BACKGROUND: Shigellosis or bacillary dysentery, an acute bloody diarrhoea, is a major public health burden in developing countries. In the absence of prompt and appropriate treatment, the infection is often fatal, particularly in young malnourished children. Here, we describe a new diagnostic test for rapid detection, in stool, at the bedside of patients, of Shigella flexneri 2a, the most predominant agent of the endemic form of the disease.
METHODOLOGY/PRINCIPAL FINDINGS: The test is based on the detection of S.flexneri 2a lipopolysaccharide (LPS) using serotype 2a-specific monoclonal antibo…

Shigella, the causative agent of shigellosis or "bacillary dysentery," has been increasingly involved in foodborne outbreaks. According to the Centers for Disease Control and Prevention's Emerging Infections Program, Foodborne Diseases Active Surveillance Network (FoodNet), Shigella was the third most reported foodborne bacterial pathogen in 2002. Foods are most commonly contaminated with Shigella by an infected food handler who practices poor personal hygiene. Shigella is acid resistant, salt tolerant, and can survive at infective levels in many types of foods such as fruits and vegetables, l…

Eleven Shigella flexneri (group B) isolates were recovered from epidemiologically linked patrons and food handlers from a restaurant-associated outbreak of shigellosis. Six isolates available for pulsed-field gel electrophoresis were identical. All strains agglutinated in group B and subgroup factor 6 sera but not in group 1 through group 6 sera.

INTRODUCTION: Shigella spp is one of the most frequently isolated bacteria causing acute diarrhea with us. Genetics of pathogenicity of Shigella spp. includes chromosomal and plasmid genes. Most virulence factors are coded by invasion plasmid antigen genes residing on a 180-230 MDa plasmid. There is a big problem with multiple resistance of Shigella spp. strains, which is mostly plasmid-borne. Genetic analysis of bacterial cells, that is plasmid profile analysis, is important for investigation of sources and ways of spreading of the infection. All isolates originating from the same clone have …

Bacillary dysentery which occurs endemoepidemically in Burundi is currently on the rise and requires close epidemiologic and therapeutic monitoring. Sensitivity tests using 13 antibiotics were performed on 299 strains of shigella isolated at the Biology Laboratory of the Bujumbura University Hospital Center. The technique used to test antibiotic sensitivity was the disc diffusion method described by Chabert et al. Of the 299 strains studied, 250 (83.6%) were species of Shigella dysenteriae type 1. All isolated Shigella bacillus were resistant to ampicillin, sulfamethoxazole-trimethoprim, tetra…

Several variants of reagents were prepared by coupling type (I, II III, IV, V and VI) and group (3, 4, 6 and 7, 8, 9) anti-Shigella flexneri sera with protein A--containing staphylococcal suspension. For most serum lots coagglutination led to a 1/10 minimum dilution. However, different efficiencies were reported between serum lots and even within the same type or group. No improvement by the coagglutination reaction could be obtained for one lot from group 3, 4 and 7, 8, 9 sera and for both type IV serum lots. The coagglutination reactions were specific both with collection strains and with re…

Shigellemia is rare in developed countries and might result from the emergence of unusually virulent strains. We compared systemic invasiveness markers of isolates from the blood of 3 temporally clustered patients with Shigella sonnei bacteremia in Boston with those of 11 unrelated contemporaneous strains from stools of people in New England. We found no difference between the two groups in O-chain length by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, mouse 50% lethal dose, in vivo response to iron, and susceptibility to serum, which varied from moderately susceptible to ultrasu…

Shigella flexneri, a member of the family of enterobacteriaceae, causes bacillary dysentery by invading the human colonic mucosa and provoking a very intense inflammation. Recent in vitro data allow us to integrate different phenomena into a model of the infectious process during shigellosis. In vivo, bacteria appear to enter the submucosa via the M cells, specialized cells that cover the follicular structures of the intestinal mucosa. Once inside the submucosa, shigellae encounter resident tissue macrophages, which are infected, and apoptosis is rapidly induced. During programmed cell death t…

Shigellae are the most frequently isolated invasive bacteria in Djibouti. 140 clinical strains collected during an eight months period have been studied for their in vitro susceptibility to antibiotics mainly used in treatment and prophylaxis by measure of MIC (agar dilution) for all antibiotics except for trimethoprim-sulfamethoxazole, trimethoprim and sulfamethoxazole (disk diffusion on lysed horse blood medium). Characterization of beta-lactamase is carried out for all strains resistant to amoxicillin and for all S. sonnei. The overall prevalence of resistance to amoxicillin is 41.7% (penic…

Reactive arthritis (ReA) occurs after a urogenital infection usually with Chlamydia trachomatis or an enteritis due to Yersinia, Salmonella, Campylobacter or Shigella, Shigella, except during epidemics, is not considered to be a frequent cause of enteric reactive arthritis. However this might be due to the lack of a reliable antibody test, which makes diagnosis difficult. We compared synovial and peripheral blood lymphocyte proliferation to various bacterial antigens in 19 consecutive patients with ReA or undifferentiated oligoarthritis. In five patients Shigella was identified as the causativ…

In 1990, a high prevalence of Shigella flexneri was documented in the gibbon population at the National Zoological Park. The enzootic strain had been determined to be resistant to most antibiotics and had been responsible for animal deaths. Prevalence of Shigella spp was high in the entire primate population. To address the clinical problem, eradication was attempted by use of enrofloxacin. Facilities disinfection comprised a large part of Shigella clearance efforts. After the clearance protocol was completed gibbons, vs other primate species, were closely monitored for 16 months by subjecting…

Shigella antigens can be detected in the excreta of convalescents after dysentery for a long time. Most frequently these antigens occur in feces, less frequently in urine and rarely in saliva. According to indirect data, S. flexneri 1-6 antigens can be detected in excreta for a longer period after convalescence than S. sonnei antigens. When antigen indication is used for the diagnosis of dysentery and epidemiological analysis is carried out, one should bear in mind the length of the agent persistence in the body, related to Shigella type.

To study the secretory immune response after Shigella infection, the anti-lipopolysaccharide and anti-Shiga-toxin response in saliva, obtained from children with confirmed shigellosis and healthy children, were determined by enzyme-linked immunosorbent assay and by Western blot. Children with infection showed high titers compared to healthy controls. After Shigella dysenteriae type 1 infection a significant change in titer could be observed in a large number of cases, in contrast to Shigella flexneri infection. It appeared that, in children living in endemic areas, infection with one serotype …

The Wellcolex Colour Salmonella and Shigella tests are rapid latex agglutination procedures for grouping Salmonella and Shigella using an antibody attached to multicolored latex particles. The tests correctly grouped 46 (100%) of 46 Salmonella clinical isolates in groups A through E and G, 42 (100%) of 42 Shigella clinical isolates, and seven (88%) of eight Shigella stock cultures. The stock culture missed had been passed through many transfers and may have lost some of its antigenicity. The Wellcolex Colour Salmonella test was also found useful in detecting and grouping Salmonella spp. direct…

To determine the role of bacterial and host factors in the pathogenesis of shigellemia, blood and fecal Shigella isolates were compared for serum resistance and siderophore production, and shigellemic patients were examined for decreased serum bactericidal activity or increased serum transferrin saturation compared to control patients with non-bacteremic shigellosis. The majority of both blood (36/38) and fecal (36/48) Shigella isolates were sensitive to normal serum (greater than 2 log kill/60 min). Shigella dysenteriae type 1 strains were the most sensitive, and Shigella sonnei strains were …

Fifty-five patients with the diagnosis of shigella gastroenteritis were studied, 29 (52.7%) of whom had convulsions. Various symptoms and signs were compared between patients with and without convulsions to define risk factors for the development of seizures. To have a younger age and high body temperature were important predisposing factors. Hyponatremia and type of shigella organism were not contributing factors in the development of seizures.

A panel of 10 mouse and rat monoclonal antibodies specific for different type- and group-specific O-antigenic determinants of Shigella flexneri lipopolysaccharide was used to serotype 240 isolates of S. flexneri from Bangladesh. Three immunoglobulin M antibodies were used in a direct slide agglutination test; seven immunoglobulin G antibodies were absorbed to Staphylococcus aureus and used in a coagglutination assay. All but 13 of the isolates could be serotyped by using the monclonal antibodies. The six most common serotypes were (in descending order) 2a, 2b, Y (E1037), 1a, 3a, and 1b and acc…

Bacterial dysentery due to Shigella sonnei remains a serious public health problem in developed countries, including Bulgaria. At the National Shigella Reference Laboratory in Sofia, 17,126 strains of S. sonnei from epidemics and sporadic cases collected from 1973 to 1987 were studied. Antibiotic susceptibility testing, phage typing, colicin typing, and biotyping were performed for all strains to allow intraspecies differentiation and to track any clonal distribution. Of all strains, 84.3% were resistant to one or more antimicrobials, the most frequent being tetracycline (Tet), streptomycin (S…
