Transcriptional upregulation of SCG10 and CAP-23 is correlated with regeneration of the axons of peripheral and central neurons in vivo.

We have compared SCG10 and CAP-23 expression with that of GAP-43 during axonal regeneration in the peripheral and central nervous systems (PNS, CNS) of adult rats. SCG10, CAP-23, and GAP-43 mRNAs were strongly upregulated by motor and dorsal root ganglion (DRG) neurons following sciatic nerve crush, but not after dorsal rhizotomy. When the sciatic nerve was cut and ligated to prevent reinnervation of targets, expression of all three mRNAs was prolonged. Neurons in the thalamic reticular nucleus and deep cerebellar nuclei transiently upregulated these mRNAs after axotomy, and showed prolonged u…

The management of cancer pain.

Any therapeutic strategy developed for patients experiencing cancer pain depends on the goals of care, which can be broadly categorized as p … ts, are also listed and discussed in some detail. Surgical and neurodestructive techniques, such as rhizotomy or cordotomy, although not frequently required or performed, represent yet other options for patients with unremitting pain and diminished hope of relief. Although cancer pain can be a complex medical problem arising from multiple sources, patients …

Central demyelination of the Vth nerve root in trigeminal neuralgia associated with vascular compression.

We have examined the ultrastructure of the trigeminal sensory nerve root in three patients with medically intractable trigeminal neuralgia. In one patient, the nerve root was sandwiched between a large vein and a small pontine artery, in the others compression was due to marked dolichoectasia of a verterbal artery. Because these were not amenable to microvascular decompression, a caudal rhizotomy was performed, by excising a short inferior segment of nerve root in the region of indentation. In all cases, examination revealed a zone of chronic demyelination in the proximal (centrally myelinated…

Normal diameter of filum terminale in children: in vivo measurement.

The hallmarks of tethered cord syndrome are a low-lying conus medullaris and a thick filum terminale. In diagnosing the tethered cord syndrome, the thick filum terminale is often defined as that greater than 2 mm in diameter. The cutoff of 2 mm in diameter was derived from myelographic measurements a few decades ago, and the true normal diameter of the filum terminale diameter in children or adults remains unknown. We measured the diameters of the filum terminale in vivo in the operating room on 31 children (age range = 2-14 years; mean age = 5 years) undergoing selective dorsal rhizotomy for …

Microvascular decompression for trigeminal neuralgia. A five-year follow-up study.

A series of 72 patients with typical trigeminal neuralgia who underwent microsurgical exploration of the trigeminal nerve in the posterior fossa is analyzed. The operations were performed between 1977 and 1980 with an average follow-up period of 4.94 years. Seventy-eight percent of the patients have remained free of pain after the operation. Of the 16 patients (22%) that were either not relieved of their pain or had a recurrence, two are well-controlled with medical treatment and the rest have required a variety of surgical procedures (mostly radiofrequency rhizotomy) for pain relief. Recurren…

Motor tics of the head and neck: surgical approaches and their complications.

Motor tics of the head and neck, especially hemifacial spasm and spastic torticollis, are the substance of this paper. Forty-six cases are p … o year's time. In spastic torticollis the accepted radical operation consists of bilateral anterior rhizotomy of the upper three roots plus bilateral spinal accessory nerve section in the neck. A tragic complication of this operation has recently been observed by ourselves, Sweet, and Hamlin. This complication is bilateral infarction of the medulla (bilater …

Radiofrequency thermoablation of the infraorbital nerve for posttraumatic trigeminal neuropathic pain: illustrative case.

BACKGROUND: Trigeminal neuropathic pain (TNP), particularly posttraumatic TNP (PTTN), is often refractory to medical therapy and difficult to manage surgically. Although radiofrequency thermoablation (RFA) of the gasserian ganglion is established, peripheral nerve RFA remains underutilized, particularly in anatomically complex cases. OBSERVATIONS: A 62-year-old man developed severe left maxillary (V2) facial pain following sinus surgery, refractory to multiple pharmacological therapies and prior gasserian balloon rhizotomy. A diagnostic infraorbital nerve block produced significant temporary …

Internal neurolysis: 'nerve combing' for trigeminal neuralgia without neurovascular conflict - early UK outcomes.

INTRODUCTION: Internal neurolysis (INL) is a surgical procedure where trigeminal nerve fibres are separated between the pons and porus trigeminus to relieve trigeminal neuralgia (TN). We report pain and functional outcomes to evaluate its safety and efficacy. MATERIALS AND METHODS: Prospective cohort of all patients undergoing retrosigmoid craniotomy and INL between 2015 and 2017 at University Hospital Southampton. Patients with type I (6) or type II (2) refractory TN and no clear neurovascular conflict were offered INL as an alternative to partial sensory rhizotomy. Barrow Pain Intensity Sca…

A multiarray electrode mapping method for percutaneous thermocoagulation as treatment of trigeminal neuralgia. Technical note on a series of 178 consecutive procedures.

BACKGROUND: We describe our method and mapping technique of the trigeminal nerve using a quadripolar electrode to minimize morbidity of percutaneous thermocoagulation as treatment of trigeminal neuralgia. METHOD: Of 381 patients selected for postgasserian thermocoagulation, 178 consecutive procedures were carried out using, in most cases, our painless and ambulatory method and technique. All patients were preoperatively subjected to 3-dimensional constructive interference in steady-state magnetic resonance and magnetic resonance angiography. Trigeminal neuralgia and glossopharyngeal neuralgia: two orofacial pain syndromes encountered by dentists.

BACKGROUND: Dentists frequently evaluate patients for oropharyngeal pain that may or may not eventually be related to oral pathology. Two rare neurological disorders that present with severe orofacial pain are trigeminal neuralgia, or TN, and glossopharyngeal neuralgia, or GPN. Both are secondary to cranial nerve compression by arteries and veins at the point at which the nerves exit the pons and brainstem. RESULTS: The authors present the results for two series of patients treated for TN and GPN. Significant success can be seen after intracranial microvascular decompression for both disorder

Subarachnoid-to-Subarachnoid Shunt for Correction of Nonfunctioning Baclofen Pump in a Severe Case of Chronic Debilitating Post-Spinal Cord Injury Spasticity.

BACKGROUND: Perhaps the most disabling condition seen in patients with spinal cord injury (SCI) is spasticity. Spasticity is characterized a … ess ITB catheter failure before considering other measures, such as neuroablative procedures (e.g., rhizotomy, myelotomy). This case reinforces the recommendation that ablative procedures, which have far greater complications, should be reserved for patients who have failed medical or other nonablative therapies.
