Influence of pharyngeal flap surgery on maxillary outgrowth in cleft patients.

The aim of this study was to investigate the influence of the pharyngeal flap procedure on the frequency of Le Fort I osteotomies in full-grown nonsyndromic cleft patients. A retrospective review of 508 full-grown cleft patients born between 1 January 1983 and 31 December 1992 was performed. Following data analysis, 140 males older than 18 years and 111 females over the age of 16 years were included. 69 of the 251 included cleft patients required pharyngeal flap surgery (27.5%). Revision flap surgery was performed in 17.4% of the cases. A significantly lower age at time of the initial pharynge

Treatment of class II division 1 malocclusion with myofunctional trainer system in early mixed dentition period.

Dentofacial growth interferences, caused by abnormal lip and tongue function in the mixed dentition period are a common clinical condition. The main purpose of the treatment in such cases is to eliminate oral dysfunction, establish muscular balance and correct or diminish maxillary incisor protrusion. The position of the teeth is determined by the lip and tongue pressure. Lip function may have been the cause of forcing teeth in to malposition they occupy. The case report is presented where the myofunctional trainer T4K is used during growth phase.

Apnoea-hypopnoea and mandibular retrusion as uncommon findings associated with Proteus syndrome.

The aetiology of Proteus syndrome (PS) is yet unclear. This disease includes partial gigantism of the hands and/or feet, nevi, hemihypertrophy due to overgrowth of long bones, subcutaneous tumours, macrocephaly, cranial hyperostosis, and pulmonary and renal abnormalities. This case report is about a 17-year-old boy with two uncommon findings associated with PS: apnoea-hypopnoea and mandibular retrusion. A multidisciplinary team was important to provide professional care for this patient. Dentists and physicians proposed an adjusted treatment plan. Maxillary disjunction was achieved with a comb

Skeletal stability following mandibular advancement and rigid fixation with polylactide biodegradable screws.

Skeletal stability during the first year after mandibular advancement surgery and fixation using biodegradable self-reinforced poly-l-lactide (SR-PLLA) screws, without postoperative intermaxillary fixation, was studied in 25 patients by means of cephalometric measurements. The magnitude of advancement was on average, 3.88 mm at pogonion (PG) (range 1.25-6.5 mm) and 4.57 mm at B-point (range 2.75-7.5 mm). After one year a mean relapse backwards of 0.59 mm at the PG (15%) and 0.78 mm at the B-point (17%) was observed. Nineteen patients (76%) and 17 patients (68%) were stable at PG and B-point, r

Correction of mandibular hypoplasia by means of bidirectional callus distraction.

The method of gradual callus distraction proved to be very useful in augmenting hypoplastic mandibles. When performing our first series using a unidirectional distractor, we faced some problems in determining the direction of lengthening. Moreover, postoperative adjustments were not possible. On the basis of this experience, we developed a bidirectional lengthening device consisting of two limbs and a variable angulation piece. This allows distinct control of the vertical and horizontal components of the distraction. The method proved effective and reliable in 18 patients with various degrees

Autogenous Fat Grafting for Chin Augmentation: A Preliminarily Clinical Study of Cosmetic Outcome.

Preoperative evaluation and surgical details of chin fat augmentation aiming at increasing the projection of the original mentum are scarcely reported. Herein, the authors systematically reviewed the application of fat grafting for chin augmentation in our institution from 2010 to 2014. This percutaneously multilayered fat-injection technology can be applied for chin augmentation for a subset of patients after strict selection, and the details of lipofilling technology and fat distribution vary depending on each individual patient's condition. Mentum can be augmented in the sagittal plane in d

Antero-posterior and vertical facial type variations influence the aesthetic preference of the antero-posterior lip positions.

UNLABELLED: SummaryObjectives: The purposes of this study were to investigate whether the antero-posterior and vertical facial type variations influenced the favoured lip positions and to elucidate whether the favoured lip positions differed between orthodontists and laypersons. METHODS: An average profile was constructed from a Japanese female with Class I occlusion and a well-balanced profile, as assessed by several cephalometric analyses. Nine facial types were composed by morphing the chin antero-posteriorly and/or vertically. Thirteen morphed lip profiles were constructed by moving the l

Auriculo-condylar syndrome.

The auriculo-condylar syndrome is caused by abnormalities of the first and second pharyngeal arches during embryonic development. Its inheritance follows the autosomal dominant pattern. Both familial and individual cases are reported in the literature. The syndrome is characterized by wide phenotypic variation, with affected individuals expressing clinical signs of variable severity due to variable expressivity of the responsible genes. Clinical signs of the syndrome include auricular malformation, hypoplasia of the mandibular condyles, anomalies of the temporomandibular joints, malocclusion,

Maxillofacial characteristics affecting chin deviation between mandibular retrusion and prognathism patients.

OBJECTIVE: To examine and compare the maxillofacial characteristics affecting chin deviation three-dimensionally in facial asymmetry patients with mandibular retrusion and mandibular prognathism. MATERIALS AND METHODS: Maxillofacial computed tomography (CT) scan data were obtained from 48 orthognathic surgery patients (24 cases of mandibular retrusion and 24 cases of mandibular prognathism) with facial asymmetry. Although chin deviation was assessed on the basis of the ANS-to-menton line on frontal cephalograms, its contributing factors were evaluated on three-dimensional CT images. Maxillary

Bilateral complex odontomas: a rare complication of external mandibular distraction in the neonate.

BACKGROUND: The authors present an unusual complication of external mandibular distraction in a child with bilateral complex odontomas in the posterior mandible. CLINICAL REPORT: The patient is a 4-year-old girl with a history of micrognathia, glossoptosis, and respiratory distress (Pierre-Robin sequence) treated with neonatal external mandibular distraction. She was referred to our service with bilateral amorphic radio-opaque masses in the body of the mandible. CONCLUSIONS: The etiology of odontomas is uncertain, but different theories exist, such as local trauma, infection, family history,

Management of condylar fractures.

Management of condylar fractures remains a source of ongoing controversy. While it appears that many condylar fractures can be managed nonsurgically, recognition of cases that require surgical intervention and selection of an appropriate procedure are paramount to success in treating these injuries. There are a variety of special considerations that are peculiar to the condylar region. This article discusses anatomic considerations, classification of condylar fractures, indications for surgery, treatment options, and complications. The goals of treatment include restoration of function and est
