We report a case of Bartonella henselae neuroretinitis with significant disc and peripapillary edema, branch retinal artery occlusion without macula involvement and well preserved central vision. A 15-year-old female presented with loss of vision over 4 weeks in the left eye. She had a history of cat exposure, but a cat scratch, insect bite or conjunctivitis was not reported. An inferotemporal arcuate scotoma developed during the acute phase and persisted over the course of the follow-up.

Central retinal artery occlusion (CRAO) causes ischemic stroke of the eye. We report a case of CRAO that was successfully treated with intravenous recombinant tissue plasminogen activator (rt-PA) and review the current literature. A 64-year-old right-handed man presented to the emergency department with acute left eye amaurosis. An ophthalmologic assessment revealed a left afferent pupillary defect, minimal visual acuity, macular edema with a cherry red spot, and multiple emboli in the inferotemporal arcade of the left eye. A neurologic examination was otherwise nonfocal; neuroimaging was norm…

PURPOSE: To describe combined central retinal vein occlusion and central retinal artery occlusion in systemic lupus erythematosus patient.
METHODS: An 18-year-old woman came to our clinic complaining of sudden marked visual loss in her right eye. Best-corrected visual acuity was only hand movement in the right eye. An examination of the right fundus revealed a pale and edematous retina with widespread flame hemorrhages, a swollen optic disk, engorged and tortuous retinal veins, and a cherry-red spot, leading to a diagnosis of combined central retinal vein occlusion and central retinal artery …

UNLABELLED: We present a case as a rare sign of traumatic optic neuropathy and central retinal artery occlusion following blunt ocular trauma. A 10-year-old child suffered complete loss of the vision of one eye following a blunt ocular injury. He sustained an occlusion of the central retinal artery and traumatic optic neurupathy of the affected eye. Isolated cases of central retinal vessel occlusions and traumatic optic neurapathy following ocular blunt trauma are rare conditions. Clinicians to be aware of the potential for blunt ocular trauma to cause optic nerve damage and retinal vessel occ…

The etiology of central retinal artery occlusion (CRAO) is usually associated with atherosclerotic risk factors and the presence of intravascular (carotid artery, aortic arch) or intracardiac embolic material. At young age, CRAO may be a manifestation of inherited or acquired thrombophilia. The authors present a case of CRAO in a 21-year-old, previously healthy, non-smoking male, with neither metabolic nor hemostatic abnormalities and with normal echocardiography. However, he had a positive family history of premature coronary heart disease and a history of anabolic-androgenic steroids (AAS) u…

PURPOSE: Debate exists in the literature on the prevalence and etiology of neovascularization following central retinal artery occlusion (CRAO). The reported prevalence varies from 2.5% to 31.6%. We conducted a retrospective study to determine the prevalence of ocular neovascularization following acute CRAO in our institution.
METHODS: A retrospective audit of consecutive patients with nonarteritic/thromboembolic CRAO presenting between 1997 and 2009 in a single tertiary teaching hospital.
RESULTS: Thirty-three patients were identified as having nonarteritic CRAO, and of this cohort 6 patien…

A 20-year-old woman was involved in a traffic accident while riding a motorcycle. The vision in her right eye was severely reduced. At the f … ng defect. Four months later, her visual acuity was light perception. Our case shows that a central retinal artery occlusion can be a complication of a blow-out fracture of the lower orbital wall and can lead to severe visual loss even with early surgical repair.

INTRODUCTION: Retinal vein occlusion is usually encountered in the elderly and is often associated with systemic vascular disease. It is rarer in young subjects and requires a serious etiological search to best adapt the treatment.
OBSERVATION: We report the case of a central retinal vein occlusion (CRVO) in a 26-year-old patient who had no pathological history, discovered at a sudden decline of vision in the right eye, reduced to light perception. Fundus examination and retinal angiography showed an edematous CRVO. Radiologic imaging revealed a narrowing of the right carotid canal, probably

Central retinal artery occlusion is one of the most challenging practices and is not an infrequent presentation at the emergency department. We describe a 46-year-old man presenting with abrupt onset of amaurosis fugax secondary to sentinel central retinal artery occlusion followed by acute idiopathic renal infarction. This patient highlights that the preceding visual events could portend a devastating thromboembolic insult in the clinical setting. Early recognition with prompt treatment may preserve organ function, avoid unnecessary management, and prevent debilitating complication.

BACKGROUND: A 75 year old lady presented with acute central retinal artery occlusion and contralateral cotton wool spots.
CASE PRESENTATION: General physical examination and investigations led to a diagnosis of CREST syndrome; however, association of central retinal artery occlusion with CREST syndrome is not well known. While diabetes, systemic hypertension, carotid atherosclerosis and cardiac pathology are common causes of CRAO it is always important to rule out giant cell arteritis.
CONCLUSION: This case highlights that inflammatory causes of central retinal artery occlusion other than gi…

Carotid cavernous fistulas are a rare entity occurring as result of head trauma and also spontaneously. The authors report a rare case of central retinal artery occlusion complicating traumatic carotid-cavernous fistula, resulting in severe visual acuity loss. Spontaneous closure of the fistula was observed. The possible mechanisms responsible for this complication are discussed.

BACKGROUND: Visual prognosis in central retinal artery occlusion (CRAO) is poor and not predictable in individual patients. There may be a correlation between visual acuity and extent of initial macular edema.
METHODS: Central retinal thickness of eleven patients with subtotal CRAO was measured with optical coherence tomography (OCT). Patients' acute-stage macular thicknesses were compared to those of later stages. Values are given as mean (+/-standard error; range).
RESULTS: The mean age of the eleven patients was 68.8 years (62-75). Eight men and three women were examined. The mean initial…

PURPOSE: To determine how the photopic negative response (PhNR) is affected in central retinal artery occlusion (CRAO).
DESIGN: Observational case series.
METHODS: Seven patients with unilateral CRAO were included. Full-field scotopic and photopic electroretinograms (ERGs) including the PhNR were recorded. Each ERG amplitude in the affected eye was expressed as a percentage of amplitude of the corresponding wave in the unaffected eye.
RESULTS: Mean of the PhNR amplitude was reduced to 12.3 +/- 11.7% of that of unaffected eyes whereas the cone b-wave amplitude was attenuated to only 73.4 +/-…

BACKGROUND AND PURPOSE: Local intraarterial fibrinolysis (LIF) is one of several methods used in treating central retinal artery occlusion (CRAO). We investigated whether LIF is more effective than conservative methods in the treatment of CRAO.
METHODS: In this retrospective study, a total of 178 patients (125 men and 53 women) with CRAO were treated at the Eye Hospital of the University of Freiburg from 1980 to 2000. The average age of the patients was 66.8 years (SD, 12 years). In group I, 116 patients were treated conservatively by anterior chamber paracentesis, massage of the globe, isovo…

BACKGROUND/PURPOSE: There is increasing evidence suggesting that nitric oxide (NO) is implicated in the pathogenic process of ischemic disease. The purpose of this study was to quantify the aqueous humor NO levels in patients with central retinal artery occlusion (CRAO) and in age-matched controls.
METHODS: Aqueous humor specimens were obtained from seven patients who had been diagnosed with CRAO; the sampling was done within 12 to 24 hours of diagnosis. All specimens were obtained by paracentesis, which was performed as a therapeutic intervention. Samples of aqueous humor were also collected…
