Target trial emulation has contributed to improvements in the design and interpretation of observational studies aimed at causal inference. By encouraging researchers to think explicitly about the design of a hypothetical randomized trial, it has helped identify and address avoidable design flaws. However, the framework cannot overcome biases and limitations inherent in observational data. Despite the aim of emulating a randomized trial, observational data cannot reproduce random treatment assignment or blinding, and may not necessarily provide greater generalizability than a randomized trial.

Neuropathic pain remains a complex condition to manage. While current literature provides best practice guidelines for the use of gabapentinoids in this indication, these recommendations are primarily based on randomised clinical trials (RCTs) and meta-analyses of varying methodological quality. To support evidence-based prescribing, we conducted an evaluation of the overall confidence in meta-analytic findings on the efficacy of gabapentinoid in adults. A systematic review of the meta-analyses of RCTs retrieved from the MEDLINE (PubMed) database was performed. The methodological quality of th

RATIONALE: Tailored implementation strategies are frequently recommended to improve healthcare professional practice. Tailoring involves the selection and design of strategies to address context-specific barriers (referred to as determinants of practice) to best practice recommended in clinical guidelines. Improvements in practice are thought to be more likely if implementation strategies are selected to address identified practice determinants. This is an update of a review published in 2015.
OBJECTIVES: To assess the effect of tailored implementation strategies, compared with a non-tailored

BACKGROUND: Despite accounting for a substantial proportion of the global population and disease burden, African countries are underrepresented in randomized controlled trials (RCTs), including those informing cardiovascular (CV) care.
OBJECTIVES: In this study, we sought to quantify African representation in RCTs published from 2019 to 2024 in: 1) 5 leading general medical journals; and 2) 3 leading CV journals.
METHODS: We conducted a systematic review of RCTs published from 2019 to 2024 in the British Medical Journal, the Journal of the American Medical Association, The Lancet, Nature Med

OBJECTIVES: To assess the concordance of results between paired target trial emulations and their benchmarking randomised controlled trials and to explore determinants of emulation success.
DESIGN: Systematic review and meta-analysis.
DATA SOURCES: Medline, EMBASE, Scopus, PsycINFO, and Web of Science (2010 to 2025).
STUDY SELECTION: Observational studies that explicitly aimed to emulate target randomised controlled trials of a health intervention were included.
DATA EXTRACTION: Data were extracted on study domain, target population, intervention, comparator, and main outcomes for each emu

BACKGROUND AND OBJECTIVES: The aim of the INveStigating ProblEmatic Clinical Trials in Systematic Reviews (INSPECT-SR) project is to develop a tool to identify problematic RCTs in systematic reviews. In stage 1 of the project, a list of potential trustworthiness checks was created. The checks on this list must be evaluated to determine which should be included in the INSPECT-SR tool.
METHODS: We attempted to apply 72 trustworthiness checks to randomized controlled trials (RCTs) in 50 Cochrane reviews. For each, we recorded whether the check was passed, failed, or possibly failed or whether it

PURPOSE: Owing to the recommendations of the Surviving Sepsis Campaign guidelines, protocol-based resuscitation or goal-directed therapy (GDT) is broadly advocated for the treatment of septic shock. However, the most recently published trials showed no survival benefit from protocol-based resuscitation in septic shock patients. Hence, we aimed to assess the effect of GDT on clinical outcomes in such patients.
MATERIALS AND METHODS: We performed a systematic review that included a meta-analysis. We used electronic search engines including PubMed, Embase, and the Cochrane database to find studi

AIM: To analyse the effectiveness of education about pain, quality of life and functionality in patients with fibromyalgia.
SUBJECTS AND METHODS: The search for articles was carried out in electronic databases. Eligibility criteria were: controlled randomised clinical trials (RCT), published in English and Spanish, that had been conducted on patients with fibromyalgia, in which the therapeutic procedure was based on patient education. Two independent reviewers analysed the methodological quality using the PEDro scale.
RESULTS: Five RCT were selected, of which four offered good methodological

BACKGROUND: Atopic keratoconjunctivitis (AKC) is a chronic ocular surface non-infectious inflammatory condition that atopic dermatitis patients may suffer at any time point in the course of their dermatologic disease and is independent of its degree of severity. AKC is usually not self resolving and it poses a higher risk of corneal injuries and severe sequelae. Management of AKC should prevent or treat corneal damage. Although topical corticosteroids remain the standard treatment for patients with AKC, prolonged use may lead to complications. Topical cyclosporine A (CsA) may improve AKC signs

In 1754, aboard HMS Salisbury, James Lind conducted a simple, controlled clinical trial. He took 12 patients with "pale and bloated skin, listlessness, an aversion to exercise, swollen gums, halitosis, ecchymotic mucous membranes, and limb edema" and allocated them to receive treatment with one of six different therapies. Since the patients receiving two of his six chosen interventions had such a dramatic recovery, he felt ethically obligated to end his trial and administer these treatments to all the remaining sailors. Today we fully recognize the impact that the controlled clinical trial can

RATIONALE: While substantial research has evaluated the safety of hemoglobin thresholds and storage length for red blood cell (RBC) transfusion, there is minimal literature on RBC transfusion volume per transfusion event in hospitalized patients. The Choosing Wisely initiative has made a recommendation to transfuse a single unit of RBC per transfusion event, although this is based on little evidence. No recommendation has been made for pediatrics. This review evaluates the effect of the volume of RBC transfusion, administered when the decision has been made to transfuse a hospitalized patient,

The PRObiotic human study Design And REporting recommendations provide a structured framework for designing and reporting human studies with probiotics, addressing challenges related to product viability. Five main study types are identified. Exploratory studies are early-phase, first-in-human investigations designed to evaluate effects or generate hypotheses and require specification of strain, dose, stability, and assessment of acceptance and adverse events. Pilot studies assess the feasibility of protocols and logistics before larger trials, with clearly defined objectives, compliance measu

OBJECTIVE: Diabetic foot ulcer (DFU) is a life-threatening complication associated with high amputation and mortality rates. While numerous topical therapies exist, their comparative efficacy and safety remain unclear. We aimed to compare the efficacy and safety of topical therapies for DFUs to identify optimal treatment strategies.
RESEARCH DESIGN AND METHODS: We searched PubMed, Embase, and CENTRAL from inception to February 1, 2025, for randomized clinical trials (RCTs) evaluating topical therapies for DFUs. We excluded non-topical interventions to ensure clinically relevant comparisons. A
