Anatomically Precise Microsurgical Resection of a Posterior Fossa Cerebellar Metastasis in an Elderly Patient with Preservation of Venous Outflow, Dentate Nucleus, and Cerebrospinal Fluid Pathways.

Background and Clinical Significance: Adults suffering from cerebellar metastases are often at high risk for rapid deterioration of their neurological status because the posterior fossa has limited compliance and the location of these metastases are close to the brain stem and important cerebrospinal fluid (CSF) pathways. In this paper, we present a longitudinal, patient-centered report on the history of an elderly individual who suffered from cognitive comorbidities and experienced a sudden loss of function in her cerebellum. Our goal in reporting this case is to provide a comparison between

Telemedicine Evaluations in Neuro-Ophthalmology During the COVID-19 Pandemic: Patient and Physician Surveys.

BACKGROUND: The novel coronavirus 2019 (COVID-19) pandemic has transformed health care. With the need to limit COVID-19 exposures, telemedicine has become an increasingly important format for clinical care. Compared with other fields, neuro-ophthalmology faces unique challenges, given its dependence on physical examination signs that are difficult to elicit outside the office setting. As such, it is imperative to understand both patient and provider experiences to continue to adapt the technology and tailor its application. The purpose of this study is to analyze both neuro-ophthalmology physi

A COMPARISON OF THE PAPER AND COMPUTERIZED TABLET VERSION OF THE KING-DEVICK TEST IN COLLEGIATE ATHLETES AND THE INFLUENCE OF AGE ON PERFORMANCE.

BACKGROUND: Sport-related concussion is a public concern with between 1.6 and 3.8 million sport- and recreation-related injuries occurring annually. An estimated 65% to 90% of concussed athletes show oculomotor disruption such as difficulty with saccades, accommodation, smooth pursuit, and fixation. A rapid number-naming saccade test, the King-Devick (K-D) test, has shown promising results as part of a multifaceted concussion assessment tool. PURPOSE: The purpose of the current study was to evaluate the two versions of the K-D in collegiate aged (18-24) athletes to determine the agreement bet

Vestibular Test Results of a Patient with Cerebral Microangiopathy and Idiopathic Thrombocytopenia.

BACKGROUND: Cerebral microangiopathy (CM) is a general term for small ischemic changes related to small blood vessels in the brain. Immune thrombocytopenia (ITP) is a rare acquired autoimmune disease that is characterized by low platelet count and increased risk of hemorrhage. We describe vestibular testing completed on a 64-year-old patient with CM and ITP presenting with complaints of general instability and listing while walking, decreased stability in dark environments, and blurred vision with head movement. Results indicated a severe loss of horizontal semicircular canal function bilatera

The Standardized Field Sobriety Tests (SFST) and measures of cognitive functioning.

OBJECTIVE: The Standardized Field Sobriety Tests (SFST) are utilised widely to assess fitness to drive when law enforcement suspects a driver's ability to drive is impaired, whether by drugs or alcohol. The SFST ostensibly achieve this through assessment of the level of drivers' cognitive and psychomotor impairment, although no studies have explicitly assessed the relatedness of cognitive ability and performance on the SFST. The current study aimed to assess the relationship between the three components of the SFST with a well validated computerised cognitive battery. METHOD: A sub-set of 61

Characterization of ocular motor deficits in congenital facial weakness: Moebius and related syndromes.

Congenital facial weakness is present in a heterogeneous group of conditions. Among them is Moebius syndrome, which has been defined as a disorder with congenital, non-progressive facial weakness and limited abduction of one or both eyes. It is typically attributed to agenesis of the abducens and facial cranial nerves. This paper details ocular motor findings of 40 subjects (23 months to 64 years; 24 females, 16 males) with congenital facial weakness: 38 presented at a Moebius Syndrome Conference and two were clinic patients. A new classification scheme of patterns based on ocular motor phenot

[Smoking and schizophrenia: epidemiological and clinical features].

FREQUENCY: The prevalence of cigarette smoking is significantly higher among patients with schizophrenia (60-90%) than in the general population (23-30%). While tobacco smoking decreases in the general population (from 45% in the 1960's to 23-30% in the 2000's), smoking in patients with schizophrenia remains high. Patients with schizophrenia smoke more cigarettes than control subjects. Patients smoke more deeply, thereby increasing their exposure to the harmful elements in tobacco smoke. IMPACT OF SMOKING IN SCHIZOPHRENIC PATIENTS: As in the general population, smoking contributes to the reduc

Assessing Cognition in Kidney Failure Using Virtual Reality Technology: A Clinical Research Protocol.

BACKGROUND: Cognitive impairment is common in patients receiving dialysis and is associated with morbidity and mortality. Existing approaches to administering face-to-face cognitive screening assessments like the Montreal Cognitive Assessment (MoCA) may be challenging to undertake in dialysis. Virtual reality (VR) technology may be a novel way to assess cognitive function in patients on dialysis. OBJECTIVE: In a cohort of patients undergoing hemodialysis, the primary objective of this study is to evaluate the test-retest reliability, diagnostic performance, and agreement of an MoCA, generated

Feasibility of unipolar signal-guided ablation in creating contiguous lines of conduction block: A proof-of-concept study.

BACKGROUND: Modification of the atrial unipolar electrogram (Uni-EGM) with loss of the near-field negative component in response to radiofrequency (RF) ablation reflects transmural loss of tissue conductivity. OBJECTIVE: This study sought to investigate the feasibility of Uni-EGM morphology-guided RF ablation in generating contiguous, transmural lesions that result in conduction block. This method was compared with ablation guided by standard ablation index parameters. METHODS: In a beating heart swine model, linear transcaval ablation was performed using an irrigated ablation catheter with

Is Thoracic Kyphosis Relevant to Pain, Autonomic Nervous System Function, Disability, and Cervical Sensorimotor Control in Patients with Chronic Nonspecific Neck Pain?

There is great interest in thoracic kyphosis, as it is thought to be a contributor to neck pain, neck disability, and sensorimotor control measures; however, this has not been completely investigated in treatment or case control studies. This case control design investigated participants with non-specific chronic neck pain. Eighty participants with a defined hyper-kyphosis (>55°) were compared to eighty matched participants with normal thoracic kyphosis (<55°). Participants were matched for age and neck pain duration. Hyper-kyphosis was further categorized into two distinct types: postural kyp

A Positive Vestibular/Ocular Motor Screening (VOMS) Is Associated With Increased Recovery Time After Sports-Related Concussion in Youth and Adolescent Athletes.

BACKGROUND: Vestibular and ocular motor impairments are routinely reported in patients with sports-related concussion (SRC) and may result in delayed return to play (RTP). The Vestibular/Ocular Motor Screening (VOMS) assessment has been shown to be consistent and sensitive in identifying concussion when used as part of a comprehensive examination. To what extent these impairments or symptoms are associated with length of recovery is unknown. PURPOSE: To examine whether symptom provocation or clinical abnormality in specific domains of the VOMS results in protracted recovery (time from SRC to

Binocular coordination in fore/aft motion.

Stabilization of images on the fovea during either fore/aft translation of a subject or fore/aft movement of a visual target in front of a stationary observer imposes complex geometrical requirements that depend upon the eccentricity of the object of interest with respect to the eyes. Each eye needs to be rotated independently with varying proportions of conjugate (version) and disconjugate (vergence) eye movements to maintain fixation of the target. Here, we describe binocular coordination in the early response to translational movements of normal subjects along their naso-occipital axis. We

Visual vestibular interaction: vestibulo-ocular reflex suppression with head-fixed target fixation.

In order to maintain clear vision, the images on the retina must remain reasonably stable. Head movements are generally dealt with successfully by counterrotation of the eyes induced by the combined actions of the vestibulo-ocular reflex (VOR) and the opto-kinetic reflex. We have studied how, in humans, the VOR gain (VORG) is modulated to provide appropriate eye movements in two situations: 1. fixation of a stationary object of the visual space while the head moves. This requires a visuo-vestibulo-ocular reaction to induce eye movements opposite in direction, and equal in velocity to head move

Efficacy of high-frequency rTMS in the treatment of gait disorder and cognition in patients with Parkinson's disease based on wearable devices and eye-movement assessments.

BACKGROUND: Postural instability and gait disorder and cognitive dysfunction are common symptoms of Parkinson's disease (PD). Scale assessment is frequently used in the clinic to evaluate PD, but this technique is limited by its lack of sensitivity to changes in disease progression and its difficulty in capturing subtle movements and changes in cognitive function. It is currently believed that high-frequency repetitive transcranial magnetic stimulation (rTMS) can improve motor and cognitive dysfunction in patients with PD, though it remains controversial. Therefore, it is imperative to monitor

[Acoustic neuroma: clinical-functional finding, results and surgical complication].

The present work provides clinical-functional findings, results and surgical complications observed in a consecutive series of 100 subjects with acoustic neuroma (AN). Analysis of the data has made it possible to draw some important conclusions. Compromised hearing is found in 90% of the ears affected by AN. Indeed the percentage of normal hearing in such cases does not exceed 5%. There is, however, no clear correlation between degree of hearing and tumor size. The symptoms of AN do not always present unilateral or asymmetrical hearing loss, unilateral tinnitus and/or dizziness. At times AN pr

Cognitive performance in healthy clinical trial participants and patients with the NeuroCart, a neurodegenerative disease measured with an automated neuropsychological and neurophysiological test battery.

BACKGROUND: The prevalence of neurodegenerative diseases increases significantly with increasing age. Neurodegeneration is the progressive loss of function of neurons that eventually leads to cell death, which in turn leads to cognitive disfunction. Cognitive performance can therefore also be considered age dependent. The current study investigated if the NeuroCart can detect age related decline on drug-sensitive CNS-tests in healthy volunteers (HV), and whether there are interactions between the rates of decline and sex. This study also investigated if the NeuroCart was able to differentiate
