PURPOSE: Several options exist for treating scaphoid nonunion. For selected cases, some authors recommend using a volar buttress plate. The aim of the study was to report the clinical and radiological outcomes achieved at a minimum of 3 years' follow-up of treating scaphoid nonunion with a scaphoid volar plate.
METHODS: We retrospectively reviewed 15 patients with symptomatic scaphoid nonunion treated with scaphoid plate osteosynthesis and a bone graft from the volar aspect of the distal radius between January 2011 and November 2013. The patients' average age was 32 years (range, 21-62 years)

We describe a personal technique for the treatment of scaphoid non-union, consisting of styloidectomy of the radius associated with axial perforation of the scaphoid, emptying of the bone, filling it again by cancellous bone taken from the styloid and securing it with a peg obtained from the cortex of the same styloid. 52 patients out of 77 operated on with this technique were reviewed. Results are shown and seem to be better than those found in literature.

Rotatory instability of scaphoid fractures was studied by a method not used previously. On two fresh specimens the scaphoid bone was fractured by osteotomy, and the fracture was fixed by a Hoffmann apparatus extending from the distal radius to the 1st, 2nd and 3rd metacarpals. This permits free rotation in the forearm. The scaphoid fragments were marked by Kirschner wires. No movement occurred between the fragments when supination/pronation was kept within the normal range of movement.

We report up-to-date one-year prospective data on the incidence of scaphoid fracture in skeletally immature patients managed in a Fracture Clinic of a Level I Trauma Centre in a University Hospital in the UK. All scaphoid fractures were immobilised until union was achieved. One hundred twenty one skeletally immature patients were referred for a suspected fracture of the scaphoid. Fourteen patients (11.5%) did have a scaphoid fracture, an incidence of 15 per 100,000 (0.55% of all paediatric injuries referred). Mean delay in being seen was 2.6 days, and mean follow-up time was 52.3 days. Only pl

Manipulative examination of the carpal bones is an important facet of the examination of the wrist. Abnormal translation of portions of the carpus in response to applied force is a commonly used clinical indicator of ligament injury. Unilateral scaphoid hypermobility during the so-called scaphoid shift test is felt to represent traumatic instability, especially in the context of wrist injury. The test, however, is subjective, and requires considerable experience to correlate the degree of scaphoid mobility with pathologic significance. We used an instrument that quantifies the load-displacemen

The compressive forces generated by various scaphoid screws were measured in the laboratory using a custom designed load washer (transducer) as a means to compare their fixation capabilities. Testing was done using simulated bones fabricated from polystyrene pipette tubing and Pedilen polyurethane foam and paired cadaver scaphoids. The Herbert scaphoid screw was found to have inferior fixation capabilities compared with screws of a more conventional design. However, its size compatibility was the best of all the screws tested. The Heune scaphoid screw demonstrated the best fixation capability,

AIM: To compare motion of the capitate, scaphoid, and lunate in wrists with a malunited distal radius and contralateral normal wrists during dart-throwing motion (DTM) by three-dimensional kinematic studies using computed tomography (CT) images.
MATERIALS AND METHODS: CT was performed simultaneously on both wrists in six patients with a unilateral distal radius malunion at three stepwise positions simulating DTM. Using volume registration technique, the kinematic variables of helical axis motion of the capitate, scaphoid, and lunate were calculated and compared between both wrists. The helica

Enchondroma represents a common bone tumor of the hand. The scaphoid is a rare location. We report the case of a scaphoid enchondroma presenting as chronic wrist pain following a relatively minor trauma. The diagnosis was suggested by radiological study and then confirmed by histological study after biopsy. Enucleation and vascularized bone graft with osteosynthesis provided good results. The clinical, radiological and therapeutic aspects of this rare condition are discussed.

We hypothesised that the use of pulsed electromagnetic field (PEMF) bone growth stimulation in acute scaphoid fractures would significantly shorten the time to union and reduce the number of nonunions in a randomised, double-blind, placebo-controlled multicentre trial. A total of 102 patients (78 male, 24 female; mean age 35 years (18 to 77)) from five different medical centres with a unilateral undisplaced acute scaphoid fracture were randomly allocated to PEMF (n = 51) or placebo (n = 51) and assessed with regard to functional and radiological outcomes (multiplanar reconstructed CT scans) at

Management of scaphoid nonunion after failed surgery for acute scaphoid fracture presents a unique treatment challenge. Prior surgery complicates patient evaluation and increases the technical difficulty of future procedures. Healing of nonunion is crucial to prevent carpal collapse and progressive arthritis. A thorough workup is required to identify technical factors or treatment decisions that may have resulted in a poor outcome after initial fixation attempts. CT is particularly useful for characterizing nonunion and planning revision surgery. Several studies have described the use of bone

OBJECTIVES: To determine whether the proximal pole (PP) of the scaphoid is denser than the distal pole (DP) in a diverse population of patients undergoing computed tomography (CT) imaging of the wrist.
METHODS: Sixty-nine patients who had CT of the wrist were retrospective reviewed. We measured CT density of the medullary bone of PP and DP of the scaphoid and calculated PP/DP ratio. To evaluate the variability of PP/DP ratio, we compared the ratios of these 3 groups. These patients were separated into 3 groups based on their diagnoses: group 1, healed scaphoids treated by casting; group 2, sc

Scaphoid fracture is a common injury encountered in family medicine. To avoid missing this diagnosis, a high index of suspicion and a thorough history and physical examination are necessary, because early imaging often is unrevealing. Anatomic snuffbox tenderness is a highly sensitive test for scaphoid fracture, whereas scaphoid compression pain and tenderness of the scaphoid tubercle tend to be more specific. Initial radiographs in patients suspected of having a scaphoid fracture should include anteroposterior, lateral, oblique, and scaphoid wrist views. Magnetic resonance imaging or bone sci

The scaphoid is the most commonly fractured of the eight carpal wrist bones. The mechanism of injury is usually from a fall on the outstretched hand. Initial radiographs may not identify a scaphoid fracture. A high index of suspicion given to these biomechanics and the physical findings will assist in the identification of this fracture. Certain fractures of the scaphoid are at high risk of avascular necrosis and nonunion requiring splinting and orthopedic referral of suspected fractures to assist in preventing such complications.

We investigated 52 consecutive patients (53 wrists) with suspected scaphoid fracture using MRI (0.2 T) within 7 days after the injury. We performed two sequences for the wrist: (1) coronal T1-weighted spin-echo and (2) T2-weighted turbo spin-echo. We also performed four sequences specific for the scaphoid with the plane parallel to the longitudinal axis of the scaphoid: (1) coronal T1-weighted, (2) coronal T2-weighted, (3) sagittal T1-weighted, and (4) sagittal T2-weighted images. In these projections, only one section is enough to cover the scaphoid fracture that definitely eased the diagnosi

The relative contribution of the scaphoid, lunate, triquetrum, and capitate to wrist motion was examined in 6 fresh cadaver forearms. A wrist-joint motion simulator was used to dynamically move each wrist through planar and nonplanar motions. During wrist flexion-extension, the motion of the capitate closely followed the motion of the third metacarpal, while the lunate motion was approximately 50% of the total motion; the triquetrum, 65%, and the scaphoid, 90%. Similar differences in motion for these carpal bones occurred during radioulnar deviation and circumduction and dart-throw motions. Th

An eleven-year-old male gymnast felt right wrist pain when performing handstands on the floor without any major traumas. Displaced scaphoid fracture was observed on the anteroposterior view of the radiograph. Both on the T1-weighted image from magnetic resonance imaging (MRI) and on the short TI inversion recovery (STIR) image from MRI, low intensity was detected at the middle of the ossification center of the scaphoid and the physis and the articular cartilage covering the scaphoid bone were preserved without any injury. The patient was diagnosed with stress fracture of the ossification cente…

Models of scaphoid non-union with static dorsi-flexed intercalated segment instability were produced in five frozen arms from cadavers or subjects following accidents by repetitive mechanical loading of the wrist joints longitudinally after a bone defect has been made at the mid-portion of the scaphoid. We designed four models of reduction: anatomical reduction; reduction with a shortened scaphoid; anatomical reduction but with the radio-lunate ligament sectioned, and a shortened scaphoid with the radio-lunate ligament sectioned. Results suggested that anatomical reduction with rigid fixation

Background Treatment of scaphoid nonunion is challenging, leading clinicians to pursue innovation in surgical technique and adjunctive therapies to improve union rates. Purpose The purpose of this study was to investigate the use of low-intensity pulsed ultrasound as an adjunctive treatment modality following surgical treatment of scaphoid nonunion in adolescent patients, for whom this therapy has not yet been FDA-approved. Patients and Methods We performed a retrospective review of adolescent patients with scaphoid nonunion treated surgically followed by adjunctive low-intensity pulsed ultras

Even today, the treatment of scaphoid nonunions remains challenging for the treating hand surgeon. Considering the biomechanical principles of wrist kinematics, especially the three-dimensional movement of the scaphoid, it is evident that a single headless compression screw does not provide sufficient stability to counteract all these forces, particularly rotational forces. Palmar plate fixation was initially introduced as a salvage procedure after failed screw fixation. It ensures very high rotational stability but also supports correction of the humpback deformity, reconstruction of scaphoid
