Managing menopause after breast cancer: balancing risks and benefits.

Many women now survive breast cancer, but find themselves at increased risk of menopausal complications. How to manage menopause after breast cancer is a complex issue, given that estrogen has a role in the development of breast cancer and valid concerns exist about estrogen replacement therapy in patients who have had breast cancer. This article explores the relationship between estrogens and breast cancer and discusses management options for a variety of menopausal complications in breast cancer survivors.

[Proposal of a computerized system for chemical risk management at biomedical laboratories].

The current italian and european directives require as an essential part of preventive procedures the risk assessment in every work setting. This procedure may be however difficult to be applied because of variability and specificity of work environment, work organisation, chemicals ect. It is true for the biomedical laboratories when the attention is focused on the identification and exposure evaluation of chemicals related to the analytical process, i.e. the first step to reach an accurated risk assessment. The starting point often has been the list of theoretically used compounds, which cou

The woman at increased risk for breast cancer: evaluation and management strategies.

Increased understanding of risk factors for breast cancer, especially the identification of genes associated with a predisposition to breast cancer, has focused attention on the issue of breast cancer prevention. At present, the only clinically available method of breast cancer prevention is prophylactic mastectomy. However, there are no absolute indications for its use, and it is not often appropriate in relation to actual risk for disease. This article reviews what is known about major breast cancer risk factors, reviews the available data on prophylactic mastectomy, and discusses current st

A risk-benefit appraisal of drugs used in the management of premenstrual syndrome.

Recent advances in the understanding of the pathogenesis of premenstrual syndrome (PMS) have allowed the development of appropriate pharmacological interventions. Although at the present time there are no approved medications for this indication in the US, several well-designed studies have been conducted that guide the clinician's treatment of PMS. As a result, less-proven nonpharmacological modalities, such as dietary modification, exercise regimens and psychotherapy, are more quickly supplanted by the use of medication. Three classes of agents have been proven efficacious and are widely use

Atresia of the esophagus. New trends in the management of high-risk neonates.

Once the reconstruction of esophageal atresia in infancy was reported, immediate repair became standard practice. High-risk infants carry an operative mortality of 30% to 80%. Staged surgical procedures were introduced to improve survival. "Delayed" reconstruction of esophageal atresia in selected cases has been reported to improve survival and eliminate staged surgical procedures. Between 1982 and 1986, 21 newborns were diagnosed as having esophageal atresia. Eight infants (32%) underwent "immediate" repair. In 13 infants repair was "delayed" for seven to 252 days. Four neonates with "pure" e

Melanocytic precursors of cutaneous melanoma. Estimated risks and guidelines for management.

There are several recognizable melanocytic precursors of cutaneous melanoma. These precursors include lentigo maligna, dysplastic melanocytic nevi, congenital nevi (of any size), and darkly pigmented lesions of acral surfaces and mucous membranes. Lentigo maligna is an uncommon melanocytic dysplasia, present in 3 per 1000 individuals over the age of 50 years and accounting for 4 percent of all cutaneous melanomas. Dysplastic melanocytic nevi are present in 2 per cent of white adults, and may account for at least a fifth of cases of cutaneous melanoma. Congenital nevomelanocytic nevi are presen

Managing a low-incidence risk: the example of toxic shock syndrome.

This article demonstrates a methodology that allows individuals to reach a personal decision on the use of products which carry very small risks to health and life but also offer considerable benefits. A combination of the principles of dominance, extended dominance, and various methods of direct risk-benefit tradeoffs are shown to reduce the number of possible decisions regarding product use to the one optimal for the value structure of a particular individual. An historical examination of toxic-shock syndrome identifies tampons as a product with risks too small to warrant public intervention

Seizure management in acute hepatic porphyria: risks of valproate and clonazepam.

Seizures may occur in acute intermittent porphyria or other hepatic porphyrias. Management is difficult, because barbiturates and hydantoins exacerbate the porphyric state. We studied one patient with major motor seizures and acute intermittent porphyria. The seizure disorder was exacerbated by phenytoin and did not respond to a high-carbohydrate diet or to intravenous hematin. Clonazepam was ineffective in treating the seizures and, in high doses, seemed to exacerbate the porphyria. Both clonazepam and valproate were porphyrinogenic in experimental test systems. Because both drugs may exacerb

Sleep Apnea and Cardiovascular Disease: Risks, Mechanisms, Complications, and Management.

Sleep apnea is a common sleep disorder characterized by recurrent episodes of breathing cessation and resumption during sleep. The three main types are obstructive sleep apnea (OSA), central sleep apnea (CSA), and mixed sleep apnea (MSA), the latter of which is often used interchangeably with complex sleep apnea (CompSA). OSA, the most prevalent form, results from upper airway obstruction during sleep. Globally, nearly one billion adults aged 30-69 are affected by OSA, with approximately 425 million having moderate to severe disease, making sleep apnea a major global health burden affecting ro

Infection Risk and Vaccination in the Management of Psoriasis: Considerations for Biologic Therapy.

This narrative review examines critical considerations for biologic treatment in psoriasis patients, with a focus on infection risks, providing current recommendations and practical considerations for prevention, including vaccination, screening, and management strategies. Since type I (Th1) inflammation and type III (Th17) inflammation protect against intracellular and extracellular infections, respectively, it is logical that biologic treatments blocking these pathways may be associated with an increased risk of infection. It has been proven that TNF inhibitors are associated with an increas

Fluoroquinolones for Dermatologists: A Practical Guide to Clinical Use and Risk Management.

Background: Fluoroquinolones, available in topical and oral formulations, are used to manage bacterial skin and soft tissue infections, including Pseudomonas aeruginosa, atypical mycobacteria, and select multidrug-resistant Gram-negative organisms. Their excellent tissue penetration, bactericidal activity, and convenient dosing make them effective for certain skin and soft tissue infections. However, their use is limited by potential safety concerns, including tendinopathy (odds ratio up to 9.1 in corticosteroid users), QT interval prolongation with risk of torsades de pointes, phototoxicity,

Optimizing Perioperative Anticoagulant Management: Balancing Thromboembolic and Bleeding Risks.

Interruption of anticoagulation for a surgical procedure is individualized and based on balancing the risk of perioperative thromboembolism versus the risk of bleeding. Risk assessment for thromboembolism is based on the indication for anticoagulation and a variety of patient-specific factors. Bleeding risk assessment is also largely based on patient factors, such as age and comorbidities, but also the type of procedure being done. The post-operative bleeding risk of surgical procedures can be categorized as high-risk, low-to-moderate-risk, and minimal risk. Each risk category has unique recom

Uncertainties, risks, or hope? Managing embryos in a multidisciplinary team of PGT.

Preimplantation genetic testing (PGT) is a genetic testing method used in assisted reproduction to select embryos without apparent genetic anomalies for transfer into the uterus. However, PGT often involves uncertainties, such as the identification of genetic variants of uncertain significance (VUS) in embryos, which complicates clinical management and embryo selection. Based on an ethnographic study conducted from 2021 to 2024 at a fertility center in Shanghai, China, including observations and interviews with 11 PGT professionals and five couples, we examined how a multidisciplinary PGT team

Multidisciplinary consensus on the management of patients with osteopenia and fracture risk in Spain.

BACKGROUND: In Spain, over 3.5 million individuals are diagnosed with osteoporosis, while an even greater number are affected by osteopenia, the precursor stage of this severe condition. Unfortunately, osteoporosis and osteopenia frequently remain undetected until a fracture occurs, potentially resulting in permanent damage or even death. METHODS: To improve the management of osteopenia, a multidisciplinary task force was established, consisting of 45 experts, including patients, doctors, nurses, pharmacists, and managers from 20 scientific organisations related to this pathology. With the sc

Understanding and Managing Fracture Risk in Patients With Cancer: A Literature Review.

One of the most important complications of cancer and its treatments is the increased fracture risk. Fractures can have a significant impact on the patient's quality of life and may be associated with morbidity, mortality, and reduced functional status. The present study aims to investigate the mechanisms underlying the increased risk of fractures in cancer patients, the effect of cancer treatments on fracture risk, and strategies to prevent fractures in this population. This is a literature review using the PubMed - National Center for Biotechnology Information (NCBI), Web of Science, Cochran
