MR imaging of Rectal Cancer.

MR imaging strengthens contemporary management of rectal cancer by depicting tumor extent, mesorectal fascia proximity, and nodal disease with exquisite anatomic detail. Both standardized technique and synoptic reporting improve reproducibility and multidisciplinary planning. Accurate T-category assignment, measurement of the circumferential resection margin, and recognition of extramural vascular invasion/tumor deposits stratify risk and help to triage patients into surgical, neoadjuvant or "watch-and-wait" pathways. Station-based evaluation of lateral pelvic nodes is critical to develop loca

A Consensus Approach to the Incorporation of Total Neoadjuvant Therapy in a Treatment Algorithm for Stage I-III Resectable Rectal Cancer.

Advances in surgical techniques, radiographic imaging capabilities, radiotherapy, and chemotherapy have led to improved outcomes for patients with rectal adenocarcinoma. Treatment strategies have correspondingly evolved, as seen with total neoadjuvant therapy (TNT) and organ preservation approaches. TNT is a treatment strategy for primary, non-metastatic, resectable mismatch repair proficient rectal cancer where the intent is to administer all appropriate adjuvant therapy in the preoperative phase, including both systemic therapy and chemoradiotherapy/radiotherapy. In this setting, TNT is incr

Rectal neuroendocrine tumor (G1) with widespread metastasis 8 years after endoscopic resection.

AbstractRectal neuroendocrine tumor (NET) G1 is a docile-looking tumor. We herein report a 77-year-old male whose rectal NET G1 measuring 10 mm recurred eight years after endoscopic mucosal resection. Tumor recurrence was detected by his mild general fatigue. Abdominal computed tomography revealed widespread metastasis, including the liver, lymph nodes around the rectum, and cervical spines. Subsequent somatostatin receptor scintigraphy, as well as histological reassessment of previously resected specimen, proved that these metastatic diseases originated from the rectal NET G1 with vascular in

Neoadjuvant treatment of colorectal cancer: comprehensive review.

BACKGROUND: Neoadjuvant therapy has an established role in the treatment of patients with colorectal cancer. However, its role continues to evolve due to both advances in the available treatment modalities, and refinements in the indications for neoadjuvant treatment and subsequent surgery. METHODS: A narrative review of the most recent relevant literature was conducted. RESULTS: Short-course radiotherapy and long-course chemoradiotherapy have an established role in improving local but not systemic disease control in patients with rectal cancer. Total neoadjuvant therapy offers advantages ov

Brachytherapy for rectal and anal cancer: Where are we and current perspectives?

Anal and rectal cancers were some of the first disease sites treated with brachytherapy due to the anatomic ease of implantation. As external beam radiotherapy grew in popularity the use of ano-rectal brachytherapy declined. However, the past few years have seen a steady resurgence in the use of brachytherapy in the ano-rectum supported by the use of large clinical series and randomized trials. The increasing acceptance by the surgical community of organ preservation as a valid treatment strategy for rectal cancer has encouraged the use of nonsurgical approaches and brachytherapy has shown its

Global productivity and research trends of colorectal carcinoma: A scientometric analysis of studies published between 1980 and 2021.

Although colorectal cancer (CRC) is a serious cause of death and has a significant impact on patients' quality of life and incidence rate of CRC has increased among the younger populations, bibliometric research of CRC has not been conducted yet. To perform a comprehensive analysis of scientific publications on CRC using various statistical and bibliometric techniques. Publications on CRC published between 1980 and 2021 were downloaded from the Web of Science database and analyzed using statistical methods. The trending topics, collaborations among countries, and citation relationships were an

Endoscopy Reports for Rectosigmoid Tumors: Frequently Missing Key Information Needed for Surgical Planning.

INTRODUCTION: Endoscopy reports by gastroenterologists describing rectosigmoid tumors often are missing crucial data for surgical planning, leading to high rates of repeat exams before surgical decision-making. We hypothesize that there will be significant deficiencies in the endoscopic reporting of rectosigmoid lesions leading to high rates of repeat endoscopic examination at our institution. METHODS: Retrospective review from January 2016 to November 2019 included 188 patients with rectosigmoid lesions referred for surgery with an outside endoscopy report. Three criteria were abstracted fro

Success rate of acquiring informed consent and barriers to participation in a randomized controlled trial of laparoscopic versus open surgery for non-curative stage IV colon cancer in Japan (JCOG1107).

BACKGROUND: Successful achievement of randomized controlled trials (RCTs) is dependent on the acquisition of informed consent (IC) from patients. The aim of this study was to prospectively calculate the proportion of participation in a surgical RCT and to identify the reasons for failed acquisition of IC. METHODS: A 50-insitution RCT was conducted to evaluate oncological outcomes of open and laparoscopic surgery for stage IV colon cancer (JCOG1107: UMIN-CTR 000000105). The success rate of obtaining IC was evaluated in eight periods between January 2013 and January 2021. In addition, reasons f

Imaging standardisation in metastatic colorectal cancer: A joint EORTC-ESOI-ESGAR expert consensus recommendation.

BACKGROUND: Treatment monitoring in metastatic colorectal cancer (mCRC) relies on imaging to evaluate the tumour burden. Response Evaluation Criteria in Solid Tumors provide a framework on reporting and interpretation of imaging findings yet offer no guidance on a standardised imaging protocol tailored to patients with mCRC. Imaging protocol heterogeneity remains a challenge for the reproducibility of conventional imaging end-points and is an obstacle for research on novel imaging end-points. PATIENTS AND METHODS: Acknowledging the recently highlighted potential of radiomics and artificial in

Metastatic Penile Adenocarcinoma in the Context of Rectal Cancer.

The presence of penile metastatic lesions proceeding from primary rectal tumors is a rare entity usually associated with a poor prognosis. Clinical presentation and localization may vary. There exists no consensus gold standard treatment for penile metastatic lesions, and there is continuous debate on whether lesions should undergo surgical, chemotherapeutic or palliative management.
