[Rotator cuff rupture].

The rotator cuff is the complex of four muscles that arise from the scapula; supscapularis, supraspinatus, infraspinatus and teres minor. The long head of the biceps tendon may be considered a functional part of the rotator cuff. The rotator cuff is key structure of the shoulder which gives dynamic stability and movements. Rotator cuff tears my occur as consequence of impingement syndrome or degenerative changes. Most tears occur in the supraspinatus tendon. The patients are usually older than 40 years. On clinical examination there is weakness of abducation and external rotation of the arm. O

Arthroscopic biceps tenodesis.

Surgical treatment of symptomatic pathology of the long head of the biceps tendon generally consists of either biceps tenotomy or tenodesis. Biceps tenodesis is generally recommended for younger patients and has been well described using open techniques. With advancements in arthroscopic ability and equipment, new arthroscopic techniques have recently been reported. These techniques can be especially useful when used in conjunction with other arthroscopic procedures such as distal clavicle resection, rotator cuff repair, and subacromial decompression. We present a modification of the technique

The terrible triad of the shoulder.

This case shows that the terrible triad of the shoulder-anterior dislocation, brachial plexus injury, and rotator cuff tear-can occur in younger populations than previously described. A high index of suspicion for this triad of injury should be maintained, particularly in those young individuals with grossly unstable shoulders. Once axillary nerve function has begun to return, simultaneous repair of all soft tissue injuries can lead to a stable, functional joint.

Full-thickness tears: arthroscopic repair.

Arthroscopic repair of rotator cuff tears is an option for a surgeon with advanced arthroscopic skills and a thorough knowledge of open repair technique. Surgical indications and a detailed description of operative technique are presented. Arthroscopic rotator cuff repair offers theoretical advantages over open repair, but long-term studies are needed to demonstrate its effectiveness.

Rotator cuff tearing and inferior subluxation of the humeral head: report of two cases.

We encountered two patients with atraumatic rotator cuff tears who exhibited inferior subluxation of the humeral head. In both patients shoulder motion was limited by pain. Repair of the rotator cuff tear relieved the pain and restored glenohumeral joint motion. Although the cause of this unusual complication of rotator cuff damage remains unclear, the inferior subluxation was associated with reflex muscle hypotonia, which would have reduced the pressure in the subacromial space and thus would have decreased the pain caused by the cuff lesion. No similar case seems to have been documented in t

Increased Risk of Rotator Cuff Tear in U.S. Army Soldiers With a History of Prior Shoulder Impingement Syndrome.

INTRODUCTION: Military populations and comparable groups of highly physically active civilians may experience increased shoulder loading due to occupational demands, fitness standards, and physical activity. These heightened upper extremity demands may increase the risk of developing shoulder impingement syndrome. Shoulder impingement is hypothesized to increase the risk of rotator cuff tears, an injury with long-term performance implications. METHODS: This is a nested, matched, case-control study using data from active-duty U.S. Army Soldiers from 2016 to 2021 (N=869,651). Incident rotator c

Office Diagnosis of Shoulder Disorders.

In brief Complex tests and procedures brief are not usually needed to identify shoulder injuries that are common in clinical practice: rotator cuff injuries, shoulder instability, and impingement syndrome, among others. The history can differentiate causes of pain or instability with questions about symptom patterns. Performing targeted physical tests such as the drop-arm test and the apprehension test establishes presence and degree of impingement or instability. A standard radiographic series that includes rotation, axillary, and supraspinatus outlet views can identify presence of associated…

Rotator cuff tear following long-standing axillary neuropathy in a female motocross racer.

A 'terrible triad' of anterior shoulder dislocation, axillary nerve damage and rotator cuff tear has been previously described. However, we are unaware of any report of anterior shoulder dislocation, humeral fracture, axillary neuropathy and subsequent rotator cuff tear requiring surgery when the axillary neuropathy was deemed permanent. We report the case of a 20-year-old woman who fell in a motocross accident and had an anterior shoulder dislocation, humeral fracture and axillary neuropathy. The fracture was treated surgically with open reduction and internal fixation. The axillary nerve inj

BACKGROUND: The Major League Baseball (MLB) Health and Injury Tracking System (HITS), created in 2010 with the MLB Players Association, was designed to overcome limitations of injured list-based reporting. It serves as a longitudinal injury surveillance platform and electronic medical record across major and minor league players. PURPOSE: To evaluate all publications related to the HITS database to provide an overview of epidemiology, treatment outcomes, and return-to-play (RTP) rates in professional baseball, guiding player safety and performance strategies. The presence of smooth muscle actin in fibroblasts in the torn human rotator cuff.

The rotator cuff frequently sustains athletic and occupational injury, often resulting in chronic pain and disability. However, despite the high incidence of such shoulder problems, the pathophysiology of rotator cuff injury and healing has not yet been fully elucidated. The notable finding of this study was the presence of a contractile actin isoform, alpha-smooth muscle actin (SMA), in nonvascular cells in all of the seven torn human rotator cuff specimens evaluated immunohistochemically. Up to 95% of cells in any one region, and over 95% of elongated cells found in association with crimped

[Sonographic assessment of typical lesions in shoulder dislocation].

The traumatic dislocation of the shoulder joint is the most often joint luxation. The involved patients are often sporting and between the age of 20 to 40 years. The therapeutic and prognostic point of interest is to recognize and reconstruct injured structures. Among the soft tissue injuries the Bankart lesions, among the osseous injuries the Hill-Sachs defect are numeric well to the fore. Hitherto existing diagnostic methods for these injuries are either invasive or with X-rays. Its is demonstrated, that dynamic ultrasonography examination of the shoulder joint can visualize besides the rota

Diagnostic shortcomings of magnetic resonance arthrography to evaluate partial rotator cuff tears in adolescents.

BACKGROUND: Recent evidence suggests an increase in the incidence of partial articular-sided rotator cuff tears in adolescent athletes, but the accuracy of diagnostic studies has not been previously assessed in this cohort. This study was performed to assess the ability of magnetic resonance imaging with arthrography (MRIa) to diagnose partial rotator cuff (PRC) pathology in the adolescent age group. METHODS: All patients under the age of 19 years who underwent shoulder arthroscopy, between August 2008 and August 2010, were grouped based on the presence of a PRC tear diagnosed by either MRIa

Partial-thickness articular surface rotator cuff tears in patients over the age of 35: Etiology and intra-articular associations.

PURPOSE: Partial-thickness articular-sided rotator cuff tears have a multifactorial etiology and are associated with degeneration of the tendon. They are often described as an injury of the young athlete, although they are also found in the older population. The aim of this study was to investigate the frequency and associations of partial-thickness articular-sided tears in patients over the age of 35 years. DESIGN: Retrospective MATERIALS AND METHODS: A retrospective study of all arthroscopic procedures for rotator cuff pathology in patients over the age of 35 years over a 2-year period by

[Clinical-ultrasonographic assessment in arthroscopic rotator cuff repair after 1-year follow-up].

INTRODUCTION: Arthroscopic repair of the rotator cuff have shown have shown encouraging clinical results. However, few authors have assessed integrity of repair with ultrasound. The presence of re-rupture by ultrasonography in a rotator cuff repair may not relate to the patient's functional status. OBJECTIVE: We used ultrasonography to assess the prevalence of re-rupture in rotator cuff repairs and its clinical relevance with minimum 1 year postoperatively. MATERIALS AND METHODS: Evidence level IV (Case series). We evaluated 27 shoulders that underwent arthroscopic rotator cuff repair. Clini

Management of the throwing shoulder: cuff, labrum and internal impingement.

Repetitive throwing or other overhead activity places great stress on the shoulder. As a result, the shoulder is a common site of injury in athletes. Addressing throwing-related injuries requires an understanding of throwing biomechanics and pathology. Nonoperative treatment is directed at restoring strength, flexibility, and neuromuscular control to the entire kinetic chain. Surgery is indicated when nonoperative treatment fails, and is directed at correcting labral, capsular, and rotator cuff pathology.

Effect of anterior supraspinatus tendon partial-thickness tears on infraspinatus tendon strain through a range of joint rotation angles.

BACKGROUND: Rotator cuff tears are common shoulder problems whose propagation is difficult to predict because of the structural and mechanical inhomogeneity of the supraspinatus tendon. We have previously shown that the supraspinatus and infraspinatus tendons interact mechanically when the supraspinatus tendon is intact or exhibits a full-thickness tear, so that an increase in supraspinatus tendon strain is paralleled by an increase in infraspinatus tendon strain. Such interaction is critical and suggests that an increase in infraspinatus tendon strain that accompanies an increase in supraspin

A Case Report Emphasizing the Advantageous Effect of Mulligan Mobilization Technique in the Management of Rotator Cuff Syndrome to Enhance Functional Recovery in a 65-Year-Old Farmer.

Adults with shoulder pain often have partial rotator cuff injuries (RCIs) as the underlying cause. RC partial tears are one of the common conditions that can have a major influence on a wide range of people, including sportspeople, workers, and sedentary adults with rotator cuff syndrome (RCS). Any injury, disease, or deteriorating condition that affects the shoulder's RC muscles and tendons is recognized as RCS. Subacromial bursitis, RC tendonitis, subacromial impingement syndrome (SIS), rotator cuff tears (RCTs), etc., are a few disorders linked to RCS. For partial RCT, nonoperative treatmen…
