The clinical experience reported by these studies does not definitively answer the question of whether or not all or even most tars are effective in the treatment of psoriasis. Cytokine levels were compared in aqueous extracts of stratum corneum from psoriatic lesions and normal heel. Samples from heel contained high levels of interleukin-1 alpha (IL-1 alpha) and beta measured in immunoassays, although only the IL-1 alpha was biologically active. No other cytokines could be detected in heel samples. Interleukin-1 (IL-1) levels were dramatically reduced in lesional samples. Knowledge of the papulosquamous disorders is an important part of the diagnosis of skin diseases in general. Recognition of these commonly encountered cutaneous problems depends upon familiarity with the configuration and distribution of lesions on various parts of the body. Diagnostic tests (such as KOH to rule out tinea) should be performed, and when therapy does not seem to be successful, the diagnosis should be reconsidered with a biopsy and/or dermatologic consultation.

Ten children, five with generalized pustular psoriasis and five with erythrodermic disease, were treated with etretinate and observed for up to seven years. In all those with pustular psoriasis, complete clearing was obtained in three weeks to four months, but in one child maintenance therapy had to be introduced twice, for one year each time. We have studied biopsies of lesional skin with monoclonal and polyclonal antibodies to determine the nature of the infiltrate in different forms of psoriasis. In guttate psoriasis the number of Langerhans cells (LC) was not increased and CD8+ cells predominated, however, most CD4+ cells were activated. We describe a patient whose clinical and histopathologic findings were originally interpreted as representative of actinic reticuloid but who later developed psoriasis with pustules. We propose that the original photosensitive eruption could have represented an unusual presentation of photosensitive psoriasis, although koebnerization of psoriasis into areas of photosensitivity remains a definite possibility.

We present the first report of the management of generalized pustular psoriasis (Zumbusch pattern) in a young child with etretinate (Tigason). Striking features were the low dosage required to control the disease (as little as 10 mg per week) and the eventual transition of the clinical features to those of psoriasis vulgaris.

Selective UV-Phototherapy with lambda 300-320 nm (SUP) as well as oral photochemotherapy with 8-methoxy-psoralene plus UVA-radiation (PUVA intern) are very effective in clearing the lesions of th generalized psoriasis and those of the chronic forms of parapsoriasis. Tissue typing was performed on lymphocytes of 46 Jewish patients with psoriasis vulgaris. HLA B13, B17, B37 and Cw6 were found increased. It is suggested that the increase in the B and C locus antigens is inter-related and may determine the genetic predisposition for the development of this disease.

HL-A typing was performed on 97 patients with pustular psoriasis. HLA-B27 was found increased for the combined three subgroups: localized psoriasis of palms and soles, acrodermatitis continua and generalized pustular psoriasis, who were associated with a high incidence of arthritis.
