From 1976 to 1983, the adult respiratory distress syndrome occurred in 14 patients during pregnancy or within a month postpartum. There were 8 survivors, giving a 43% mortality. All but 2 patients had obstetric-related precipitating events--labor problems, infections, eclampsia-toxemia, and obstetric hemorrhages. During emergency cesarean sections, 3 patients had respiratory problems that may have caused their respiratory distress syndrome. The average duration of mechanical ventilatory support was 16 days. An association between the development of postpartum hypothyroidism and high levels of IgG1 subclass microsomal (M) antibodies has been reported. Using an assay designed to detect reasonable levels of all the four IgG subclasses, we found no differences in the proportion of each IgG subclass in M antibodies of patients with postpartum hyperthyroidism or hypothyroidism compared with control postpartum patients with M antibodies but no thyroid dysfunction. Thyroid evaluation was performed in 85 women at childbirth, then between 2 and 7 months, and 14 months later. Transient subclinical hypothyroidism was found in one woman; antithyroid antibodies were found in 10 and were persistent in 6, suggesting a late thyroiditis. For these 6 cases, 5 HLA typing showed 3 Locus DR4. The proportion of antithyroid antibody was the same in a reference population. This case study describes the diagnosis of a thrombosis of the inferior vena cava in the course of a sonographic examination of the upper abdomen. Using ultrasound, large abdominal vessels can be examined easily. Following diagnosis, sonographic examinations under intensive care allowed observation of the further progress of the thrombus up to its dissolution.

We report the case of a woman with hereditary porphyria cutanea tarda which manifested 3 weeks after she gave birth to her second child. The mother of the patient had also been diagnosed and treated for porphyria cutanea tarda. Reduced red cell uroporphyrinogen decarboxylase activity was found in the patient, the new-born child and the patient's mother. Normal enzyme activity was found in the patient's first child.

Renal diseases occur in intravenous drug abusers, especially heroin addicts, in the form of interstitial nephritis, nephrotic syndrome or acute renal failure due to rhabdomyolysis. We report a case of acute renal failure not ascribable to rhabdomyolysis nor to the main pathogenetic mechanisms of pregnancy-related acute renal failure in a pregnant heroin addict woman after vaginal delivery following uncomplicated pregnancy. Drug-related immunological abnormalities and microcirculatory distress may be involved.

Gentamicin is an aminoglycoside antibiotic frequently used in the treatment of mixed polymicrobial infections. Because patients with preeclampsia exhibit compromised renal function, decreased intravascular volume, and increased extravascular fluid, it was theorized gentamicin may be handled differently in normotensive versus preeclamptic patients. In a young woman a strong carpal-tunnel-syndrome was observed after everey of her three deliveries, apparently at the termination of the lactation. Pain and tingling in the arms and hands was very good to be influenced by diuretics. It has be concluded from these facts, that this type of carpal-tunnel-syndrome is to trace back to a temporary generalized oedema, probable caused by the hypothalamical oxytocin-producing centre.

A patient in premature labour was treated with an intravenous salbutamol infusion for 56 hours before delivery by Caesarean section. Ergometrine was given 45 minutes after the salbutamol infusion was stopped and the patient developed pulmonary oedema postoperatively. We disucss the pharmacological implications of the interaction between these drugs and their effect on the sympathetic system.

An 18-year-old primipara developed acute hypertension leading to cerebral edema and convulsions following the IV injection of a bolus of 10 units of oxytocin with 0.2 mg methylergonovine maleate. Oxytocin in a dose of more than 2 units should not be administered IV in a single injection, as severe hypotension may result.
