training can further this effect. METHODS: In 1,098 nursing shifts in 2 six-month periods one year apart, hierarchical analyses examined the effects of nursing shift and patient characteristics, the effect for each shift of nurses' skill and motivation to use empathy, and whether empathy

Working Alliance and Its Relationship With Treatment Outcome in a Sample of Aboriginal and Non-Aboriginal Sexual Offenders.

The relationship that develops between a client and therapist is arguably one of the most important factors toward achieving positive outcomes from therapy. The present study examined the therapeutic alliance, as measured by Horvath and Greenberg's Working Alliance Inventory (WAI), as a function of Aboriginal ancestry and the relationship of alliance to important program outcomes, in a Canadian correctional sample of 423 treated sexual offenders. The men rated their primary therapists on the WAI 3 months into treatment. Higher self-report ratings on the WAI and its Task, Bond, and Goal subscal

Talking about sex with health professionals: the experience of people with cancer and their partners.

Changes to sexuality can be one of the most difficult aspects of life following cancer. This study examines the experience of discussing sexuality post cancer with health care professionals (HCPs), from the perspective of women and men with cancer (PWC), and their partners (PPWC), across a range of cancer types. A total of 657 PWC (535 women, 122 men) and 148 PPWC (87 women, 61 men) completed a survey containing closed and open-ended items, analysed by analysis of variance and thematic analysis. Discussions about sexuality with a HCP were more likely to be reported by men (68%) compared to wom

Communication challenges for chronic metastatic cancer in an era of novel therapeutics.

Advances in the production of novel therapies for cancer management are creating new challenges for the support of increasing numbers of persons surviving for extended periods with advanced disease. Despite incurable and life-limiting metastatic conditions, these patients are living longer with serious disease, pushing the boundaries of what science explains and clinicians can confidently interpret using available evidence. Here we report findings from an early subset of such individuals within a longitudinal qualitative cancer cohort study on clinician-patient communication across the cancer
