Risk Factors Associated With Quantitative Evidence of Lung Emphysema and Fibrosis in an HIV-Infected Cohort.

INTRODUCTION: The disease spectrum for HIV-infected individuals has shifted toward comorbid non-AIDS conditions including chronic lung disease, but quantitative image analysis of lung disease has not been performed. OBJECTIVES: To quantify the prevalence of structural changes of the lung indicating emphysema or fibrosis on radiographic examination. METHODS: A cross-sectional analysis of 510 HIV-infected participants in the multicenter Lung-HIV study was performed. Data collected included demographics, biological markers of HIV, pulmonary function testing, and chest computed tomographic exami

The role of pulmonary hypertension in the pathogenesis of bronchial asthma.

In patients with bronchial asthma, when blood pressure in the pulmonary artery is measured, very often hypertension is registered. This pulmonary hypertension is not secondary to hypoventilation, as in patients with chronic bronchitis or pulmonary emphysema, since patients with bronchial asthma are hyperventilating. The pulmonary hypertension in them must be therefore classified as primary. It is probable that its origin is in an allergic vasoconstriction of the pulmonary artery, occurring simultaneously with the well known bronchoconstriction.

Artificial Intelligence-derived Measurements of Myosteatosis from Coronary Artery Calcium CT Scans to Predict COPD: The Multi-Ethnic Study of Atherosclerosis.

Purpose To evaluate the predictive value of myosteatosis as an opportunistic finding in coronary artery calcium (CAC) CT scans for clinically diagnosed chronic obstructive pulmonary disease (COPD) and compare it with an artificial intelligence (AI)-measured biomarker of emphysema derived from the same scans. Materials and Methods In this prospective study, baseline CAC CT scans and 20-year follow-up data were analyzed. Myosteatosis was defined as the lowest quartile of thoracic skeletal muscle mean attenuation (males < 33.5 HU, females < 27.0 HU). The emphysema-like lung biomarker was quantifi

Association of incidental emphysema with annual lung function decline and future development of airflow limitation.

OBJECTIVES: Emphysema is one of the prognostic factors for rapid lung function decline in patients with COPD, but the impact of incidentally detected emphysema on population without spirometric abnormalities has not been evaluated. This study aimed to determine whether emphysema detected upon computed tomography (CT) screening would accelerate the rate of lung function decline and influence the possibility of future development of airflow limitation in a population without spirometric abnormalities. MATERIALS AND METHODS: Subjects who participated in a routine screening for health checkup and

Pulmonary cysts in progressive systemic sclerosis (scleroderma).

A young man with progressive systemic sclerosis (PSS) and pulmonary complaints exhibited enlarging lung cysts on chest x-ray. These rather uncommon findings in PSS result from alveolar dissolution and obstructive emphysema. Diffuse pulmonary fibrosis is basic to this process as well as to the more familiar pulmonary roentgen changes in PSS.

[Pneumopericardium after hyperemesis. Possible result of the Macklin effect].

Pneumopericardium is a rare but serious finding. It may indicate severe chest trauma and be associated with tracheobronchial and oesophageal injuries. On the other hand, oesophageal rupture such as Boerhaave's syndrome may also be accompanied by pneumopericardium. A case of spontaneous pneumopericardium and pneumomediastinum after alcohol-induced emesis is reported. Tracheal, bronchial, and oesophageal injury were excluded by radiology and endoscopy. Interstitial lung emphysema is discussed as the pathogenesis.

A Plant-Centered Diet is Inversely Associated With Radiographic Emphysema: Findings from the CARDIA Lung Study.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a significant public health concern and intercepting the development of emphysema is vital for COPD prevention. Smokers are a high-risk population for emphysema with limited prevention strategies. We aimed to determine if adherence to a nutritionally rich, plant-centered diet among young ever-smokers is associated with reduced risk of future radiographic emphysema. METHODS: We studied participants from the Coronary Artery Risk Development in Young Adults (CARDIA) Lung Prospective Cohort Study who were 18-30 years old at enrollment an

Functional associations of pleuroparenchymal fibroelastosis and emphysema with hypersensitivity pneumonitis.

BACKGROUND: Pleuroparenchymal fibroelastosis (PPFE) has been described in hypersensitivity pneumonitis (HP) yet its functional implications are unclear. Combined pulmonary fibrosis and emphysema (CPFE) has occasionally been described in never-smokers with HP, but epidemiological data regarding its prevalence is sparse. CTs in a large HP cohort were therefore examined to identify the prevalence and effects of PPFE and emphysema. METHODS: 233 HP patients had CT extents of interstitial lung disease (ILD) and emphysema quantified to the nearest 5%. Lobar percentage pleural involvement of PPFE was

[Signs of right heart stress in diurnal normoxemic patients with chronic obstructive lung disease and nocturnal hypoxemia].

Nocturnal hypoxemia in daytime normoxemic patients with COPD may lead to an increased right ventricular afterload due to pulmonary hypertension. We investigated the frequency of clinical, electrocardiographical, and radiological signs of right cardiac insufficiency (SRCI) in 178 consecutive COPD-patients [71 bronchitis, 25 emphysema, 82 bronchitis plus emphysema; PaO2 = 60 mm Hg]. Patients with asthma, left ventricular impairment, obstructive sleep apnea syndrome, primary pulmonary hypertension, and neuromuscular diseases were excluded. Polysomnography was performed in all patients. They were

[Negative Pressure Wound Therapy as a Treatment Option for Extensive Post-Trauma Subcutaneous Emphysema].

Subcutaneous emphysema can be a secondary complication of chest trauma or one of the complications of ruptured bullae in advanced chronic obstructive pulmonary disease. Massive subcutaneous emphysema impairs the respiratory mechanics and affects the venous returns of the head and neck. It can lead to respiratory insufficiency with the need for mechanical ventilation. The treatment should focus on the primary pathology. Nonetheless, in patients with subcutaneous emphysema as the only but serious symptom, the treatment can zero in solely on this complication. The standard procedure consists in t

[High-resolution computed tomography study (HRCT) in so-called emphysema B. The correlations between the radiogram, the anatomical findings yielded by high-resolution computed tomography and the clinico-functional assessment].

The results are reported of a prospective study on 28 emphysematous patients with clinical, functional and radiologic correlations. Thirteen of 28 patients had type-B, the clinical "blue and bloater", emphysema. The grade of emphysema was investigated on conventional chest films and on HRCT scans, with a visual scoring system. Hyperinflation was assessed on conventional chest films and correlated with the extent of emphysema on HRCT images and with functional impairment. In our series, the extent of emphysema in B-type patients was markedly lower than in other patients of the same age, with th

Evidence that genetic emphysema in tight-skin mice is not caused by neutrophil elastase.

Mutant tight-skin mice that had severe pulmonary emphysema were crossed with beige mice that contained neutrophils deficient in elastase activity. The resultant tight skin-beige cross mice were deficient in neutrophil elastase activity, yet they were still afflicted with the same degree of severe emphysema as the tight-skin mice. The results indicate that neutrophil elastase does not cause the emphysematous lesions found in tight-skin mice.

[Physiological effects of lung reduction surgery: the contribution of an animal model of emphysema].

INTRODUCTION: Lung volume reduction surgery (LVRS) is a technique largely used for palliation of symptoms in selected patients with emphysema. Despite some encouraging early results, it is often still regarded as an experimental technique due to the significant associated mortality and unpredictable results. STATE OF ART: The aim of this study was to explore the pathophysiological basis of benefit from LVRS when applied to a hamster model of emphysema induced by elastase and thus allow better selection criteria of patients being considered for this treatment. PERSPECTIVES: Despite a positive

[Pulmonary structure and function].

Computed tomography (CT) is the method of choice for the morphological assessment of pulmonary structure. Airways. pulmonary parenchyma and their pathologies are shown with high spatial resolution. Airway diseases exhibit bronchial dilatation, wall thickening and an increased visibility in the lung periphery. CT is also the imaging gold standard for the detection and the characterisation of emphysema because a high degree of concordance with histopathological studies. Centrilobular, panlobular and paraseptal emphysema can be differentiated. CT can also provide functional information. Paired in

Bronchopleural fistula: successful selective bronchial occlusion with a Fogarty's catheter in a preterm infant.

A preterm infant (26 weeks' gestation) mechanically ventilated for respiratory distress syndrome developed severe interstitial emphysema of the right lung with a bronchopleural fistula, pneumothorax and mediastinal shift. Selective occlusion of the right main bronchus with a Fogarty's catheter produced rapid improvement in the clinical condition and radiological features. Occlusion of the main bronchus in a newborn with a bronchopleural fistula and pulmonary interstitial emphysema is an easily performed manoeuvre that can be life-saving.

Progress in Imaging COPD, 2004 - 2014.

Computed tomography (CT) has contributed substantially to our understanding of COPD over the past decade. Visual and quantitative assessments of CT in COPD are complementary. Visual assessment should provide assessment of centrilobular, panlobular and paraseptal emphysema, airway wall thickening, bronchiectasis, findings of respiratory bronchiolitis, and enlargement of the pulmonary artery. Quantitative CT permits evaluation of severity of emphysema, airway wall thickening, and expiratory air trapping, and is now being used for longitudinal evaluation of the progression of COPD. Innovative tec

Selective bronchial intubation in infants with lobar emphysema: indications, complications, and long-term outcome.

Thirteen infants underwent selective intubation of a mainstem bronchus (SBI) for lobar emphysema of varying etiologies. Seven infants had pulmonary interstitial emphysema (PIE) with lobar hyperinflation secondary to hyaline membrane disease and mechanical ventilation. Six of these improved with SBI and five maintained permanent resolution upon cessation of SBI. Two infants with localized areas of emphysema who were subsequently shown to have histologic evidence of bronchopulmonary dysplasia had unsuccessful SBI. SBI was also unsuccessful in permanently resolving congenital lobar emphysema alth

State of the art. Mechanistic heterogeneity in chronic obstructive pulmonary disease: insights from transgenic mice.

Alveolar destruction is a cardinal feature of emphysema but is not traditionally believed to contribute to the pathogenesis of "classical" asthma. However, the relationship between chronic obstructive pulmonary disease (COPD) and asthma is controversial and the variety of mechanisms that can mediate the alveolar destruction in emphysema have not been adequately defined. To address these issues, we used overexpression transgenic approaches to define the effects of Th1/Tc1 and Th2/Tc2 cytokines in the mature murine lung and compared findings in these transgenic systems to the effects of similar
